When a defect in the immune system is inherited (carried through the genes) or occurs without a known underlying cause (such as HIV or due to medical treatment for cancer), it is called primary or inherited immune deficiency. PID includes over fifty known disorders that affect the cells, tissues, and proteins of a persons immune system. The immune system is the body's defense mechanism that prevents infections from invading a person's body. When a person has primary immunodeficiency, one or more of the four parts of the immune system (B-lymphocyte, T-lymphocyte, Phagocyte, and Complement systems) are missing or not functioning properly. Thus, the body is more vulnerable to becoming ill because the immune system is unable to fight off infections. Individuals may develop PID at any stage in their life. Patients with PID may suffer from more severe illnesses than normal, but they more commonly suffer from chronic and or frequent recurrences of a particular illness and longer duration of the illness due to the weaker immune system. Children may have other medical disorders associated with their PID that also create symptoms, i.e., asthma, autoimmune, and gastrointestinal complaints. Though the primary immune deficiency diseases can be serious, they are rarely fatal and can generally be controlled.

Some deficiencies are deadly, while some are mild. But they all have one thing in common: they may open the door to multiple infections. In the more severe forms of PI, germs which cause only mild infections in people with healthy immune systems may cause severe or life-threatening infections. Although infections are the hallmark of PIs, they are not always the only health problem, or even the main one. Some PIs are associated with other immune system disorders, such as anemia, arthritis, or autoimmune diseases. Other PIs involve more than the immune system; some, for instance, are associated with symptoms involving the heart, digestive tract, or the nervous system. Some PIs retard growth and increase the risk of cancer. Today, thanks to rapid advances in medicine, many PI diseases can be successfully treated or even cured. With proper treatment, most people with PIs are not only surviving once-deadly diseases, they are usually able to lead normal lives. Children usually can go to school, mix with playmates, and take part in sports. Most adults with PI are leading productive lives in their communities.

"Until now, a relative scarcity of study subjects has limited a researcher's options for investigating these lesser known diseases," says Anthony S. Fauci, M.D., director of the NIAID.
