    Extractions: Polycystic Ovary Syndrome (Stein-Leventhal Syndrome) Polycystic ovary syndrome (PCOS) is a disorder in which many benign cysts form on the ovaries under a thick, white covering. It is most common in women under 30 years old. The ovaries are glands located on either side of the uterus (womb) in a woman's lower abdomen. The ovaries produce the female hormones oestrogen and progesterone. Before menopause, they also produce eggs. Ovarian cysts are fluid-filled sacs that result from ovulation cycles. Many ovarian cysts go away without treatment. The most common cysts are just enlargements of normal egg follicles. This disorder has also been called Stein-Leventhal syndrome. Polycystic ovary syndrome is caused by an abnormal production of two hormones by the pituitary gland in the brain. These two hormones are LH (luteinising hormone) and FSH (follicle-stimulating hormone). Imbalance of these hormones prevents the ovaries from releasing an egg each month. The ovaries produce more of the male hormone testosterone. They continue to produce oestrogen but not progesterone. What are the symptoms?

The uterosacral ligaments are clearly seen as vertical bands of tissue since the uterus is being put on stretch. If you look closely, you can see black lesions of endometriosis near the top of each ligament. If you look even more closely, however, you can see that each ligament is involved by subtle white fibrosis indicating moderately invasive endometriosis in each ligament. Looking closer still, you can see small white islands of endometriosis near each ligament (one is indicated by an arrow). Surgeons looking only for "black powderburn" lesions would miss most of the disease in this pelvis. The patient would continue to hurt, and the doctor would think that she had a very "resistant" case of endometriosis. Endometriosis is a progressive disease affecting 5 to 10 percent of women. It can cause dyspareunia, dysmenorrhea, low back pain and infertility. A definitive diagnosis can be made only by means of laparoscopy. Medical treatment designed to interfere with ovulation generally provides effective pain relief, but the recurrence rate following cessation of therapy is high, and this type of treatment will not resolve infertility. Surgical treatment improves pregnancy rates and is the preferred initial treatment for infertility caused by endometriosis. Surgery also appears to provide better long-term pain relief than medical treatment. Bilateral oophorectomy and hysterectomy are treatment options for patients with intractable pain, if childbearing is no longer desired. (Am Fam Physician 1999;60:1753-68.) E ndometriosis is characterized by the presence of endometrial tissue on the ovaries, fallopian tubes or other abnormal sites, causing pain or infertility. The disease tends to progress under the repetitive influence of the menstrual cycle. Interrupting or decreasing menstruation is the mainstay of medical therapy. The goal of surgery is to remove endometrial lesions. Endometriosis is likely to remain problematic as long as menstruation persists. Fortunately, symptoms can be modulated or alleviated with appropriate treatment.

An article on this topic is available in this issue of AFP What is endometriosis? The word "endometriosis" (say it this way: end-oh-me-tree-oh-sis) means a problem with the lining of the uterus. The lining is called the endometrium. Normally, every month when you have your period, the lining (which is also called endometrial tissue) comes out in your menstrual flow. Endometrial tissue is usually only inside your uterus. However, if you have endometriosis, tissue from the lining of the uterus moves through the fallopian tubes and gets on your ovaries, in your pelvis, on your bladder or in other areas. When you have your period, this tissue swells and bleeds, just like the lining of your uterus. This is often painful, and scar tissue can form in your pelvic area. Is endometriosis harmful to me?

I was diagnosed with PCOS (Polycystic Ovarian Syndrome) on the 18th of December, 1998. On hearing this, I soon started searching for information on PCOS through the Net. Of all the things I found, only ONE site was based on New Zealand. Diane-35. From there, you can get pamphlets on PCOS and Diane-35 sent to you (sorry, only sent around NZ at the moment it seems). The treatment I was on, was Diane-35. Which had been subsidised for 2 years for me. Sad thing is, I found out that it made my condition (lower abdomen pain and cramping) worse , so I had to stop taking it. After a gyno's visit, it was decided that I could be put on some other hormone treatment. Again, a failure because I had migraines as a withdrawl symptom from it. I can't remember the name of the drug, but it didn't do a thing for me.

    Extractions: Sixteen-year-old Anne had never thought much about her period . It could be a bit of a hassle at times, but it wasn't really a big deal. But this had changed over the past few months. Her periods had become very painful - so painful that she totally dreaded their arrival. Anne assumed that she was experiencing the normal menstrual cramps that many of her friends talked about. So every month she would curl up on the couch with a heating pad and take some ibuprofen for a few days. Sometimes she would have to miss a couple days of school, too. Anne and her mom became concerned, so they made an appointment with a gynecologist . After talking with Anne and examining her, the gynecologist diagnosed Anne with dysmenorrhea , which is simply the medical term for painful periods. The doctor prescribed an oral contraceptive, or birth control pill , to help with the pain (these pills moderate the menstrual cycle). Over time, though, Anne's pain got worse and even started happening between her periods. Sometimes she felt a sharp, stabbing sensation that got so bad she had to stop what she was doing and lie down. Anne's mom called the gynecologist again, and this time the doctor had a different theory: she thought Anne might have endometriosis. So she referred Anne to another doctor who specialized in treating this condition. It turned out Anne's doctor was right. What Is Endometriosis?
