    Peripheral vascular disorders: Disorders affecting the arteries, veins, and lymphatics of the extremities. Peripheral vascular disorders may be arterial (occlusive or functional), venous, combined arteriovenous (eg, arteriovenous fistula), or lymphatic. Occlusive arterial disease includes peripheral arterial occlusion and thromboangiitis obliterans. Functional arterial disorders may be vasospastic (Raynaud's phenomenon and disease, acrocyanosis) or vasodilatory (erythromelalgia); may be secondary to a local fault in the blood vessels or to disturbances in sympathetic nervous system activity; or may accompany organic vascular disease.     Vascular Disorders
    The vascular supply of the spinal cord originates superiorly from the vertebral arteries and, in the low cervical and thoracic area, from branches of the aorta, often from a particularly large and important low thoracic branch. Branches of the anterior spinal artery supply the anterior 2/3 of the cord, and branches of the posterior spinal artery supply the posterior 1/3.
    INFARCTION
    Because most of spinal circulation is supplied by two or three major arterial branches, segments of the cord in the watershed area between the branchesaround the 2nd to 4th thoracic segmentsare especially vulnerable to ischemia. Vascular thrombosis is uncommon, however, and vascular compression (eg, from tumors, acute disk compression) or occlusion from remote causes (eg, aortic surgery, dissecting aneurysm) more commonly causes infarction than does intrinsic disease of spinal arteries. Rarely, polyarteritis nodosa causes occlusion of spinal arteries. Sudden pain in the back and in the distribution of the affected segment is followed by bilateral flaccid weakness and a dissociated sensory loss with impaired pinprick and temperature sensation below the level of the infarct. The distribution of the anterior spinal artery is usually affected, so that touch, proprioception, and vibration sense are likely to be spared because they are conducted in posterior tracts of the cord. As with all infarcts, the deficit is most marked during the first few days and may partially resolve with time.

Other causes of hepatic congestion include constrictive pericarditis, obstruction of the inferior vena cava and hepatic veins (Budd-Chiari syndrome) and occlusion of the small hepatic veins (veno-occlusive disease). Small Intestinal Vascular Disorders page 257 23.1 Acute Mesenteric Ischemia Nonocclusive bowel ischemia is the most common and lethal form of intestinal vascular insufficiency, accounting for at least 50% of all cases, with a mortality rate approaching 100%. It is commonly associated with reduced cardiac output, intra-abdominal sepsis and advanced malignant neoplasms. Digitalis constricts the splanchnic circulation and may aggravate or even precipitate mesenteric ischemia. The typical patient is over 50 years of age, with arteriosclerotic or valvular heart disease, poorly controlled long-standing congestive heart failure, hypotension, recent myocardial infarction or cardiac arrhythmias. Abdominal pain is characteristically periumbilical and crampy. In the early stages, physical signs are often minimal. The abdomen is soft, sometimes slightly distended, with mild tenderness on palpation. Abdominal pain of any degree of severity associated with minimal abdominal findings and a high WBC (often over 20,000/mm

    The Department of Radiology is pleased to announce the opening of our newest section, Interventional Neuroradiology. Dr. Jeffrey Farkas will head the division. Dr Farkas has recently joined the staff at University Hospital (UMDNJ) after completing a fellowship in Interventional Neuroradiology at the Massachusetts General Hospital and Brigham and Women’s Hospital in Boston Massachusetts.
    Therapy
    Interventional Neuroradiology is a relatively new field that uses state of the art equipment to treat complex cerebrovascular problems from within the blood vessels. Minimally invasive endovascular treatments have had a major impact on the treatment of complex cerebrovascular disorders.
