    Extractions: To understand diverticulosis, it is helpful to know about the anatomy and function of the intestine. The small intestine is the long, thin segment of bowel that begins at the stomach and ends at the large intestine or colon. The colon starts in the right lower abdomen and forms a large question mark across the entire abdomen ending in the rectum. Just above the rectum is the sigmoid (S-shaped) part of the colon. Liquid stool enters the right colon and, as it moves through the colon, is dehydrated so that a formed stool eventually enters the rectum. The Sigmoid Colon Diverticulosis means that there are pockets or projections extending out from the colon. These occur very gradually over a long period of time. They occur along the natural weak points in the bowel wall. The pockets develop because of the pressure exerted within by the contracting colon. As noted earlier, since the sigmoid colon has the highest pressure in the colon, it is here that most diverticuli occur. Because of these balloon-like projections, the sigmoid often becomes thickened and narrowed. When this happens, significant changes in the bowel function can occur, such as discomfort, diarrhea, and/or constipation.

Diverticula Diverticula form when a small spot of the inside layer of the intestine pouches out through the muscular layer. The area of the colon most commonly affected is the sigmoid colon (the colon that is the last section before stool leaves your body) and the descending colon but can form anywhere in the large intestine. Once these pouches form they remain for life but frequently cause no problems. Diverticulitis If the diverticula become inflamed they cause the condition known as diverticulitis. If this happens, usually one or all of the following symptoms occur. Lower abdominal discomfort / pain Change in bowel habits such as diarrhea or constipation Bloating or flatulence = gas Pain in the lower left area of the abdomen. The symptoms usually last for several days and may have to be treated with antibiotics and pain medication if severe enough. Treatment Antibiotics are used to fight the infection that may be in the inflamed pouch and the pain medication is to provide comfort until the episode stops.

Signs and Symptoms The majority of people with diverticula are asymptomatic. Some have symptoms such as intermittent abdominal pain, bloating, excessive flatulence, irregular stools, nausea, anorexia, passage of pellet-like stools, and attacks of diarrhea. More severe symptoms include rectal bleeding. Diagnosis Diagnosis is most often made with barium enema and colonoscopy. Treatment The principle treatment revolves around a high fiber diet. The patient should avoid foods like seeds, nuts and popcorn. The use of bulk forming agents is helpful in adding to the 20 or 30 grams of fiber needed per day.

The formation of the pouches themselves is a relatively benign condition known as diverticulosis. The acute form can manifest itself with one or more severe attacks of infection and inflammation. In chronic diverticulitis, inflammation and infection may subside, but they may never clear up completely. The inflammation of diverticulitis can eventually result in a bowel obstruction, which is indicated by constipation, thin stools, diarrhea, abdominal distention, and abdominal pain. If the obstruction persists, abdominal pain and tenderness will increase, and you may experience nausea and vomiting.
