Q. I began having serious problems controlling my emotions about a year ago. I got very depressed missed a lot of work, lost interest in everything. My diagnosis was borderline personality disorder. Paxil (40mg/day) seemed to be helping with my moods, but I was doing destructive things like taking OTC sleeping aids, occassionally cutting myself to see how it felt, binging and purging, and using alcohol to escape. The sleep aids became a consistent problem. My psychiatrist refused to continue working with me if I didn't control their use and reduce the Paxil to 20mg/day. This made me more depressed, so I took more pills. I'm finally feeling more stable with a new doctor who has me taking Serzone - currently 400mg/day. I started at a lower dosage in early May and worked up to 400 by 5/28. I'm still having mood swings. I'm still crying and feeling worthless.

This instability often disrupts family and work life, long-term planning, and the individual's sense of self-identity. Originally thought to be at the "borderline" of psychosis, people with BPD suffer from a disorder of emotion regulation. While less well known than schizophrenia or bipolar disorder (manic-depressive illness), BPD is more common, affecting 2% of adults, mostly young women. There is a high rate of self-injury without suicide intent, as well as a significant rate of suicide attempts and completed suicide in severe cases. Patients often need extensive mental health services, and account for 20% of psychiatric hospitalizations. Yet, with help, many improve over time and are eventually able to lead productive lives.
