Skip this book If you are looking for good information on MVPS, DON'T get this book. It is written for the author himself to read. Websites like MVPSupport.com offer more information than this book Shame on the author This book is written for a second grader. Shame on the author for being so patronizing to those of us who suffer from this life-altering diease. I'm not a psychologist, but I could have written the general information in this book in fact it could have been written for just about any disease by any Psych 101 student. It's a form letter. Don't bother. Disappointed I agree with the other reviewer.I found this book to offer little information on MVP. Sorry I ordered it. Check out the other books, they are much better.

GENERAL PRINCIPLES OF VALVE MALFUNCTION VALVULAR STENOSIS: Pressure in upstream chamber >> Pressure in downstream chamber during time of flow (when valve is normally open). Hemodynamic Hallmark = "PRESSURE GRADIENT"
VALVULAR REGURGITATION: Also termed "INSUFFICIENCY" or "INCOMPETENCE" )
Retrograde flow of blood "upstream" during time when valve is normally closed Hemodynamic Hallmark = "VOLUME OVERLOAD"

EXAMPLES OF VALVULAR HEART DISEASE MITRAL STENOSIS: Pathophysiology: Obstruction of blood flow from LA to LV during diastole, causing increased pressure in the left atrium, pulmonary capillaries and, eventually, the right side of the heart. (see fig 1 and fig 1A
As the valve area (i.e. the cross-sectional area of the valve opening during diastole) becomes smaller, the pressure gradient increases. The relationship between the mitral valve area, the forward cardiac output and the pressure gradient across the valve during diastole is complex, and is defined by this equation:
CARDIAC OUTPUT (mI/min) MITRAL VALVE AREA DIASTOLIC FILLING PERIOD (sec/min)
SQ ROOT of PRESSURE GRADIENT (mm Hg) Examine the relationship between mitral valve gradient and flow (cardiac output) for various valve areas.

L IFELONG H YPERTENSION AND V ALVULAR H EART D ISEASE E MERGE A S S TRONG S TROKE P REDICTORS PREVIOUS HYPERTENSION RAISES STROKE RISK Current blood pressure is not the only thing that counts, said Dr. Seshadri, a researcher with the Framingham Heart Study. "We all know that hypertension is the most important risk factor for stroke," Dr. Seshadri said. She noted that risk is usually calculated using current blood pressure levels, and this is based on the assumption that previous levels are inconsequential. However, stroke is primarily a disease of the elderly, and up to one third of these strokes occur in normotensive subjects. The Framingham investigators suspected that this might reflect hypertensive damage from earlier periods. "The aim of this study was to evaluate the effect of past blood pressure levels on the future risk of ischemic stroke," Dr. Seshadri said. The study sample included the 5,197 subjects in the original Framingham cohort, on whom there is 50 years of information. Patients were free of stroke at baseline. Blood pressure was recorded once every two years, and there was continuous surveillance for stroke throughout this period. The investigators classified subjects by their age at baseline. The main exposure variable was blood pressure. Dr. Seshadri examined both current blood pressure and past blood pressure. "For the past blood pressure we looked at the previous decade as well as earlier decades and averaged all the available blood pressure records in the decade of interest," she said. The outcome variable was the time to first completed ischemic stroke. Transient ischemic attacks (TIAs) were not counted as end points.

valvular heart disease. A common function. Definition Valvular dysfunction in heart disease can be classified in one of three ways 1
http://www.bms.ed.ac.uk/services/webspace/ssm/thoraxvalvular.htm
Valvular Heart Disease
A common disease process within the heart, especially as age increase is valvular dysfunction. Essentially dysfunction refers to a general failure of any of the four cardiac valves ( aortic, pulmonary, trisucpid and bicuspid valves) to carry out their intended function.
Definition:
Valvular dysfunction in heart disease can be classified in one of three ways:
stenosis ; failure of a valve to open completely thereby impeding forward blood flow
incompetence/insufficiency ; failure of a valve to close completely thereby allowing reversed flow of blood (regurgitation)
mixed ; when both conditions coexist in the same valve.
Causes:
Valvular stenosis is almost always due to a primary cuspal abnormality and therefore will usually present as a chronic process of dysfunction. Valvular incompetence can have several generalised causes as listed below:
  • intrinsic damage due to disease of the valve cusps damage to or distortion of the supporting structures (aorta, mitral annular, chordae tendonae, papillary muscles) without primary change in the cusps
Valvular insufficiency can appear acutely as seen in infective endocarditis or chronically with leaflet scarring and subsequent retraction.

http://www.lacard.com/html/services/valvularheart.html
The heart has four valves including the mitral, the aortic, the pulmonic, and the tricuspid. These valves exist to direct and control blood flow through the heart. Sometimes the valves can become scarred and narrow leading to a need for improving the opening and the blood flow. The most commonly treated valves are the mitral and aortic. We have an ongoing interest and an extensive experience in managing valvular disease. We have achieved a procedural success rate in excess of 95%. We have treated patients ranging from their early teenage years to those in their early 90s. In valvuloplasty, a small balloon is used to dilate the valves resulting in a larger opening and improved blood flow.
