65. USE OF INTRAVENOUS INNOVAR IN VESTIBULAR DISORDERS
USE OF INTRAVENOUS INNOVAR IN vestibular disorders. Innovar has beenshown to be an effective drug for relief of vertigo in patients
http://depts.washington.edu/hearing/innovar.html
USE OF INTRAVENOUS INNOVAR IN VESTIBULAR DISORDERS Innovar has been shown to be an effective drug for relief of vertigo in patients with peripheral vestibular disorders. It suppresses vestibular reflexes and in certain patients, may provide long-term relief from symptoms related to their disorder. Most of the patients treated with Innovar have a diagnosis of Meniere's Disease
Pharmacologically, Innovar is a combination of a narcotic analgesic (Fentanyl) and a neuroleptic agent (Droperidol). This combination has been used for some time by anesthesiologists as a neuroleptic anesthetic. The drug works primarily by inhibiting the enzymatic destruction of neurotransmitting agents in the central nervous system. The major untoward effects of the drug are temporary low blood pressure and respiratory depression.
When administering Innovar for treatment of vertigo, the techniques include some major differences from the use of the drug for neuroleptic anesthesia. First, no pre-medication is used. Second, the drug is contraindicated in poor-risk patients (those with known cardiovascular or pulmonary disease, epilepsy or hypertension). Third, a smaller dose is given than is typically used for neuroleptic anesthetic. A maximum dosage of 0.03 cc/kg is administered under the direct supervision of a physician experienced in clinical pharmacology, such as an anesthesiologist.
The procedure for administering intravenous Innovar follows:
1. The otologist determines the need for an Innovar injection. The patient should be screened for contraindications to the use of Innovar, including drug allery, known cardiovascular disease, hypertension, pulmonary disease, and epilepsy.

Advanced Search document.write(hashTable['Home'].parentMenu) Home Patient Care Care Centers and Specialties Otolaryngology ... Vestibular Therapy Facial nerve disorders do not occur as frequently as auditory and vestibular disorders. When they do occur they tend to accompany these disorders since the facial nerve is closely associated with the auditory and vestibular systems. Insults that affect one system may affect any combination of the three. About the Clinical Center The Clinical Center for Auditory, Vestibular, and Facial Nerve Disorders at The University of Texas Southwestern Medical Center at Dallas is dedicated to serving the vast number of individuals who suffer from these related diseases. The Clinical Center combines patient care and clinical research with basic science expertise of a multidisciplinary nature, in the hope of providing a basis for more effective prevention and treatment. While patients who suffer from hearing loss, tinnitus, or disequilibrium continue to present a challenge to the medical profession, new technological and medical advances permit many patients to be successfully evaluated and treated. The Clinical Center is designed to serve physicians as a resource for their patients whose medical problems related to hearing, balance, and facial nerve paralysis are either elusive, advanced, or complex. Accomplishments Faculty within the Department of Otolaryngology - Head and Neck Surgery at UT Southwestern comprise the core clinical and research group of the Clinical Center for Auditory, Vestibular, and Facial Nerve Disorders. These physicians work in concert with faculty in other related departments to form a multidisciplinary team. Affiliated faculty include individuals from the UT Southwestern departments of Neurology, Neurological Surgery, Ophthalmology, Radiology, Anesthesiology, and Psychiatry.

Comprehensive management of vestibular disorders
Functional Anatomy History: Vestibular disorders (commonly known as dizziness) affect about 20% of the general population. Their prevalence increases with age: approximately 1/4 to 1/3 of the elderly complain of some form of dizziness. Dizziness can be related to a vast number of causal factors, but in most patients one or several of a handful of etiologies can easily be identified. The causes of dizziness are rarely secondary to a serious ailment. New facts: Traditionally, vestibular disorders have been evaluated and managed by various medical specialists, including otologists. Over the last three decades, otoneurology (or neuro-otology) has started to emerge as a new subspecialty within otolaryngology and, most recently, within neurology. Currently, there are only about a dozen neurologists in this country who specialize in the evaluation of vestibular disorders. The shift in specialty reflects the realization that many cases of dizziness don't require ear surgery and are actually neurological. Another factor prompting specialization in vestibular disorders is their presentation, which consists of a vague, evasive, and rather complicated set of symptoms. Dizziness can mean various sensations to different patients. It can present as vertigo (spinning), lightheadedness, near-fainting, disequilibrium, or a combination of these. Most commonly, dizziness is used by patients to mean any and all of the above. It is a subjective feeling, difficult to describe, and it can be caused by many classes of diseases. Almost half of patients with vestibular problems indeed have more than one type of dizziness.

Collaborator University of Pittsburgh The primary objective of this research is to increase our understanding of the combined effects of aging and vestibular disease on postural control in an effort to reduce the risk of falling in older adults. Patients with vestibular dysfunction commonly report a feeling of imbalance in situations with complex and rapidly changing visual stimuli such as in supermarkets. It is possible that reduced sensory function in older adults, or a slowing in central processing, may render the central integration of such information more difficult, resulting in greater imbalance. The proposed research will examine the postural sway response of older and younger adults, with and without vestibular disorders, as they view a moving visual scene. Understanding how older subjects with vestibular disorders integrate sensory information during these conditions will enhance our ability to develop balance rehabilitation programs in an effort to reduce the risk of falls in older adults.
