Le psoriasis concerne environ 3 % de la population. En effet, plus de 300.000 personnes en Belgique sont atteintes à des degrés divers. Elles se répartissent d'une manière égale entre les deux sexes, présentant des expressions cliniques différentes pouvant apparaître à tout âge.

Lorsqu'il est réalisé, il montre une hyperkératose avec parakératose et une acanthose de l'épiderme liée à une prolifération excessive des kératinocytes. En outre, l'épiderme est le siège de micro-abcès à polynucléaires (micro-abcès de Munro-Sabouraud). Dans le derme existe un infiltrat lymphocytaire T-et un grand développement capillaire avec allongement des papilles dermiques. Les localisations habituelles, très caractéristiques de l'affection, sont surtout les zones exposées aux contacts : coudes et bord cubital des avant-bras, genoux, région lombo-sacrée, cuir chevelu, ongles. Le psoriasis du cuir chevelu, classiquement non alopéciant, peut réaliser des plaques circonscrites, de taille variable, arrondies, bien limitées, couvertes de larges squames traversés par les cheveux ou bien former une véritable carapace recouvrant la totalité du cuir chevelu. La localisation occipitale est fréquente. Dans la région antérieure, à la lisière du cuir chevelu, les lésions sont souvent très inflammatoires et réalisent une couronne séborrhéique.

Psoriasis är en vanlig sjukdom med varierande svårighetsgrad och klinisk bild. Olika terapiförslag presenteras med hänsyn till dessa faktorer. Psoriasis är en ärftlig sjukdom med en inflammatorisk och fjällande reaktion i huden. Sjukdomen läker sällan spontant. Den inflammatoriska reaktionen kan utlösas av en halsinfektion av streptokocker eller av en skada i huden (Köbnerfenomen). Psoriasis kan i intensitet och utbredning variera starkt mellan individer men förblir ofta konstant hos en och samma individ. Debutåldern för sjukdomen kan variera från tidigaste barndom till hög ålder. Den vanligaste debutåldern är dock i tonåren. Sjukdomen psoriasis förekommer hos nästan 3% hos befolkningen. Den är ovanlig hos barn men förekommer i åtminstone 5% hos människor över 50 års ålder. Ärftligheten är autosomalt recessiv, det vill säga det fordras ett anlag från vardera föräldern. Trots den recessiva ärftligheten förekommer den ej sällan hos föräldrarna eftersom sjukdomen är så vanlig. Psoriasis är emellertid ingen enhetlig sjukdom, utan kan genetiskt betraktas som flera sjukdomar som idag inte kliniskt kan skiljas åt. Risken för barn att någon gång i livet få psoriasis anges i Tabell I. Möjligheten att lokalisera psoriasisgener med kopplingsanalys föreligger trots den ovan nämnda heterogeniteten. Hittills har två genloci för psoriasis upptäckts men fynden har ännu ej kunnat verifieras.
