For the rest of my pages on fungi, please click http://TomVolkFungi.net Coccidioides immitis (kok-sid-ee-OID-eez IMM-ih-tiss) is the cause of a nasty fungal disease called coccidioidomycosis (kok-sid-ee-oid-oh-my-KOH-sis). Like the other true-pathogenic, systemic human fungal diseases histoplasmosis blastomycosis , and paracoccidioidomycosis, Coccidioidomycosis starts out as a lung disease caused by inhalation of the conidia, shown to the left. Most often the disease causes mild flu-like symptoms, but usually is resolved in the lungs. This fungus is a dimorphic pathogen, which means it can change from the room-temperature hyphal form at to the body-temperature spherule form (shown to the right) containing endospores. These endospores can be transported by the bloodstream to other parts of the body, particularly to the brain and central nervous system, where they can germinate and grow to cause even more severe disease. The dimorphism helps the fungus to evade the immune system by the changing of the surface antigens of the fungus. The disease often begins as a benign, inapparent or mildly severe upper respiratory infection that usually resolves rapidly. Recovery from mild forms of the disease usually results in lifelong immunity to reinfection. However, if there are enough spores inhaled, or if the person's immune system is compromised in some way, the disease can spread to other parts of the body, Rarely the disease is an acute or chronic severe disseminating fatal mycosis. If infection is established, the disease may progress as a chronic pulmonary condition or as a systemic disease involving the meninges (lining of the brain), bones, joints, and subcutaneous and cutaneous tissues. Such involvement is characterized by the formation of burrowing abscesses. Although the symptoms of the disease are quite variable, but often the patient has an allergic reaction to the circulating fungus, producing reddening of the skin known as "desert bumps," shown to the left.

RIFT VALLEY FEVER Overview Rift Valley Fever (RVF), is a zoonosis (a disease which primarily affects animals, but occasionally causes disease in humans). Since 1930, when the virus was first isolated during an investigation into an epidemic amongst sheep on a farm in the Rift Valley of Kenya, there have been outbreaks in sub-Saharan and North Africa. In 1997-98, there was a major outbreak in Kenya and Somalia. In September 2000, RVF was for the first time reported outside of the African Continent. Cases were confirmed in Saudi Arabia and Yemen. This virgin-soil epidemic in the Arabian Peninsula raises the threat of expansion into other parts of Asia and Europe.

A year ago, my life was altered beyond comprehension. I was forced to watch the world go by when I was diagnosed with Valley Fever.
I had spent the two months prior to the diagnosis working nine hours a day, Monday through Friday, and attending play rehearsals from 6 p.m. until midnight almost every night. As you can imagine, I was exhausted. I quit my job when school started and noticed I was constantly tired. I assumed it was because I was concentrating on my school work, but began to worry when the fatigue grew worse.
One weekend, I didn't sleep more than three hours a night and fought desperately for every breath, willing the pain in my chest to recede. Every eight hours, I took a large dose of pain medication. My breathing became incredibly shallow and I could not speak more than five words before taking a breath. I had hoped to endure the pain until Monday so I could visit my family doctor, but I finally made my mother take me to Urgent Care.
I spent three hours in the waiting room before the doctor called me. He spent several minutes asking questions and checking my breathing, pulse and blood pressure. He ordered x-rays, and after examining them, told me I had pneumonia. He prescribed an antibiotic and pain medication as well as an over-the-counter expectorant to loosen my lungs.
I went home Sunday night thankful that the doctor allowed me to be treated at home. My joy did not last, though. I awoke Monday evening to find a horrible rash covering my body. I began vomiting and went to my family doctor Tuesday morning. She took one look at my rash and told me she was sure I had Valley Fever.

Valley Fever Vaccine Project
Coccidioidomycosis, also known as Valley Fever, is a lung infection caused by a fungus that is common in the southwestern United States, northern Mexico, and some areas of Central and South America. About sixty percent of the people who inhale spores from the cocci fungus do not get sick. For some, it may feel like a cold or the flu. But for those sick enough to seek medical help, Valley Fever can lead to serious illness or death. Public health experts have observed that the cost of Valley Fever can be enormous in personal and economic terms - with victims becoming too ill to work for days, weeks, and even months. In its most serious form, Valley Fever can permanently damage lung or bone tissue, or cause meningitis by attacking the membranes surrounding the brain. Currently, there are no available drugs or treatments for this disease, and the relapse rate is high. The Valley Fever Vaccine Project is a research collaboration working to create a candidate vaccine that would lead to human clinical trials by the end of 2004. For Phase 1 of this project, IGH and its partners assessed the safety and activity of a skin-test antigen called coccidioidin. Phase 2, which began in Fall 2002, will focus on conducting more extensive skin tests among select populations in California.

Travel Health Advisory REPORTS OF RIFT VALLEY FEVER IN KENYA AND SOMALIA Health Canada’s Laboratory Centre for Disease Control has been closely monitoring reports of Rift Valley fever in Kenya and Somalia. Based on laboratory tests at the National Institute for Virology in Johannesburg, South Africa, the World Health Organization has confirmed that an outbreak of Rift Valley fever may have affected humans and domestic animals (goat, sheep, cattle and camels) in Kenya’s North-Eastern, Eastern, Rift Valley, Central and Coast provinces, including some national parks. Cases have also been reported in areas near Nairobi and Mombasa. An outbreak is also reported in the flooded areas in Somalia delimited by the towns of Belet Weyne and Johar on the Shabelle River. Much of the outbreak areas is difficult to access and the full extent of the outbreak is not yet known but reports indicate that approximately 300 people and hundreds of animals have died from disease in Kenya and a similar number in Somalia. Rift Valley fever is a virus that is widespread in Africa. It is transmitted by mosquitoes as well as exposure to infected animals or their tissues. Symptoms include high fever, diarrhea, jaundice, intestinal problems, and haemorrhagic complications. Currently, there is no vaccine available for Rift Valley fever.

