C. immitis grows as a mold in the soil. When the soil is disturbed, the mold hyphae fragment, forming structures that become airborne. If inhaled by animals or humans, they transform into thick-walled spheres that produce thousands of endospores, which are capable of reproducing. Epidemics of coccidioidomycosis have occurred following large-scale soil disturbances caused by dust storms, earthquakes, and excavations. In persons with competent immune systems, the majority of infections produce no symptoms or mild, self-limited upper respiratory tract illness. At least two-thirds of coccidioidal infections go unnoticed. Moderate to severe illness involving the lower respiratory tract takes several weeks to develop. Acute infections usually resolve without therapy within a few weeks or months. In 5-10% of persons, however the infection progresses to chronic pulmonary disease or disseminates via the blood or lymph circulation to the skin, bones, meninges, or other sites.

Extractions: Coccidioidomycosis (also known as Valley fever and California valley fever) is a fungal disease caused by Coccidioides immitis. It is endemic in certain parts of Arizona, California, Nevada, New Mexico, Texas, Utah and northwestern Mexico. C. immitis resides in the soil in certain parts of the southwestern United States, northern Mexico, and a few other areas in the Western Hemisphere. Infection is caused by inhalation of airborne, fungal particles. The disease is not transmitted from person to person. Symptomatic infection (40% of cases) usually presents as an influenza-like illness with fever, cough, headaches, rash, and myalgias. Some patients fail to recover and develop chronic pulmonary infection or widespread disseminated infection (affecting meninges, soft tissues, joints, and bone). Severe pulmonary disease may develop in HIV-infected persons. Extractions: This 53 y/o female with chest pain and arthralgia was referred for chest CT for evaluation of an abnormality seen on a chest x-ray. Selected images from the chest CT shown above demonstrate dense consolidation of the antero-medial right upper lobe. Repeat CT after 2 weeks of antifungal therapy demonstrates some clearing of the consolidation with early cavitation/mycoma formation. What you see isn't always what you get. The imaging key to this case is probably the blood vessels in the consolidated lung in the second image from the initial study. The history, in a patient who is a resident of Southern California, should also raise the suspicion of the diagnosis. The patient's symptoms and chest findings resolved after antifungal therapy.
