They describe the role of magnetic resonance imaging in such diagnosis as unclear, citing a 1988 comparative study of two cases as proof. We use magnetic resonance imaging as the preferred imaging technique in patients with suspected cerebral infections. It has improved resolution with multiplanar capability, and it is far more sensitive at detecting subtle white matter abnormalities as may be found in patients with herpes simplex encephalitis. The patient was admitted with fever, malaise and right shoulder pain. Soft tissue abscess was seen on ultrasound examination. It was presumed due to gram(+) microorganisms, so 4 g day (IV) ampicillin/sulbactam was started empirically once the abscess was drained. Nocardia asteroides was found in the pus specimen. On the second day in hospital, severe headache, ataxia and signs of meningeal irritation appeared. The cranial CT showed two intracranial abscesses in the frontal lobe and cerebellum. We assumed Nocardia asteroides was the infective agent for the cerebral abscesses, so antibiotic therapy was switched to trimethoprim-sulphamethox-asole (3x160/800 mg/d). Nausea and vomiting occurred on the fifth day of therapy, improving after drainage from the frontal abscess. However, these complaints recurred five days later. CT showed cerebellar abscess had become bigger. The patient's complaints improved after the second surgical drainage.

Extractions: History of Present Illness A 60-year-old woman was diagnosed with paucimmune glomerulonephritis by renal biopsy in July 2000. She was started on prednisone 30 mg daily in July and mycophenolate (Cellcept) was started in August. In November of 2000 she presented to her primary care physician with a right foot drop and right leg weakness. Her symptoms progressed to numbness of the right foot as well as the right arm and neck. She developed right-sided twitching without loss of consciousness and a CT scan of the brain showed multiple ring-enhancing lesions. She was transferred for further evaluation Top of page Past Medical History Top of page Medications Top of page Social History The patient lives in Delaware with her husband and works in a mentoring program in an elementary school. She does not drink alcohol, smoke, or use illicit drugs. There is no history of recent travel outside of Delaware and Maryland.

Extractions: A 19-year-old woman with congenital cyanotic heart disease (Epstein anomaly and ventricular septal defect) had persistent headaches and intermittent fever for five weeks. Physical examination revealed central cyanosis, finger clubbing and fever. The leucocyte count was elevated. Cerebral MR imaging showed the characteristic features of brain abscesses in the right frontal lobe, including multiloculation with adjacent satellite lesions, ring enhancement, T1-hyperintense and T2-hypointense signal areas within the abscess rim, as well as hypointense internal concentric rings on T2-weighted images. The diagnosis of brain abscesses was confirmed by craniotomy and pus drainage. She made a good recovery with a combination of antibiotics and surgery. Follow-up CT scans showed only changes of encephalomalacia at the healed abscess site. The aetiology, clinical features, and the role of CT and MR imaging in the diagnosis and management of brain abscess are discussed.

Extractions: Book: Localization in Clinical Neurology - This is a very detailed book. It is dry and moderately difficult reading, but is comprehensive in its subject matter. 1st you localize, then think of pathophysiology and DDx. In that sense I can say that this is the best neurological DDx book I have seen. Patten is only better in illustrations. Book: Diagnostic Cerebral Angiography - As a professional neuroradiologist, I recommend this book to all radiologists, neurologists and neurosurgeons who wish to choose just one book in the field of cerebral angiography. In this era of MRA and CTA, this book can give readers not too much, not too little knowledge about normal anatomy and pathological findings. Figures were clearly drawn and clinical DSA images were carefully chosen. Maybe those who wish to become professional neurointerventionists prefer those historical books edited by Dr. Newton, but I suppose even such people would love this book and read it daily.
