Peart 203-432-1326 #4 Embargoed For Release: 4 p.m. EST July 10, 2001 New Haven, Conn. - Like drug and alcohol abuse, pathological gambling should be viewed as a chronic medical condition, Yale researchers assert in an article published in the July 11 issue of the Journal of the American Medical Association (JAMA) "Pathological gambling should be conceptualized as a chronic medical illness so that we can gain better understanding of the biological, genetic and environmental factors that contribute to the problem, develop better treatment strategies and treat individuals with the disorder within a medical context," said principal investigator, Marc Potenza, M.D., assistant professor of psychiatry at Yale. Potenza, who is also director of the Problem Gambling Clinic and the Women and Addictive Disorders Core of Women's Health Research at Yale, said that as a nation, America has seen a robust increase in gambling. Studies show that with the rapid increase and availability of legalized gambling, there are a growing number of problem and pathological gamblers. In 1998, 86 percent of the general adult population was estimated to have gambled at some time in their lives, up from 68 percent in 1975. Traditional forms of gambling include wagering in casinos or on lotteries or horse racing. Potenza said new gambling forms, such as video poker and Internet gambling, may be more habit-forming than traditional gambling, given their greater rapidity of action and ability to be used in isolation.

The tangled interplay between mood disorders and substance abuse often presents significant diagnostic and treatment challenges and is found to result in greater severity of symptoms and poorer treatment outcomes. There is also a relatively high frequency of comorbid occurrence of pathological gambling, substance abuse and mood disorders. This is not surprising given the frequent coupling of gambling and alcohol within gaming facilities. McCormick and Richard (1993) assessed 171 substance abusers for their level of gambling behaviour. Of this group, 87% had no significant problem, 7.2% were found to have a probable problem, and 5.8% had a severe gambling problem. Substance abusers with a gambling problem scored higher on measures of negative affect including depression, anxiety, guilt, and anger and have higher rates of impulsivity, and disinhibition of aggressive/ hostile responses. As a group, they use significantly more escape - avoidance and confrontive coping strategies more often overuse avoidant coping styles to deal with depression, anger and anxiety and have more negative situations to cope with in their lives. Compared to other substance abusers problem gamblers were also found to have higher levels of substance abuse and tended to abuse more substances . In McCormick's study (1994) of coping skill enhancement of pathological gamblers he found that problem gamblers with comorbid substance abuse problems use significantly more escape, avoidance and confrontational coping strategies. This subset of problem gamblers are more apt to overuse avoidance coping strategies to deal with depression, anger, anxiety and other negative emotional states increasing the risk of triggering gambling and substance use .

A phase II study in patients suffering from pathological gambling demonstrated that nalmefene reduced gambling related urges and behavior statistically significantly compared to placebo. The multi-center, placebo controlled study of 200 patients in the U.S. focused on the safety and efficacy of daily oral administration of nalmefene in patients suffering from pathological gambling. A psychometric scale (PG-YBOCS, developed by Drs. Hollander and DeCaria at Mount Sinai Hospital, New York) measuring gambling-related thoughts, urges and behavior was used for primary evaluation of efficacy. Based on the study results nalmefene was effective in patients suffering from pathological gambling: after four months treatment, mean scores on the PG-YBOCS scale were almost twice as high in patients who were on placebo when compared to the patients receiving nalmefene. The difference between the study groups was statistically significant. No serious adverse effects related to the use of nalmefene were observed during the study. The company is now seeking a licensing partner for this drug development program.

Gamblers Anonymous which, like its sister project Alcoholics Anonymous, is a 12 step programme involving individual abstinence reinforced by group discussion and support. Since the advent of the National Lottery calls to Gamblers Anonymous have increased by 17%. Counselling The charity Gamcare have a success rate of 53% with their group counselling sessions. Due to their encouragement of follow-up sessions, they can claim a 46% success rate six months after the initial counselling course is finished.
