Patrick Mauney, needed either a well-heeled or well-funded volunteer if he wanted to fulfill a promise made during the presiding bishop's visit in January. The Rev. Hope Imoroa in the Diocese of Warri suffers from Meige's syndrome, a rare disorder that causes spasm of the eyelid and facial nerves so that the eyes are constantly shut tight. Imoroa is functionally blind. There is a treatment and it involves a risky, delicate operation and one of the most expensive drugs in the world: Botox (botulinum toxin), the drug so loved by wealthy Hollywood stay-youngs. The procedure requires specialized expertise because, unlike the injections given by neurologists and dermatologists to combat wrinkles, the area of the eyelid is more fragile. The lids need to be paralyzed, but only partially, by injecting the drug into different areas. There is risk of piercing the eyeball and if the drug is placed even a few millimeters away from the target, the lid could be paralyzed shut. No more than a few dozen neurologists or ophthalmologists in the United States have extensive experience with the procedure. Most see no more than one patient with Meige's syndrome in their lifetimes. Not one doctor in Nigeria has experience with the procedure.

It has been revised substantially to include up-to-date clinical information for physicians treating patients with this complex, frequently misunderstood neurobehavioral (neurobiological) disorder. Tics are involuntary, rapid, repetitive and stereotyped movements of individual muscle groups. They are more easily recognized than precisely defined. Tic disorders are generally categorized according to age of onset, duration of symptoms, severity of symptoms and the presence of vocal and/or motor tics. Transient tic disorders often begin during the early school years and can occur in up to 18% of all children. Common tics include eye blinking, nose puckering, grimacing and squinting. Transient vocalizations are less common and include various throat sounds, humming or other noises. Childhood tics may be bizarre - palm licking, poking and/or pinching the genitals are examples. Transient tics last only a few weeks or months and are usually not associated with specific behavioral or school problems. They are especially noticeable during times of heightened excitement or fatigue. As with all tic syndromes, boys are three to four times more often affected than girls. While transient tics by definition do not persist for more than a year, it is not uncommon for a child to have recurrent episodes of transient tics over the course of several years.

Not all dry eye symptoms are due to a dry eye and dry symptoms and findings do not necessarily worsen over time, and in fact may improve. The symptoms are always the same a sandy, gritty, burning feeling. Unfortunately, the causes of the dry eye are not. There are many different aetiologies of KCS. All too often we associate dry eye symptoms with a disorder of the tear film or tear secretion. Of the 25 patients, 7 patients (28%) produced marked and lasting improvement with sulpiride and 12 patients (48%) showed mild or transient improvement. This favorable therapeutic response to sulpiride suggests that striatal glutamate underactivity may play a role in the pathophysiology of Meige's syndrome as a primary or secondary defect.
