Cysts generally develop in weak segments of the tubules that carry urine from the glomeruli. The cyst's growth displaces healthy kidney tissue. The kidneys expand to accommodate the cyst, which can weigh as much as 20 pounds. Three factors determine cyst classification: its cause (acquired, inherited), its features (complicated, simple, multiple, single), and its location (outer [cortical] or inner [medullary] kidney tissue).

Sacs of fluid (called cysts) grow in the kidneys. If too many cysts grow or if they get too big, the kidneys become damaged. The cysts may also cause pain or may get infected. PKD is the most common inherited disease in the United States. Children of parents with PKD have a 50% chance of getting the disease. Return to top Most people with PKD can lead a normal life. In many people, the disease is mild and causes only minor problems. PKD is more severe in some patients and can cause kidney failure. About 60% of patients develop high blood pressure, which can be treated with blood pressure medicine. About 50% of patients with PKD have kidney failure by age 60. Dialysis (blood filtering) and kidney transplants are both effective treatments for kidney failure. PKD is generally worse in men, blacks and patients with sickle cell disease. Kidney disease is not limited to any particular age or breed, but is one of the most common medical problems of older dogs and is a leading cause of death. Depending on the study, kidney failure is the second or the third leading cause of death in dogs after heart disease and/or cancer. Furthermore, autopsies done on older dogs reveal abnormalities in as many as 85% of all kidneys examined in dogs over the age of five. Kidney disease may have many different causes, including inherited defects, infections and toxic substances. Frequently, the cause cannot be determined, but with careful examination and testing, the severity of the disease can be evaluated and a treatment protocol can be established. The kidneys have great reserve capacity, but if kidney disease destroys the kidney's ability to perform properly, waste materials accumulate in the blood and signs of actual kidney failure are likely to appear. Extractions: The human kidney is part of the body's efficient waste disposal system. A healthy kidney cleans the blood by filtering out waste products which are then routed to the urine. Over time, high blood sugars can damage the cells and tiny blood vessels that perform this cleansing. The result is a damaged kidney that routes waste back into the body and releases excess amounts of protein into the urine. Symptoms of kidney failure are fatigue, decreased appetite, nausea and vomiting. About 30 to 40 percent of people with Type I diabetes, and 20 to 30 percent of those with Type 2 diabetes, will develop moderate to advanced kidney disease. However, since the damage occurs slowly over time, when action is taken early there is time to prevent the worst of it. Prevention In the DCCT, the intensive control group (the group that successfully controlled their blood sugars) experienced up to 54 percent less kidney disease compared to the moderate control group (which had less control). So, once again, the best way to prevent kidney disease is to keep blood sugars as normal as possible. Remember that this reduction in kidney damage is just due to improved blood sugar control in those with early kidney disease. Other protective measures that are more powerful with later kidney disease are discussed in Chapter 30.
