Extractions: Less common varieties of migraine are the so‑called migraine equivalents and unusual varieties of complicated migraine. "Migraine equivalent" is a term used to denote conditions or symptomatology believed to be migrainous in origin but without the typical history of either classic or common migraine headache. While the idea of migraine equivalents has not always been well received, according to Sacks the concentrated experience of working with migraine patients must convince the physician, whatever his previous beliefs, that many patients do suffer repeated, discrete, paroxysmal attacks of abdominal pain, chest pain, fever, etc., which will fill every clinical criteria of migraine save for the presence of headache. Very rarely true abdominal migraine occurs in adult life. In this situation there may be sudden severe abdominal pain with vomiting and even abdominal rigidity. The usual brevity of the attack or previous similar benign episodes or strong history of migraine help confirm the diagnosis. Other migraine equivalents include periodic diarrhea, fever, mood changes, and possibly attacks of chest pain known as precordial migraine. Acute confusional states have been attributed to migraine in juveniles.

However, several variations of migraine are unique to children and rarely if ever occur in adults. Migraine may present with prominent nonheadache symptoms in young children (migraine without headache), or neurologic symptoms (aura) may be much more prominent than the headache. Various recognized childhood syndromes assumed to be pathophysiologically related to migraine include benign paroxysmal vertigo of childhood, abdominal migraine, cyclic vomiting of childhood, and acute confusional migraine (acute confusional state). Basilar migraine (particularly in adolescent girls) may present with prominent dizziness and near-syncope and/or syncope with or without a subsequent headache. I am not sure how much I can help but I can relate to some of what you are going through and I have some more questions... To start, my daughter Emily has epilepsy; but she also has some other things going on that make it hard to tell what exactly is seizure-related and what isn't. By "abdominal migraine" I assume you mean a migraine accompanied by vomiting? Emily is severely sensitive to light (especially flickering light) and has episodes that right now are unclear as to whether they are migraine or seizure- she will cover her right eye, complain of head pain, become completely unresponsive to us, vomit, then need to sleep. The neuro feels she has a migraine disorder and hopefully we will start treating these episodes soon- they come almost daily and take so much from the quality of her life.
