NGU (NonGonococcal Urethritis) is an infection of the urethra caused by pathogens (germs) other than gonorrhea. Several kinds of pathogens can cause NGU, including:
  • Chlamydia trachomatis Ureaplasma urealyticum Trichomonas vaginalis (rare) Herpes simplex virus (rare) Adenovirus Haemophilus vaginalis Mycoplasm genitalium
NGU is most often caused by chlamydia, a common infection in men and women. The diagnosis of NGU is more commonly made in men than women, primarily due to anatomical differences. How can I get NGU?
Sexual:
Most germs that cause NGU can be passed during sex (vaginal, anal or oral) that involves direct mucous membrane contact with an infected person.
These germs can be passed even if the penis or tongue does not go all the way into the vagina, mouth or rectum, and even if body fluids are not exchanged.

Recombinant Refers to compounds produced by laboratory or industrial cultures of genetically engineered living cells. The cells' genes have been altered to give them the capability of producing large quantities of the desired compound for use as a medical treatment. Recombinant compounds often are versions of naturally occurring substances.

DEFINITION Urethral inflammation most often manifested by urethral discharge, dysuria, or meatal pruritis confirmed by the laboratory finding of increased number of polymorphonuclear leukocytes (PMNs) on Gram-stained urethral smear or in the sediment of a first voided urine. I. Etiology Gonococcal urethritis: Neisseria gonorrhoeae. Nongonococcal urethritis: Chlamydia trachomatis Ureaplasma urealyticum Trichomonas vaginalis Herpes Simplex Virus Candida albicans Miscellaneous bacteria Mycoplasma genitalium Bacteroides urealyticum Other ( E. Coli , Haemophilus species) Unknown II. Epidemiology Urethritis affects an estimated 4 million American men each year. NGU is more common than GU in the U.S. and in much of the developed world; GU may account for up to 80% of the cases of acute urethritis in some undeveloped areas. The ratio of NGU to GU is greater among groups of higher socioeconomic status in the U.S. For example, comparisons of men with NGU versus GU show that those with NGU more often are white, with higher education, higher socioeconomic status, and fewer number of sexual partners. On college campuses, more than 85% of urethritis is nongonococcal.

7.04 Urethritis (Drip)
Presentation
A male complains of dysuria, a burning discomfort along the urethra, or a urethral discharge. A copious, thick, yellow- green discharge which stains underwear is characteristic of gonorrhea, whereas a thin, white, scant discharge with milder symptoms is characteristic of chlamydia. Urethritis in a female may be asymptomatic or indistinguishable from cystitis or vaginitis, or may be manifest as UTI symptoms with a low concentration of bacteria on urine culture, or tenderness localized to the anterior vaginal wall. In addition to increased vaginal discharge, women may have intermenstrual bleeding, especially postcoital spotting and cervical friability.
What to do:
  • Gram stain any urethral discharge, looking for gram- negative diplococci inside white cells, which imply gonococcal infection.
  • Order a serologic test for established syphilis. Further antibiotic treatment is required if the RPR or VDRL is positive .
  • Examine the urine sediment for swimming protozoa, implying infection with Trichomonas vaginalis, best treated with metronidazole (Flagyl) 250mg qid x 7d, or 2gm po once.

This is the tube that carries urine and semen through the penis and forms the opening at the end of the penis. Men between ages 20-35 are most at risk for developing pain with urination, as are men who have multiple sexual partners or those who engage in high-risk behaviors such as not using condoms or anal intercourse. Urethritis may be caused by the same germs that cause urinary tract infections (also called bladder infections), and the symptoms may be similar.

Abstract: A retrospective population-based case-control study of sexually transmitted urethritis was conducted at a large military base over a 21-month period. During the study, 9514 patients were seen for sexually transmitted disease. The analysis was restricted to active duty males and showed that Blacks had 14.8 times the incidence rate for gonococcal urethritis (GCU) and 4.7 times the rate of nongonococcal urethritis (NGU) compared to Whites. There were slightly fewer cases of NGU than GCU. A case-control study of active duty soldiers were 1.65 times as likely to have NGU as uncircumcised subjects (95% CI): 1:37-2.00). However, circumcision was not associated with an increased incidence of GCU. Definition Urethritis is an inflammation (irritation with presence of extra immune cells) of the urethra.
Female Urinary Tract
Male Urinary Tract Urethritis may be caused by the same organisms that cause urinary tract infections (for example, E. coli or klebsiella) and by some sexually transmitted diseases (for example, chlamydia gonorrhea , and Ureaplasma urealyticum infections). Viral causes of urethritis include herpes simplex virus and cytomegalovirus. Urethritis may also develop because of a chemical irritation (such as spermacide in condoms or contraceptive jelly, cream, or foam). Trauma may also cause urethritis. Men between the ages of 20 and 35, those with multiple sexual partners, and those who engage in high-risk sexual behavior (such as lack of condom use, particularly during active anal intercourse) are at higher risk.
