In brief
Air embolism, vascular disorders in plants Air embolism, a phenomenon feared by scuba-diving enthusiasts, also occurs in plants. Air bubbles can form in the vessels that transport the water taken up from the soil towards the aboveground parts of the tree, and this can have serious consequences because there is the risk that the leaves will no longer be supplied with water and will therefore dry out irreversibly. Drought and frost are the two climatic factors than can lead to embolism. For the last fifteen years or so, an INRA team has been studying the reasons behind this phenomenon as well as the ability of various tree species to resist it. The information obtained could be used to select tree varieties that can withstand embolism. In plants, pressure in the vascular system is usually low, whereas it is high in animals. Depression in plant vessels is caused by the leaves drawing up water. In fact, leaves function rather like pumps: they take water up from the soil via the tree's conducting vessels, and they release this water into the atmosphere through transpiration. The atmosphere is what drives this pump: the dryer the weather, the more the leaves are subjected to high evaporative demand. Depression can therefore be quite considerable. The vascular system is vulnerable and runs the risk of being drained out if air enters the vessels. Two phenomena are capable of causing embolism: frost and drought. During periods of drought, depression in tree vessels can reach extreme values. Under such conditions, bubbles of air can be drawn in through the vessel walls. This susceptibility to embolism varies considerably from one species to another, and the most resistant are those that are also known to be highly resistant to drought, such as cedar and holm oak for example. When depression in the vessels reaches the threshold for embolism, the plant responds by limiting its water losses: it closes up its stomata, minuscule orifices on the leaf surface, through which the carbon dioxide necessary for growth is absorbed but also through which water is released. The stomatal closure threshold corresponds to the embolism threshold: it would therefore appear that the tree will limit transpiration so as to prevent its conducting vessels from being damaged. Tree resistance to embolism in the summer is indeed an important element in understanding how they adapt to drought.

THE ANTICARDIOLIPIN SYNDROME
Some lupus patients have an antibody to a substance called cardiolipin, which causes a false-positive result in the blood test for syphilis. Consequently, lupus sufferers may be repeatedly treated with penicillin in the erroneous belief that they have a venereal disease. (In some cases, a false-positive test for syphilis may be the first manifestation of lupus.) It has been shown that the anticardiolipin antibody can be present for unknown reasons, as well as in association with other connective tissue diseases, and need not be restricted only to lupus patients. The presence of anticardiolipin antibody is associated with a tendency to increased blood clot formation. This tendency may cause strokes or heart attacks in young people, recurrent phlebitis, multi-infarct dementia (a form of senile dementia due to multiple small strokes), and recurrent miscarriage. The test for anticardiolipin antibody is now routinely performed in anyone suffering these problems, and is particularly important for women who recurrently miscarry. The antibody is believed to cause miscarriage by stimulating thrombosis of the small veins in the placenta. Anticardiolipin syndrome is treated with anticoagulant drugs, generally either low-dose aspirin or the Coumadin-type of anticoagulants, depending on the severity of the clinical manifestation and the duration for which the anticoagulation would have to be maintained. Pregnant women are given lowdose aspirin or, occasionally, heparin, since Coumadin would threaten the health of the fetus.

The use of acupuncture and tens in peripheral vascular disorders
Name Palle Rosted MD
Address 200 Abbey Lane, Sheffield, S8 OBU, UK England The beneficial effect of TENS and acupuncture in the treatment of peripheral vascular disorders has been stressed in a number of papers. In common for the majority of the published papers is a small number of patients and lack of control group. A controlled randomised study using TENS in the treatment of leg ulcers is presented. 36 patients were included in the study and were randomly allocated to either TENS treatment or MOCK-TENS treatment. Patients were using the equipment 30 minutes three times a day for three months and electrodes were placed on the left hand in the region of LI-4 (negative electrode) and SI-5 (positive electrode). Patients was assessed after one two and three months treatment. Follow up was carried out three and six months after termination of treatment. The effect of the treatment was assessed by measuring the ulcers, Doppler measurement, Visual analogue scale for pains, Visual analogue scale for well being, McGill, intake of pain killers and skin temperature. Both TENS and MOCK-TENS was found to be beneficial in the healing of the leg ulcer and no difference was found between the two groups. Regarding pain management, TENS was found better compared to MOCK-TENS. Surprisingly, an increase of the Doppler was found in the TENS group compared to the MOCK-TENS group, despite no change in the skin temperature was observed.

Abstract
Over the last decade, various studies have been reported to evaluate the circadian pattern of cardiovascular and cerebro-vascular diseases. The data from Indian population is lacking. We undertook this prospective observational study to evaluate the circadian variation in disorders like cerebro-vascular accidents and transient ischemic attacks. Total of 146 patients (events) were studied. Only 10 patients had TIA's. 55% had hemorrhage and 45% had infarction. The 24 hours period was divided into 6 equal portions of 4 hours each. The maximum events were seen between 4 am to 8 am and 12 noon to 4 pm (23.28%) each. Minimum events were seen between 12 midnight to 4 am 14/146 - 9.58%). The circadian variation in occurrence of cerebro-vascular disorders was present with two equal peaks.
