Tuberous Sclerosis (TS), or Bourneville disease, covers a broad range of afflictions, but neurologically afflicts 1/6000 newborns in the United States . Its name derives from the "tuber-like growth, on the brain which calcify with age and become hard or sclerotic" . Brain tubers have no distinction between white and gray matter, as indicated by the arrow in the biopsy.
TS was first observed by von Recklinshausen in 1862, and was named by Bourneville as tuberous sclerosis in 1880; he described it as a "'tuberous sclerosis of the cerebral circumvolutions' for the unique cerebral pathologic changes he found in the brain of a 15 year-old epileptic and mentally handcapped girl" . The patient's face was also scarred with lesions, now know as facial angiofibroma . By the early 1900's TS was made according to evidence of what is known as the Vogt triad, including seizures, mental retardation, and the facial angiofibroma. It is now known that this triad is not necessarily needed to diagnose TS. The current diagnosis is based upon the following: Multiple ungual fibroma

What is Tuberous Sclerosis Complex?
Tuberous Sclerosis Complex (TSC) was discovered in the 1880's by a French physician named Bourneville. TSC, also known as Bourneville's Disease and Epilola, is an incurable multisystem genetic disorder which can have a wide range of effects. Approximately one in 8,000 adults and one in 6,000 newborns are affected by TSC. Although TSC is often inherited, new mutations have been implicated in up to 75% of all cases ( TSA of Greece ). Males and females of equally likely to have Tuberous Sclerosis and the chance of passing it on to offspring is 50%.
Physical Symptoms
The physical symptoms of TSC which often lead to its diagnosis include hamartomaceous lesions of the brain, heart, kidneys, skin, lungs, and eyes, mental retardation, seizures, autism, fibromas of the finger and toenails, pitted teeth, and dermatological lesions. Three symptoms which constitute a positive diagnosis of TSC are seizures, mental retardation, and adenoma sebaceum, which are known as the clinical triad. People with TSC may exhibit only a few or many of these symptoms. Hamartomaceous lesions, or hamartomas, are tumour-like growths which are referred to as tubers. The most common tuber forms of the internal organs are cerebral hamartias and subependymal giant cell astrocytomas of the brain, rhabdomyomas of the heart, and angiomyolipomas of the kidneys (Kwiatkowski and Short, 1994 cited in Henske

  • Cody's Page (Basic Info) Tuberous Sclerosis (TSC) is a genetic disorder that causes benign tumors to form in many different body organs including the brain, skin, heart, kidneys, lungs, and eyes. In any one individual, clinical symptoms will vary depending upon the specific organs affected. While the true prevalence of TSC is not known, it is estimated that TSC occurs in approximately 1 in 10,000 live births. TSC is seen in both sexes and among all racial and ethnic groups. The diagnosis of TSC is made through clinical evaluations of the organ systems involved. A thorough evaluation typically includes careful examinations of the skin, brain, kidneys, heart, and eyes to determine whether any organ involvement has occurred. Additional tests, including ultrasounds, MRI scans, or CT (“cat”) scans may beeeded to confirm a diagnosis. Of the two genes associated with TSC, the TSC2 gene has been located on chromosome 16. Dr. Jonathan Haines is performing mutational analysis of the TSC2 gene and Dr. Vijaya Ramesh has begun characterization of the corresponding protein, tuberin . DNA diagnostic testing for TSC2 mutations is now available for a subset of research families through the clinical laboratory of Dr. Katherine Sims. Dr. Sims also runs the tuberous sclerosis clinic at Massachusetts General Hospital for patients and families affected by this disorder. Dr. Alan Buckler is working on locating the TSC1 gene on chromosome 9 by applying newly developed strategies for physical mapping of human chromosomes and disease gene identification.

    Overview
    The Tuberous Sclerosis Clinic at Children's Hospital Medical Center of Cincinnati, the largest in the United States, was organized in 1993 to assist in the medical care of children and adults who have or are suspected of having Tuberous Sclerosis. The purpose of the Tuberous Sclerosis clinic is not to replace care from the child's pediatrician or family physician, but to assist the primary care doctor in dealing with those aspects of Tuberous Sclerosis that affect the child's health or development. The basis of the Tuberous Sclerosis Clinic is the realization that people with Tuberous Sclerosis are different from other individuals who have epilepsy, learning disabilities, behavior problems, etc. For too long the unique problems found in this disease have been lumped together with similar disorders in children without tuberous sclerosis. Yet, research has increasingly shown that disorders of the brain, learning, heart, kidney, and other organs in Tuberous Sclerosis are different than the same conditions in individuals without Tuberous Sclerosis. The Tuberous Sclerosis Clinic is managed by the Neurology Department at Children's Hospital Medical Center of Cincinnati, and is part of the University of Cincinnati Medical Center.

    The constellation of findings is highly consistent with a diagnosis of tuberous sclerosis. Clinically these patients present with the triad of seizures (90%), mental retardation (50%), and adenoma sebaceum (90%).

    The brains of most tuberous sclerosis patients show tubers (hard, potato-like nodules in gyri) of the cerebral cortex and candle gutterings (irregular calcified nodules) in the lateral, third and fourth ventricles.
    The face often shows angiofibromas on the cheeks and forehead, the heart may have rhabdomyomas and the kidneys may show cysts or angiomyolipomas.

    Clinical triad is adenoma sebaceum, seizures, and mental retardation. Etiology/Pathophysiology:
    A hamartomatous proliferation of fibrous and connective tissue in the brain, skin, skeleton, kidneys, and heart. Pathology:
    The hamartomatous lesions in the brain consist of proliferating glial tissue and ganglion cells. Imaging Findings:
    The intracranial tubers are usually calcified and usually are in a periventricular / subependymal location but they can occasionally be found in the brain parenchyma. The tubers may undergo malignant degeneration into giant cell astrocytomas. DDX:
