Support Us Human papilloma virus (HPV) is the most common sexually transmitted infection seen at sexual health clinics in the UK and it is likely that most of us have had HPV at some time in our lives - although we may not have known it. So what is HPV, how can it be prevented and treated, and what does papilloma mean anyway? Papilloma means a growth or wart, which is why HPV is also known as the wart virus. In practical terms, however, only about 10% of people with HPV develop warts. The vast majority of people with HPV don't, and are therefore unlikely to know they are infected or that they may be passing the virus to their sexual partner(s). Young sexually active people are at greatest risk of getting genital HPV, with the highest rate of infection in those between the ages of 20 and 24. Recent research suggests that many young women may become infected with the virus during their first sexual experience, and the risk of infection increases with the number of sexual partners. HPV prevalence seems to decrease with age.

They are common, with a million consultations annually in the USA, and over 50,000 newly diagnosed patients in the UK every year. Bandolier was asked whether a recent editorial [1] suggesting that self-administered treatments were effective was correct. How can anyone tell? This seemed like a good excuse for a swift search for evidence as an exemplar of how a rapid review might be helpful and how it could be performed by a healthcare professional for, say, a health authority or primary care group wanting to make policy. Two self-administered treatments were mentioned, imiquimod and podophyllotoxin creams. The way forward seemed to be to look for high quality and relevant trials. The ideal would be randomised double blind trials of self-administered treatment versus placebo in patients with properly diagnosed anogenital warts. Therefore the search used PubMed, the names of the treatments, and the subheading of randomised controlled trials. The Cochrane Library was also searched using a similar strategy. Searches revealed no previous systematic reviews.

Common skin warts of the hands or feet are caused by a similar virus, but they do not cause genital warts. HPV is a virus consisting of more than 70 different types. Certain types cause genital warts. Sometimes HPV will not cause any symptoms at all but is still capable of being transmitted because the virus (HPV) is present in the tissue of the genitals or rectum. This is called an "asymptomatic infection" . These warts appear on the penis and around the anus in men, and on any part of the external genitals - vaginal lips, perineum (area between the vagina and anus), and anus in women. They also occur inside the vagina and on the cervix. They may occur singly, or in groups. Sometimes they itch or burn, but often they have no symptoms at all. This type of wart is sexually transmitted. It is highly contagious. The incubation period - the amount of time between contact with a person who has condyloma and the time you notice condyloma - typically ranges from three weeks to eight months, with an average of three months. Condyloma may, however, appear years after a person has been exposed. Transmission Condyloma are caused by a virus called human papillomavirus (HPV). The virus is generally sexually transmitted in adults. Some people apparently can have the virus, not show any signs of condyloma and still transmit the virus to a partner, who may develop condyloma. Genital warts are the most common complaint diagnosed at sexually transmitted diseases clinics in the UK, accounting for around a fifth of all diagnoses in 1999. During the late 1990s, there was a rising incidence of genital warts in the UK, affecting both men and women, and in 1999 there were 70,000 cases. Though younger people tend to be more frequently affected, anyone who is sexually active may be exposed to the genital wart virus. Some forms of the genital wart virus can lead to cervical or anal cancer. This risk is higher in people with HIV because there is more chance that the wart virus will become re-activated due to loss of immunity. Transmission The genital wart virus can be contracted during unprotected anal, vaginal or oral sex. It can also be transmitted by close physical contact with the genital warts themselves, as these may shed the wart virus. Prevention Using a condom for anal, vaginal, or oral sex offers a degree of protection from infection with genital warts, or from passing on the virus to somebody else. People who are sexually active are advised to have regular sexual health check-ups, where genital warts can be diagnosed if present, and tests for other sexually transmitted infections performed. Most HIV treatment centres have an associated sexual health clinic where free and confidential treatment can be obtained without referral from your GP or HIV doctor.

We are unsure of how many women are infected with the virus, HPV, that causes genital warts. Some medical studies show that 1 out of every 10 women has HPV, while others reveal an incidence of up to 1 out of every 2 (50%)! HPV, short for Human Papilloma Virus, is a virus that can infect humans through sexual contact, or, less commonly, through prolonged exposure to such things as shared underwear or swimming clothes. There are over 70 different types of this virus. Like most viruses HPV can infect someone without that person knowing it, and can "set up shop" inside their body for many years before causing symptoms. In some lucky individuals, HPV is present in their tissues but never causes symptoms. (In these people sophisticated and expensive diagnostic tests can sometimes reveal the presence of HPV).
