What is a stomach or duodenal ulcer?
Facts About Stomach and Duodenal Ulcers During normal digestion, food moves from the mouth down the esophagus into the stomach. The stomach produces hydrochloric acid and an enzyme called pepsin to digest the food. From the stomach, food passes into the upper part of the small intestine, called the duodenum, where digestion and nutrient absorption continue. An ulcer is a sore or lesion that forms in the lining of the stomach or duodenum where hydrochloric acid and pepsin are present. These ulcers are referred to as peptic ulcers gastric ulcers in the stomach and duodenal ulcers in the duodenum.
  • About one in 10 Americans develop at least one ulcer during their lifetimes. Ulcers affect about five million people each year. More than 40,000 people each year have surgery because of persistent symptoms or problems from ulcers. Each year, nearly 6,000 people die of ulcer-related complications.

What is a peptic ulcer? A peptic ulcer is a sensitive, raw patch, very much like a mouth ulcer, which forms a break in the lining of the stomach or the duodenum. Gastric ulcers occur in the stomach and duodenal ulcers occur in the duodenum, the first part of the intestine after the stomach. How is an ulcer formed? A very important cause of developing an ulcer is a germ or bacterium called Helicobacter pylori , known as H. pylori . This is found in the lining of the stomach of about half of the adults in Britain and it is now certain that having this germ present makes developing ulcers much more likely. Patients with duodenal ulcer almost always have this germ present and at least 80% of gastric ulcer patients also have it. Medical research is actively investigating why it is that only some people who are infected with the germ get ulcers. Who has an ulcer? Ulcers are common in Britain and men are more prone to suffer than women. About one in ten men and one in fifteen women suffer from an ulcer at some time in their lives, but in most people they heal up with treatment. People who have H. pylori

Peptic ulcer
Peptic ulcer refers to an area of the stomach or duodenal lining which becomes eroded by the stomach acid. These are known as stomach and duodenal ulcers, collectively known as peptic ulcers.
Symptoms
You may notice no symptoms although you have an ulcer, but frequently they are associated with indigestion or pain in the upper abdomen or even lower chest. This pain may be worse before or after eating and frequently wakes you in the early hours of the morning. The pain may go to the back or sometimes be perceived elsewhere. If an ulcer bleeds you may vomit blood or partially digested blood (which looks like coffee grounds) or pass black stools (melaena), which contain changed blood, when you go to the toilet. If any of these happen call the doctor immediately (even out of hours).
Causes
Many factors make you more likely to have an ulcer.
  • It often seems to run in families.
  • Smoking increases the risk.
  • Heavy drinking.
  • Certain drugs eg aspirin and other anti-inflammatory drugs
  • We now know that a large percentage of people who have ulcers have a germ (Helicobacter pylori) in their stomach which they probably picked up many years previously, which makes the ulcer more likely to occur and more difficult to heal.
Diagnosis
Your doctor may treat you on the assumption of the diagnosis, but may well arrange either a barium meal X-Ray or a visual inspection using a fibre-optic tube passed down the throat (upper gastrointestinal endoscopy). The endoscopy is preferred by the experts as they can see the lining of the stomach and duodenum and take biopsies which allow them to rule out more serious conditions as well as looking for the presence of Helicobacter pylori.

An ulcer in the lining of the stomach or duodenum, where hydrochloric acid and pepsin are present, is referred to as a peptic ulcer. When the peptic ulcer is located in the stomach, it is called a gastric ulcer . When the peptic ulcer is located in the duodenum, it is called a duodenal ulcer What causes gastric and duodenal ulcers? In the past, it was believed lifestyle factors, such as stress and diet caused ulcers. Later, researchers determined that stomach acids - hydrochloric acid and pepsin - contributed to ulcer formation. Today, research shows that most ulcers (80 percent of gastric ulcers and 90 percent of duodenal ulcers) develop as a result of infection with a bacterium called Helicobacter pylori (H. pylori). It is believed that, although all three of these factors - lifestyle, acid and pepsin, and H. pylori - play a role in ulcer development, H. pylori is considered to be the primary cause in most cases.

    Acid reflux
    Acid reflux is a condition where gastric juices containing acid travel back from the stomach into the oesophagus (swallowing tube). Some 20% of the population experience this problem on a weekly basis and have a condition called gastro oesophageal reflux disease or GORD.
    Symptoms of acid reflux include heartburn (a burning feeling rising from the stomach or lower chest up towards the neck), regurgitation, chest pain, a difficulty in swallowing (dysphagia), hoarseness, dental diseases and asthma.
    The food in the stomach is partially digested by stomach acid and enzymes. Normally, the partially digested acidic content in the stomach is delivered by the stomach muscle into the small intestine for further digestion. In patients with acid reflux, stomach acid content refluxes backwards into the oesophagus, causing inflammation and damage.
