MACDOUGALL, University of Texas - San Antonio / Health Science Ctr, USA Cleidocranial Dysplasia (CCD) is an autosomal dominant disorder characterized by bone defects, supernumerary teeth, tooth hypoplasia and delayed root formation and tooth eruption. Mutations in the runt-related transcription factor 2 (RUNX2) are associated with this disorder. The molecular mechanisms that correlate RUNX2 mutations with the CCD dental phenotype, especially related to tooth eruption and root formation, remains obscure. Objective: To perform a broad survey of gene expression profiles in periodontal ligament (PDL) cells of CCD versus normal patients to identify critical RUNX2 down-stream target genes. Methods: Age matched normal and CCD PDL cells were established from primary explant cultures. Cells were characterized for cell proliferation, ALP activity, vonKossa staining and gene expression profiles. For genes differentially expressed samples were screened using an Affymetrix human genome U133 gene array. To confirm these results, quantitative real time PCR (QRT-PCR) was performed on selected transcripts. Finally gene promoter analysis was performed of potential RUNX2 target genes using the www.generegulation.com program. Results: Of the 39,000 transcripts tested by gene array, 5520 genes (14%) were up-regulated while 9462 (24%) were down-regulated in CCD as compared to normal PDL cells. The QRT-PCR data confirmed several of the significant differences. Key bone remodeling factors such as OPG increased 11.39 fold, while BMP 7 (1.13), BMP 6 (1.37), OSN (1.63), and RUNX2 (3.02) itself were down-regulated in CCD patients. Gene promoter analysis demonstrated at least one potential RUNX2 binding site for all putative genes analyzed.

Extractions: March/April , Volume 19, Issue 2 This patient report describes the treatment of a 42-year-old woman with cleidocranial dysplasia. Endosseous implants were used to restore the mandibular and maxillary arches with fixed prostheses. Six implants were placed in the mandible and immediately loaded with an acrylic resin fixed prosthesis. In the maxillary arch, 10 implants were submerged for 4 months prior to functional loading. A transitional denture was relined and placed in the maxilla 10 days after implant placement. Three months later, a definitive mandibular prosthesis was fabricated. The definitive maxillary restoration was delivered 6 months after surgery. The most recent follow-up, 6 months after delivery, confirmed a satisfactory treatment result to date.
