Extractions: Introduction and background. Epstein-Barr virus (EBV) is a ubiquitous human herpesvirus that infects nearly every human being by young adulthood. Most EBV infections occur asymptomatically during childhood, but EBV is associated with a wide variety of human diseases including the infectious mononucleosis syndrome, Hodgkin's lymphoma, African Burkitt's lymphoma and nasopharyngeal carcinoma. EBV is also an opportunistic pathogen in immunocompromised patients, associated with lymphoproliferative diseases and oral hairy leukoplakia. EBV infection is transmitted from person to person by contact with infectious body fluids. Oral contact with infectious saliva is the most common route of transmission, but EBV infection may also be transmitted by sexual contact or exposure to breast milk. Like all herpesviruses, EBV establishes a life-long, persistent infection of its host.

In an otherwise asymptomatic individual, recognition of oral lesions, such as oral candidiasis or oral hairy leukoplakia, may give the first indication of HIV disease. For patients who have documented HIV infection, NIH clinical guidelines suggest that oral examinations should be performed by the primary care provider during every physical examination. Routine oral examinations should include careful inspection of the soft tissues, with particular attention paid to the soft palate and lateral margins of the tongue. Oral hairy leukoplakia (OHL) is a benign oral lesion caused by Epstein Barr virus (EBV). Recognition of OHL in a previously healthy patient should prompt HIV testing. This lesion may be present in more than 50% of HIV-positive homosexuals and bisexuals in the United States. Often, the appearance of this lesion is a harbinger of progressive disease; the average CD4+ lymphocyte count is typically 400 at the time of diagnosis of OHL. Clinically, OHL appears as asymptomatic, hyperplastic, verrucous, white to grayish epithelial plaques on lateral tongue. The patient may report that the lesion changes from day to day, but does not rub off.

Methods: This cross-sectional study evaluated 631 adult dentate HIV-1 seropositive persons (race: White/ Black only) examined for OC and OHL from 1995 - 2000 at the University of North Carolina Hospitals in Chapel Hill, NC. Data collected from medical record review, interview questionnaires and clinical examinations were analyzed using chi-sq tests, t-tests, and ANOVA.
