Glaucoma therefore is a disease of the optic nerve, the nerve bundle which carries images from the eyes retina to the brain. The eyeball is basically a rigid sphere filled with fluid. As the total amount of fluid within the eye increases, so does the pressure, similar to over inflating a tire. Many people know that glaucoma has something to do with increased pressure inside the eye. The higher the pressure inside the eye, the greater the chance of damage to the optic nerve. Glaucoma is said to be one of the leading causes of blindness in people over the age of 40. It affects approximately 2% of that population. Loss of peripheral (vision off to the side) is the earliest symptom. Left untreated the field of vision will continue to narrow leading to tunnel vision. If detected early, loss of vision can most often be prevented.     Extractions: Think of your eye as a fountain, with fluid constantly circulating to nourish the tissues and maintain the eyeball's round shape. Glaucoma is a "clogged drain" in this delicate system. In chronic open-angle glaucoma (by far the most common kind), the fluid drains away too slowly (figure 1), which raises pressure within the eyeball and eventually damages the optic nerve. In acute angle-closure glaucoma (a less common type but a real medical emergency), drainage becomes completely blocked and pressure builds up very quickly. What are the signs of glaucoma? Chronic glaucoma progresses very slowly. Often, the first sign of trouble is reduced side vision. Other indicators may include blurred vision, seeing colored halos around lights, adjustment problems on entering a dark room, and repeated difficulties that new eyeglass prescriptions do not help. But as Dr Podolsky emphasizes, primary care physicians can help reduce ocular damage by identifying high-risk patients and recognizing optic nerve changes at an early stage of disease. This article focuses on diagnosis and treatment of the primary types of disease, open-angle glaucoma and angle-closure glaucoma. G laucoma is a significant cause of blindness world-wide. It is the second leading cause of blindness in the United States and possibly the leading cause among African Americans (1). In addition, glaucoma is prevalent in nearly 3% to 4% of the population over age 70. Early detection and treatment of glaucoma are essential in preventing loss of vision and blindness. Definitive diagnosis and treatment are the domain of ophthalmologists, but primary care physicians play an essential role in screening patients through careful history taking and physical examination and in identifying those at high risk. In ancient times, the term "glaucoma" referred to a broad group of blinding diseases. By the 1700s, elevation of intraocular pressure, detected by palpation of the globe, was known to be a distinct ocular disease. When the ophthalmoscope was introduced in 1854, glaucoma was redefined as diseases of the eye characterized by optic nerve damage and elevated intraocular pressure. The distinctly different mechanisms of open-angle glaucoma and angle-closure glaucoma were not revealed until the 1930s, when reliable gonioscopy allowed examination of the angle of the anterior chamber of the eye (2).

The fluid which fills the eye is constantly being produced, circulates around the lens, and exits the eye through the iridocorneal angle (see drawing). The balance between fluid production and outflow results in normal intraocular pressure between 10-30 mm Hg. Elevation in intraocular pressure occurs as a result of poor fluid outflow. Glaucoma most commonly occurs as a primary inherited disease in many breeds of dogs particularly the Cocker spaniel, Basset hound and Chow. Inadequate fluid outflow may also occur from secondary causes such as dislocation of the lens, trauma, inflammation, or tumors. In these cases, treatment will be directed at the primary disorder. The prognosis for vision in an eye with glaucoma depends on the degree and duration of elevated pressure. Irreversible damage to the retina and optic nerve may occur within a day or two if the pressure is markedly elevated. If only one eye is involved, your pet may compensate for his loss of vision until permanent damage has occurred in the involved eye. Since glaucoma will often ultimately affect both eyes, evaluation of the "normal" eye is very important. Gonioscopy is an examination of the iridocorneal angle that will assist in this evaluation. Treatment for glaucoma depends on whether permanent vision loss has occurred. In early cases, medications may be given to lower the pressure immediately. However, rarely is medication alone successful in the long-term. In most cases, surgery offers the best opportunity to restore and maintain vision. Laser cyclophotocoagulation is a relatively new technique which uses a diode laser to burn small areas in the tissue in the eye (ciliary body) which produces fluid so that less fluid is produced. This procedure has the advantages of being relatively quick with minimal post-operative discomfort. Results are variable though and occasionally, more than one procedure may be required. Some cases require a combination of medical and surgical management and all cases require close clinical supervision. You should watch for signs of loss of vision, pain, or a cloudy appearance to the eye.
