Plasma cells are normally present in the bone marrow (making up less than 5% of the bone marrow) and are responsible for the production of antibodies when the body is dealing with infection. Cancerous plasma cells (myeloma cells) build up in the bone marrow, interfering with normal immune response and bone marrow production of normal blood cells. These myeloma cells invade and damage the bone, causing tumors (myelomas) to form. Myeloma cells then travel to other sites in the body, causing further tumors (hence the name "multiple myeloma"). Common features or symptoms of multiple myeloma are bone pain (especially with movement), anemia, fatigue, hypercalcemia, and increased incidence of infections (such as bladder or kidney infections, skin infections, pneumonia, or sinusitis).

During the admissions history and physical the patient reported a one week history of left posterior pleuritic chest pain that felt better when he rubbed the area with his hand. He had dyspnea on exertion but not at rest. He denied cough, fever, or hemoptysis. He also reported an episode of left lower extremity pain that began 3 weeks ago. The pain has decreased but the swelling has persisted. His arterial blood gases on admission were PH 7.45 Pco2 33 Po2 64 He had a low grade temperature 37.9 The chest x-ray showed a pleural based density in the left costophrenic angle and a linear density in the posterior aspect of the left lower lobe. The perfusion scan showed an area of decreased perfusion in the posterior aspect of the left lower lobe that matched the area of infiltrate in the left lower lobe on the chest radiograph. The ventilation scan showed decreased ventilation in the posterior aspect of the left lower lobe.

Multiple myeloma is a cancer of plasma cells. Plasma cells are white blood cells made in the bone marrow, which is the spongy tissue inside bones. Normal plasma cells make antibodies to fight infection. In multiple myeloma, abnormal plasma cells grow rapidly and uncontrollably to form tumors in bone. They make abnormal antibodies, which are not useful, and the tumors interfere with the growth and function of normal cells made by the bone marrow. This results in anemia and lowered resistance to infection. Fewer blood platelets are produced, so your blood clots more slowly than normal. The uncontrolled production of plasma cells in the bone marrow also causes pain in the bones. Bone tissue may be destroyed, and the bones may break more easily. Calcium from destroyed bone tissue can accumulate in the blood and damage the kidneys. Collapsed bones in the spine may compress nerves, causing numbness or paralysis. If it is not treated, multiple myeloma can spread throughout your body and be fatal.
