NINDS - AIREN criteria for the diagnosis of vascular dementia I. The criteria for the clinical diagnosis of probable vascular dementia include all of the following:
  • Dementia defined by cognitive decline from a previously higher level of functioning and manifested by impairment of memory and of two or more cognitive domains (orientation, attention, language, visuospatial functions, executive functions, motor control, and praxis), preferable established by clinical examination and documented by neuropsychological testing; deficits should be severe enough to interfere with activities of daily living not due to physical effects of stroke alone.

    The term vascular dementia is now used to encompass several
    pathologically different and clinically distinguishable syndromes et al . [1] to describe dementia associated with multiple cerebrovascular occlusions. With improved imaging technology it became clear that vascular dementia represented a wider entity than MID and the Hachinski Ischemic Scale has now been reassessed [2]. In particular, Magnetic Resonance Imaging (MRI) revealed the presence of diffuse white matter lesions, presumably due to ischemic changes, in many demented patients. Thus, vascular damage can occur through various mechanisms ( Table 1 ). The term vascular dementia is now used to encompass several pathologically different and clinically distinguishable syndromes ( Table 2 Table 1. Mechanism of vascular damage.

    Arteriosclerosis Changes in the brain’s blood supply may result in the death of some brain tissue (neurons). The initiating event may be a single stroke (brain attack); a number of small strokes; a temporary decrease in brain blood flow transient ischemic attack (TIA); a traumatic brain injury (TBI) or the slow clogging of the arteries in the brain (arteriosclerosis). The location in the brain where the decreased blood flow occurs will determine the seriousness of the impact on cognition and type of symptoms that arise. Symptoms that begin suddenly may be a sign of this kind of dementia. People with vascular dementia may show signs of improvement or remain stable for long periods of time, then quickly develop new symptoms if more damage occurs.     Causes
    Vascular dementia can be caused in several different ways.
    • Most commonly there is blockage of small blood vessels (arteries) deep within the brain. When any part of the body is deprived of blood, which carries oxygen and nutrients, it dies - this is called an infarct or ischemia. When brain tissue dies it is also called necrosis.
      Blockages may be caused by build up of plaque on the inside of the arterial wall or by clots which have broken off, jamming a smaller tributary upstream. Clots can also result from abnormal heart rhythms, or other heart pathology, or can form on the inside of the major carotid arteries that run up the side of the neck and supply the brain.
      Rarer causes of vascular dementia are associated with auto-immune inflammatory diseases of the arteries such as systemic lupus erythermotsis (SLE or lupus) and temporal arteritis. Both of these can be treated with cortisone type drugs or other drugs to suppress the immune system.
    Types of Vascular Dementia
    Vascular dementia is due to impaired blood supply to the brain and can be divided into different types depending on the nature of the vascular disease.

    Medical thinking about vascular dementia has changed considerably over the past few decades. At one time, doctors attributed almost all mental impairment associated with old age to "hardening of the arteries" in the brain. As scientists gained deeper understanding of other dementing illnesses, Alzheimer’s gradually earned recognition as the most common dementia, and vascular dementia was considered the second most frequent. Experts now acknowledge that neither the frequency nor the characteristics of vascular dementia are well understood. Several professional bodies have developed different sets of criteria for diagnosing the disorder, making it difficult to estimate its frequency with any precision. Estimates found in recent scientific literature indicate that vascular dementia accounts for anywhere from one-tenth to one-third of dementia cases

    F01 Vascular Dementia
    Vascular (formerly arteriosclerotic) dementia, which includes multi-infarct dementia, is distinguished from dementia in Alzheimer's disease by its history of onset, clinical features, and subsequent course. Typically, there is a history of transient ischaemic attacks with brief impairment of consciousness, fleeting pareses, or visual loss. The dementia may also follow a succession of acute cerebrovascular accidents or, less commonly, a single major stroke. Some impairment of memory and thinking then becomes apparent. Onset, which is usually in later life, can be abrupt, following one particular ischaemic episode, or there may be more gradual emergence. The dementia is usually the result of infarction of the brain due to vascular diseases, including hypertensive cerebrovascular disease. The infarcts are usually small but cumulative in their effect.
    Diagnostic Guidelines
    The diagnosis presupposes the presence of a dementia as described above. Impairment of cognitive function is commonly uneven, so that there may be memory loss, intellectual impairment, and focal neurological signs. Insight and judgement may be relatively well preserved. An abrupt onset or a stepwise deterioration, as well as the presence of focal neurological signs and symptoms, increases the probability of the diagnosis; in some cases, confirmation can be provided only by computerized axial tomography or, ultimately, neuropathological examination.

They may be caused by poor diet, lack of sleep, or too many medicines, for example. Feelings of loneliness, boredom, or depression also can cause forgetfulness. These problems are serious and should be treated. Often they can be reversed. Sometimes, however, mental changes are caused by diseases that permanently damage brain cells. The term dementia describes a medical condition that is caused by changes in the normal activity of very sensitive brain cells. These changes in the way the brain works can affect memory, speech, and the ability to carry out daily activities. Multi-infarct dementia is the most common form of vascular dementia, and accounts for 10-20% of all cases of progressive, or gradually worsening, dementia. It usually affects people between the ages of 60-75, and is more likely to occur in men than women.
