What is tricuspid valve disease? The tricuspid valve is located between the right atrium (top chamber) and right ventricle (bottom chamber). It's role is to make sure blood flows in a forward direction from the right atrium to the ventricle. Tricuspid valve disease refers to abnormal function of the tricuspid valve. Two types of tricuspid disease include:
  • Tricuspid regurgitation - the valve is leaky or doesn't close tight enough, causing blood to leak backwards across the valve Tricuspid stenosis - the valve leaflets are stiff and do not open widely enough, causing a restriction in the forward flow of blood. Your physician may refer to this as an increased pressure gradient across the valve, found by echocardiogram or cardiac catheterization.

    TRICUSPID REGURGITATION

    Retrograde flow of blood from right ventricle to right atrium due to inadequate apposition of the tricuspid valves.
    Symptoms, Signs, and Diagnosis
    In addition to low output symptoms (eg, fatigue, cold skin, dyspnea, edema), the only specific symptom of severe TR is the sensation of pulsations in the neck due to the high jugular regurgitant (CV) waves from the transmitted RV pressure. Right upper quadrant abdominal discomfort due to hepatic congestion may occur. Atrial fibrillation or flutter, which usually occurs when the right atrium enlarges, further decreases the CO and may precipitate sudden, severe heart failure. The jugulars may have various degrees of v wave and y descent, depending on the degree of TR, which is associated with various degrees of systolic pulsation of the liver synchronous with the v waves. With gross TR, there is often a systolic murmur and thrill over the right jugular. A pansystolic TR murmur may be high-pitched if trivial and secondary to pulmonary hypertension, or of medium frequency if rather gross and primary. It often increases on inspiration (Carvallo's sign). Usually, it is best heard at the 4th and 5th intercostal spaces close to the sternum or in the epigastrium, but it is loudest at the apex if the RV usurps that area and over the free edge of the liver if secondary to cor pulmonale from emphysema.

    The tricuspid valve lies between the right atrium and the right ventricle. It prevents backflow of blood from the ventricle into the atrium. The valve may become damaged or diseased causing stenosis and/or regurgitation of the valve. Both of these complications may prevent the valve from functioning correctly.

    The tricuspid valve may become narrowed or 'stenotic' causing the valve not to open fully. This will restrict the flow of blood into the right ventricle, which in turn restricts the flow of deoxygenated blood to the lungs.

    How is Tricuspid Valve Disease diagnosed?
    • Listening through a stethoscope - When blood passes through a narrowed or leaky valve, the resulting noise is a heart murmur that can be heard through a stethoscope when placed on the chest. An electrocardiogram (ECG) will identify thickening of the right atrium and any arrhythmias such as atrial fibrillation if mitral valve disease is also present. An echocardiography (Echo) confirms severity of regurgitation and right atrium enlargement. It may yield additional information as to the cause of the disease. Chest X ray will show enlargement of the atrium caused by its thickening and dilatation and evidence of pulmonary congestion. Cardiac catheterisation (angiogram) can detect both stenosis and regurgitation of the valve. It is also carried out to detect any coexisting coronary artery disease that may be evident by the symptoms of angina.

    Tricuspid valve disorders most often occur in conjunction with other heart valve problems, particularly with mitral valve disorders.     In this relatively rare condition, the tricuspid valve fails to open as wide as it should, reducing the amount of blood entering the right ventricle. The tricuspid valve's three leaflets and related structures become stiff, develop scar tissue, or partially fuse together. Tricuspid valve stenosis is most often due to injury to the leaflets caused by medications or rheumatic heart valve disease. The narrow heart valve must accommodate the same amount of blood, resulting in increased pressure in the right atrium. The right atrium compensates for this increased pressure buildup by stretching. Less blood is pumped to the lungs. Patients may experience pain in the upper right abdomen and enlargement of the liver. Eventually, tricuspid valve stenosis can damage the heart muscle and can cause irregular heartbeats and possibly heart failure or other complications, including stroke or heart infection.
