Pathologic gambling and problem gambling affect approximately 5 to 15 million Americans and are common in young people. The community-minded family physician is in a good position to identify and assist patients who have gambling-related problems and thereby prevent or treat the resultant personal, family and social disruptions. Provider and community education about the depth and breadth of this condition is crucial for the identification and treatment of a growing problem. As with many psychologic conditions, identification of the disorder and treatment of the patient by the family physician comprise the primary treatment. Screening tools, treatment programs and self-help groups provide additional resources for the family physician. An illustrative case report demonstrates the importance of heightened awareness of and screening for this common condition. (Am Fam Physician 2000;61:741-9.) Pathologic gambling is identified in every social class. Unfortunately, no systematic process of educating, screening and treating pathologic gamblers is currently in place. Family physicians need to have a heightened awareness about the impact of the pathologic gambler's behavior on the family and should be familiar with screening instruments and treatment options.

Many mental health professionals have limited information regarding the etiology, phenomenology and treatment of this patient population. Method: Computerized literature search using PsycLIT and MEDLINE for the years 1984 to 1999. Results and conclusion: There are several competing conceptualizations of pathological gambling: as an impulse control disorder, a mood disorder, an obsessive-compulsive spectrum disorder, or a non-pharmacological addiction. An alternative model of pathological gambling is as a heterogeneous disorder with different subtypes sharing certain characteristics. Both biological and psychological factors play a role in the etiology of pathological gambling. Treatment options include several medications, psychotherapies and attendance to Gamblers Anonymous, although none of them are established treatments. Extractions: The study involved telephone interviews with a representative sample of 1044 persons selected for gender, age and profession drawn from the entire population over the age 18, and face-to-face interviews with a proportional sample of 1000 casino clients. The interview questionnaire contained socio-demographic items, a series of questions relating to all types of gambling behaviour, the internationally validated SOGS scale, which is derived from the DSM-II diagnostic criteria, and the three questions taken from the gamblers Anonymous questionnaire. Analysis of the results of the study shows that half of the adult population of Ticino gamble at least once a year, notably on the state Lottery (47%), and 6% visited a casino in the year prior to the survey. While 98.8% of Ticino residents do not appear to have gambling problems, 0.6% are likely to be problem gamblers and another 6% pathological gamblers (making a total of round 3,000 problem and pathological gamblers among the resident adult population of the canton). This prevalence rate is at the lower bound of those observed in studies in studies conducted in other countries.
