NEUROLOGY CARLOS EDUARDO REIS Medstudents' Homepage Neurofibromatosis Neurofibromatosis(NF) is an autosomal dominant disease characterized by disordered growth of ectodermal tissues, and is part of a group of disorders called Phakomatoses (neurocutaneous syndrome). NF is divided in two Types: Neurofibromatosis 1(Von Recklinghausen's disease) Neurofibromatosis 1(NF1) is characterized by spots of increased skin pigmentation(café au lait spots), combined with peripheral nerve tumors and a variety of others dysplastic abnormalities of the skin, nervous system, bones, endocrine organs and blood vessels. Etiology The reponsible gene is located on the long arm of chromosome 17 Epidemiology Its incidence is 1 per 3.000 births and present in about 30 persons per 10.000 population. It is inherited as an autosomal dominant trait, but about 50 percent of cases arise as mutations. Pathology The peripheral nerve tumors of two types, schwannomas and neurofibromas. Both types of tumor occasionally become malignant. Clinical Manifestations The pigmented spots are irregular in shape with relatively even borders, vary in size, and are of brownish coffe color(café au lait). They are most proeminent over the trunk, in the axilla(axillary freckles), and about the pelve.

Refiere presentar desde hace 6 años aparición de nódulos superficiales duro elásticos no adheridos a planos profundos de crecimiento progresivo en región parieto-occipital, realizándose extirpación quirúrgica del mismo. Presenta luego aparición de nódulos de similares características en antebrazo y tronco, con máculas de color café con leche, por lo que consulta siendo internada para diagnóstico y tratamiento. Datos positivos al examen físico: Signos vitales dentro de parámetros normales. Presencia de nódulos superficiales y subcutáneos múltiples hipercrómicos en tronco y antebrazos no adherido a planos profundos, no dolorosos sin flogosis (Fotos A y B). Máculas café con leche de bordes definidos de forma redondeada u oval que respetan cuero cabelludo, párpados, palmas de manos y plantas de pies (Foto C), Máculas hipercrómicas tipo efélides en forma generalizada. No adenomegalias

Hierdoor ontbreekt een bepaald eiwit (neurofibromine). Het ontstaan van gezwellen kan het gevolg zijn (tumoren). Niet elke patiënt heeft precies dezelfde kenmerken, zelfs niet binnen één familie. De meest voorkomende kenmerken zijn de lichtbruine vlekken (café-au-lait vlekken) op de huid, die vaak al bij de geboorte aanwezig zijn. Soms ontstaan deze na een paar maanden tot een jaar. Vanaf de puberteit kunnen gezwellen (tumoren) ontstaan in of onder de huid, die uitgaan van de huidzenuwen (neurofibromen) en Lisch knobbeltjes, kleine geheel ongevaarlijke bobbeltjes in het regenboogvlies (iris) van het oog. Daarnaast is er op de kinderleeftijd vaak al sprake van een groot hoofd, kleine lengte, motorische problemen, leer-, spraak- en gedragsproblemen. Vertraagde geestelijke ontwikkeling is mogelijk. Er kunnen ook andere ernstige complicaties optreden, zoals onder andere gezwellen in het centrale zenuwstelsel (bijvoorbeeld in de oogzenuw), misvormingen door de neurofibromen, stofwisselingsstoornissen en een licht verhoogde kans op een kwaadaardige tumor.
