Extractions: About PDQ Malignant mesothelioma, a rare form of cancer, is a disease in which cancer (malignant) cells are found in the sac lining the chest (the pleura) or abdomen (the peritoneum). Most people with malignant mesothelioma have worked on jobs where they breathed asbestos. A doctor should be seen if a person has shortness of breath, pain in the chest, or pain or swelling in the abdomen. If there are symptoms, the doctor may order an x-ray of the chest or abdomen. The doctor may look inside the chest cavity with a special instrument called a thoracoscope. A cut will be made through the chest wall and the thoracoscope will be put into the chest between two ribs. This test, called thoracoscopy, is usually done in the hospital. Before the test, the patient will be given a local anesthetic (a drug that causes a loss of feeling for a short period of time). Some pressure may be felt, but usually there is no pain. The doctor may also look inside the abdomen (peritoneoscopy) with a special tool called a peritoneoscope. The peritoneoscope is put into an opening made in the abdomen. This test is also usually done in the hospital. Before the test is done, a local anesthetic will be given.

Various surgical procedures may be possible in selected patients, providing long-term survival without cure. In large retrospective series of pleural mesothelioma patients, important prognostic factors were found to be stage, age, performance status, and histology. [ ] For patients treated with aggressive surgical approaches, factors associated with improved long-term survival include epithelial histology, negative lymph nodes, and negative surgical margins. [ ] For those patients treated with aggressive surgical approaches, nodal status is an important prognostic factor. [ ] Median survival for malignant local pleural disease has been reported as 16 months and extensive disease as 5 months. In some instances the tumor grows through the diaphragm making the site of origin difficult to assess. Cautious interpretation of treatment results in this disease is imperative because of the selection differences among series. Effusions, both pleural and peritoneal, represent major symptomatic problems for at least two thirds of the patients. A history of asbestos exposure is reported in about 70% to 80% of all cases of mesothelioma. [

Mesothelioma from the (Greek meso+ thelioma, tumor of middle lining tissue) is an uncommon cancer, originating from the cells which form the membrane lining the abdominal cavity (peritoneal membrane or peritoneum) which houses the intestines, or the chest (pleural membrane or pleura) cavity housing the heart and lungs, in which the cells making up those tissues begin to grow out of control. Mesotheliomas involving the lung and pleura characteristically present as progressive shortness of breath due to the thickening of the lining membrane of the lung with gradual contraction of the breathing space; often, fluid accumulates in the lung spaces as well, further interfering with breathing, Mesotheliomas involving the abdominal cavity present with digestive symptoms, and abdominal swelling due to thickening of the lining membranes of the gut, and accumulation of large amounts of fluid in the abdomen.

Extractions: Members Mesotheliomas are uncommon tumours arising from the mesothelial surface (pleura, peritoneum, and rarely from sites such as the pericardium and tunica vaginalis testis). Diffuse mesotheliomas are invariably malignant. At the time of presentation, pleural mesothelioma usually is often confined to the thorax, however, disease typically contaminates the entire pleural cavity and pleural effusion is common. Later, there is extension to the chest wall, mediastinum and encasement of the lung with a thick rind of tumour. Regional lymph nodes may become involved. Extension through the diaphragm may cause peritoneal involvement and ascites. Hematogenous metastases are uncommon but may be seen particularly in patients surviving longer than average. Patients may present with chest pain, dyspnea and cough. The referring clinical problem may be pleural effusion of unknown etiology. Advancing disease causes respiratory insufficiency, chest wall pain, cardiovascular complications from invasion of the heart and general constitutional deterioration. (Revised May 2001)
