Abstract
Background and Objective: The risk of pregnancy in patients with mechanical valve prosthesis has traditionally been attributed to the use of oral anticoagulants. The impact of residual hemodynamic Tricuspid valve disease (TVD) after corrective surgery has not been examined. Our aim was to determine the effect of TVD on fetal and maternal outcome of pregnancy in patients with mechanical valve prosthesis.
Methods: We examined the data of 101 pregnancies in 69 females with mechanical valve prosthesis. Their ages ranged from 19 to 45 years. Fourteen females with 21 pregnancies suffered from TVD severe enough to necessitate diuretic use (Alductone 100-150 mg daily and/or Frusemide 40mg).
Results:
Conclusion: The presence of TVD adversely affects the fetal outcome of pregnancy in patients with mechanical valve prosthesis. Warfarin use did not show a significant impact. Fetal loss may be due to placental insufficiency because of systemic congestion or reduced placental blood flow induced by diuretic use.

    (Kardiol. Pol. 2002, 56, 217) INTRODUCTION An organic tricuspid valve stenosis is very rarely encountered, particularly as an isolated disorder. This anomaly almost always coexists with mitral or aortic valve disease and is most commonly caused by rheumatic fever. A significant tricuspid valve disease leads to progressive heart failure, cachexia and death. Medical therapy is not effective. During surgery, a diseased tricuspid valve is replaced by bioprosthesis or a mechanical valve. In patients with coexistent mitral or aortic valve disease, a simultaneous replacement of another diseased valve is performed. We present a relatively young patient with a long-standing critical tricuspid stenosis with subsequent enormous right atrial enlargement. Because of a progressive heart failure, unresponsive to pharmacological therapy, high-risk surgery was successfully performed.

    Tricuspid valve disease refers to damage to the tricuspid heart valve. This valve is located between the atrium (upper chamber) and the ventricle (lower pumping chamber) of the right side of the heart. The tricuspid valve has three cusps, or flaps, that control the direction and flow of blood. The two main types of tricuspid valve disease are:

Valve Disease Every time your heart beats, blood flows into, through, and out of your heart. In fact, your heart moves about 100 gallons (379 liters) of blood through your body every hour. Blood is pumped through your heart in only one direction. Heart valves play key roles in this one-way blood flow, opening and closing with each heartbeat. Pressure changes behind and in front of the valves allow them to open their flap-like "doors" (called cusps or leaflets) at just the right time, then close them tightly to prevent a backflow of blood. There are 4 valves in the heart:
    • Tricuspid valve Pulmonary valve Mitral valve Aortic valve
    Blood without oxygen returns from the body and flows into the heart's upper-right chamber (the right atrium). From there, it is forced through the tricuspid valve into the lower-right chamber (the right ventricle). The right ventricle pumps the blood through the pulmonary valve and into the lungs. While in the lungs, the blood picks up oxygen. As the right ventricle is preparing to push blood through the pulmonary valve, the tricuspid valve closes to stop blood from flowing back into the right atrium.

    WHAT IS TRICUSPID VALVE DYSPLASIA (TVD)?
    To understand tricuspid valve dysplasia (TVD), it is first important to know a bit about the anatomy of the canine heart. The canine heart, like the human heart, is divided into two halves. The right side of the heart receives oxygen depleted blood from the tissues of the body and pumps this blood into the lungs through the pulmonary artery. After becoming oxygenated in the lungs, the blood returns to the left side of the heart through the pulmonary veins where it is then pumped to the tissues of the body by way of the aorta.
    Each half of the heart is subdivided into an upper and lower chamber. The upper chamber, called the atrium, receives the blood and then passes the blood to the lower chamber called the ventricle. The ventricle then pumps the blood to its destination. To prevent backflow of the blood from the ventricle to the atrium, the heart is equipped with valves that allow blood to flow in only one direction. There are two atrioventricular valves which control blood flow between the atria and the ventricles: the tricuspid valve which is located between the right atrium and right ventricle and the mitral valve which is located between the left atrium and left ventricle.
    In the canine, the tricuspid valve is made up of two irregularly shaped flaps composed of connective tissue. The tissue of the flaps is connected to the papillary muscle of the ventricle by tendon-like cords called chordae tendineae. As the right atrium contracts, blood passes through the tricuspid valve into the right ventricle. When the ventricle contracts, some blood flows backward, pushing upward against the flaps of the valve causing them to meet and form a barrier which prohibits the blood from re-entering the upper chamber. The chordae tendineae prevent the flaps from opening backward into the atrium which would break the seal and thus allow backflow of the blood.

    Types of Valve Disease
    Diseases of the heart valves are grouped according to which valve or valves are involved and the amount of blood flow that is disrupted by the problem. The most common and serious valve problems happen in the mitral and aortic valves. Diseases of the tricuspid and pulmonary valves are fairly rare. Diseases of the Mitral Valve Mitral Valve Prolapse The mitral valve regulates the flow of blood from the upper-left chamber (the left atrium) to the lower-left chamber (the left ventricle). Mitral valve prolapse (MVP) means that one or both of the valve flaps (called cusps or leaflets) are enlarged, and the flaps' supporting muscles are too long. Instead of closing evenly, one or both of the flaps collapse or bulge into the atrium. MVP is often called click-murmur syndrome because when the valve does not close properly, it makes a clicking sound and then a murmur. What causes MVP?
