Norberto, MD, and Raphael Bueno, MD Malignant pleural mesotheliomas are locally aggressive, invasive, and almost universally fatal. Background: Diffuse malignant pleural mesotheliomas (DMPMs) are highly lethal tumors that are becoming more common. Standard management approaches have provided limited effectiveness. Conclusions: Nodal status and histologic subtype are major predictors for survival in patients with early DMPM. A uniformly accepted staging system would allow comparison of treatment approaches from various institutions. More effective management interventions are required. Mesotheliomas of the pleural cavity are relatively rare tumors. Generally, two types of pleural tumors can be referred to as mesotheliomas. The less common is the solitary (or localized) fibrous tumor of the pleura, previously known as "benign mesothelioma." This slow-growing, commonly benign, well-circumscribed tumor is pedunculated on a pleural-based pedicle and often is cured by resection. The tumor appears to originate from submesothelial rather than mesothelial or epithelial cells.

Extractions: Background Mesotheliomas form a spectrum of tumors that usually arise in the pleura or in the abdominal cavity. The malignant tumors arising within the pleura are strongly associated with prior asbestos exposure in up to 90% of cases. The time period from initial exposure to development of the cancer may range from 25-45 years and the lifetime risk of developing mesothelioma following heavy exposure ranges from 7-10%. It diffusely spreads through the pleural space, directly invading other thoracic structures and associated with pleural effusion. The underlying lung is usually encased within the tumor. The abdominal peritoneal variant is also related to asbestos exposure and 50% of these patients may have pulmonary fibrosis. Although 50% of these cases may be limited to the abdominal cavity, intestinal obstruction is common frequently leading to death. The classic histopathology of the mesothelioma is a biphasic tumor with both an epithelioid and sarcomatoid or spindle cell component. From this description, it can be readily appreciated how the diagnosis may be very difficult to confirm. The table below lists the major histologic variants of the tumor. The task of the pathologist is to separate the mesothelioma from its many mimics including adenocarcinoma from the lung (in pleural cases) and soft tissue sarcomas such as malignant fibrous histiocytoma (in peritoneal cases). Immunoperoxidase studies and electron microscopy may aid in this distinction.

Mesothelioma is one form of cancer found in the chest which has been shown to have a direct link to previous exposure to asbestos. Patients with this diagnosis have very poor chances of cure. Chemotherapy, radiation, or surgical treatment each used alone have not improved survival. A combination of surgery followed by chemotherapy and radiation has shown only minimal improvement to this outcome. Chemotherapy given pre-operatively has been shown to increase the chances for survival in patients who have a diagnosis of lung cancer and esophageal cancer. Hopkins researchers in the Thoracic Oncology Program feel that this approach will yield similar results for cases of mesothelioma. Dr. Yang, Dr. Ettinger, and Dr. Gabrielson are working to offer better treatment to patients with this diagnosis by specifically targeting the genetic material in mesothelioma cells.