Health Canada is monitoring an outbreak of Rift Valley Fever (RVF) in Saudi Arabia. As of October 24, the World Health Organization (WHO) has confirmed 443 suspected cases of RVF with 85 deaths in the Jizan and Asir Provinces in southwestern Saudi Arabia. Health Canada is also monitoring an outbreak of RVF in northern Yemen as reported by the Yemeni Ministry of Health.As of October 19, there have been 653 suspected cases of RVF with 80 deaths reported in the Hodeidah, Hajjah, Mahweet and Sadeh Governorates in northwestern Yemen. Teams of scientists led by the WHO are in Saudi Arabia and Yemen to carry out extensive investigations, and assist in prevention and treatment efforts. At this time, the Health Ministries of Saudi Arabia and Yemen are implementing control measures against RVF, including the disposal of dead animals and insecticide spraying to destroy mosquitoes. Prior to these outbreaks, RVF, an acute viral haemorrhagic fever affecting domestic animals and humans, had occurred only on the African continent in the eastern and southern areas.

VENTURA COUNTY, Calif. A deadly fungal infection called valley fever is spreading in and around Los Angeles. Dozens of people have become sick after being exposed to spores in the aftermath of last fall's wildfires. Ventura County resident Carolyn Burnett (pictured, left) is a victim of valley fever. Since October, there have been at least 80 cases in Ventura County. Health officials report it's an epidemic. The 44-year-old got it by simply breathing. "I still cry now because it's not fair that you can get something just from breathing because it's just not right and it ends up to where I can't even walk, or move at times. That's just how bad it is," Burnett said.

Red Cross officials say the outbreak of deadly Rift Valley Fever in Kenya has now spread from the north east to parts of the coast and central provinces.They say the numbers who have died from the disease may be double the government figure of 350 since late last year. Graham Easton of BBC Science has this report on the outbreak, and the nature of the disease.
Experts initially thought the outbreak in Kenya was due to anthrax, but now scientists from the World Health Organisation have found the Rift Valley Fever virus in samples from several human victims of the mysterious outbreak. Rift Valley fever is a viral disease spread by mosquitoes. It's primarily an illness of livestock such as sheep, goats and camels. During epidemics in livestock the virus causes spontaneous abortions and deaths in young animals. But the disease can have devastating effects on humans too.
Symptoms include fever, vomiting and often severe bleeding After an incubation period of about 3-6 days, the early symptoms in humans are fever and headache which can progress to bleeding from the gums and nose, and sometimes a fatal coma. Rarely the infection results in damage to the eyes, inflammation of the brain, and, as in this latest outbreak, severe bleeding.

Valley Fever Vaccine Project Advances

Scientists with the Valley Fever Vaccine Project, administered by California State University, Bakersfield Foundation, announced today that they have selected a candidate for a Valley Fever vaccine after four years of intensified research at five research centers around the country.
The development was outlined today during a meeting of project directors, one of the scientists involved in the research, U.S. Rep. Bill Thomas and the CSUB Foundation Board of Directors.
“We believe that we have identified the antigens we want to take forward. The antigens are not the entirety of what constitutes a vaccine, but they are the critical component. The combination of the research data plus the recent results from the primate trial brought us to a decision point. Even though the trial is still in progress, the results to date are convincing enough to validate our choice of the antigens. This is very good news."
During the meeting Thomas, R-Bakersfield, chairman of the House Ways and Means Committee, was recognized by the CSUB Foundation board for the $750,000 grant providing funding for the project through the Centers for Disease Control. These funds will be used exclusively for the early phases of the pharmaceutical development of the vaccine, critical to achieving the goal of planned human testing.

Sunday, November 10, 2002 Secession lost heavily in the city and maintaining a 4,000 vote majority in the San Fernando Valley. But secession's legacy is important to this city. Already, the issues raised by secessionists are becoming top priorities for the city. Boroughs are getting a revival. Businesses too are benefiting from the attention paid by cityhood advocates on the city's jobkilling regulations. As for the candidates who ran for Valley offices, many of them are heartened by the efforts so much so that they are running again, this time for the Los Angeles City Council. The winners of Valley offices are not retreating from the good fight, they have formed the "Collective" to press on for reforms.
More importantly, the independence movement forced people all over to pay attention to the Valley's unique culture. Before, what the country knew about the Valley was filtered through Hollywood's lens, that of a bland suburbia peopled with vapid white girls and their obnoxious dialect. Now, the ethnic diversity, the slumminess, its relationship with Los Angeles and its pastoral roots are what the country is seeing. I prefer the nuanced gritty picture to the plastic fantastic vision pitched by developers. I've always felt secession was a radical plan but it was the only solution offered and not a bad one at that. I can't pass on any judgment on whether the voters' decision was a disappointing colossal mistake until I see the sort of city Los Angeles will fashion itself as come the next city council elections in April.
