Abstract - Challenging clinical criteria for vascular dementia (VaD).
by T. Erkinjuntti VaD relates to different vascular mechanisms and changes in the brain, and has different causes and clinical manifestations. VaD is not only the traditional multi-infarct dementia. The pathophysiology of VaD incorporates interactions between vascular aetiologies (cerebrovascular disorders and vascular risk-factors), changes in the brain (infarcts, white matter lesions, atrophy), host factors (age, education) and cognition. To which extent the given aetiologies and changes in the brain cause, compound or only coexist with the VaD syndrome, and which type, extent, side, site and tempo of vascular lesions in the brain relates to different types of VaD is a matter of debate. Classification of VaD may be based according to the primary vascular etiology, the primary type of ischemic brain lesions, the primary location of brain lesions, and the primary clinical syndrome. Proposed types include the cortical VaD or multi-infarct dementia, the subcortical VaD or the small vessel dementia, the strategic infarct dementia, hmorrhagig dementia, hereditary vascular dementia, and combined or mixed dementia (Alzheimer’s disease with cerebrovascular disease). HONOLULU, June 10 – For centuries, the herb Chinese gastrodine has been used in China to treat disorders such as dizziness, headache and even ischemic stroke. Research presented today at the American Heart Association's Second Asia Pacific Scientific Forum shows treatment with a gastrodine compound granule is effective in improving impaired memory, orientation, language and other effects of stroke in patients who were diagnosed with mild to moderate vascular dementia (VaD) after their stroke. Vascular dementia is one of the most common dementias ranked after Alzheimer's disease. It's a growing problem in China, with a prevalence of about one to three percent, similar to prevalence rates in the United States and Europe. Because some medications for the condition are very expensive for most patients in China, researchers focused on finding treatments that would be effective and less costly. Chinese herbal medicines not only are less expensive and have fewer side effects than standard chemical medications, but they are also more accepted by the Chinese people, according to lead researcher Jinzhou Tian, M.D., Ph.D., director of the Institute of Geriatrics, Beijing University of Chinese Medicine. In the gastrodine group, there were significant increases of the average MMSE (mini-mental state exam) score compared to baseline scores (20.83 vs.18.96). In the areas of memory, orientation, calculation, and language, the MMSE showed similar improvement in both groups.

Recently new criteria for diagnosing vascular dementia (VaD) have been suggested by (a) the State of California Alzheimer's Disease Diagnostic and Treatment Centres (ADDTC) and (b) the NINDS-AIREN group after an international workshop convened by the National Institute for Neurological Disorders and Stroke (NINDS), with support from the Association Internationale pour la Recherche et l'Enseignement en Neurosciences (AIREN). Aricept Efficacious for Vascular Dementia
Data presented Wednesday at the American Academy of Neurology (AAN) Annual Meeting showed that Pfizer (NYSE: PFE) and Eisai's Aricept (donepezil hydrochloride tablets) was both efficacious and well tolerated in patients with vascular dementia. The results are a significant step towards increased use and label expansion. The study reproduced observations from earlier investigation, solidifying their resultant conclusions. The cognitive decline in vascular dementia, characterized by confusion, memory loss, impaired orientation, and impaired judgment, is initiated by a stroke or series of strokes, and is second only to Alzheimer disease (AD) as a cause of dementia.
This confirmatory randomized, double-blind, placebo-controlled, 24-week trial evaluated 603 patients with vascular dementia without a prior AD diagnosis. Study subjects received high or low dose Aricept or placebo daily, and were assessed with various well-verified tools used in the clinical evaluation of cognitive decline.
High and low dose Aricept recipients achieved statistically significant advantages in terms of cognitive function as measured by the Alzheimer's Disease Assessment Scale (ADAS-cog), cognitive function as measured by the Mini-Mental State Examination (MMSE), and slowing of functional deterioration as measured by the Alzheimer's Disease Functional Assessment and Change Scale (ADFACS).

Dementia 1996:7:196-200 Abstract Pre- and postsynaptic elements of the 5-hydroxytryptamine (5-HT, serotonin) system were studied in a control group and in patients with vascular dementia (VAD). The 5-HT uptake site was used as a presynaptic marker for 5-HT terminals and 5-HT1A and 5HT2 receptors were used as postsynaptic markers. The binding sites were quantified with radioligand binding techniques, where the radioligands used were [3H]paroxetine, [3H]8-OH-DPAT and [3H]ketanserin, respectively. The presynaptic uptake site was studied in frontal and temporal cortices and caudate nucleus. 5-HT1A and 5-HT2 receptors were studied only in frontal and temporal cortices. There were no differences between control and VAD groups in any of the regions investigated with respect to the number of binding sites (Bmax) and binding affinity (Kd). This indicates that both pre- and postsynaptic parts of the 5-HT system are intact in these brain areas in VAD.
