Treatment recommendations Fungal infections are caused by microscopic organisms (fungi) that can live on the skin. They can live on the dead tissues of the hair, nails, and outer skin layers. Fungal infections include mold-like fungi (dermatophytes, which cause tinea infections) and yeast-like fungi (such as Candida Heat and humidity bring excess moisture and sweating to our bodies, creating the perfect environment for fungi to grow or fungal infections to develop. Fungi tend to thrive in warm, moist places, such as between the toes, in the groin, under the breasts, and other parts of the body. Cutaneous candidiasis is called Intertrigo when occuring under heavy skin folds.
Fungi are yeasts and molds that can infect and live in the topmost layer of the skin. Fungal infections of the skin may affect the feet, groin, scalp, nails and other parts of the body. Some fungal infections can be difficult to recognize because they may cause no symptoms or only cause slight redness and dryness. Other fungal infections may cause itching, swelling, blistering and severe scaling or dry flaky skin.
One of every five persons gets a fungal infection at some time. You can get a fungal infection by touching a person who has one. Some types of fungus live on damp surfaces such as the floors in public showers or locker rooms. You can even catch a fungal infection from one of your pets. Sometimes, the clothes we wear encourage fungi to develop, such as tight-fitting socks or close-fitting clothing.

Today's Answer (Published 08/07/2000) Ringworm hasn't got anything to do with worms. It is a common fungal infection that you can pick up from other people and from dogs and cats. These infections, caused by the tinea fungus, typically affect children but adults can get them, too. A ringworm is a pinkish red rash that develops on the skin and spreads in a circle that rings a center of normal looking skin. The edge of the circle is raised and looks something like a worm under the skin. Tinea skin infections on the trunk of the body, or on the scalp, are called ringworm, but tinea infections on the feet are known as athlete's foot; and when tinea affects the groin area, it's called "jock itch." Ringworm is diagnosed on the basis of its distinctive appearance, but because it sometimes can be mistaken for other skin conditions, doctors usually scrape off a few scales for examination under the microscope. The infections rarely cause serious problems, although people with weakened immune systems may have some trouble getting rid of them.

Fungal infection of the nails is known as "onychomycosis". It is increasingly common with increased age. It rarely affects children.

Onychomycosis can be due to:
  • Dermatophytes such as Trichophyton rubrum (T rubrum), T. interdigitale. The infection is also known as tinea unguium. Yeasts such as Candida albicans. Moulds especially Scopulariopsis brevicaulis.

The term "tinea" is a general name referring to a group of related fungal skin infections. Tinea can affect most skin sites, depending on the specific fungal type. Tinea pedis is the medical name for "athlete's foot"; Tinea cruris is "jock itch"; Tinea capitis is "ringworm" of the scalp; and tinea corporis is "ringworm" of the body and nails. These descriptive terms refer to the location of the infection and not which specific type of fungus is involved. Fungal infections of the scalp may appear as patchy areas with dandruff-like scaling and hair loss (with broken stubbles of hair). Sometimes, there may be a swollen, inflammed mass known as a "kerion" which may be confused with impetigo or cellulitis . The classical features of tinea are itching, redness on the skin and a circular patchy lesion that spreads along its borders, at the same time clearing at the center. In time, it may appear as a ring or a series of rings around a clear center, hence its name "ringworm." This pattern may not always be seen in every infected person. Additional signs or symptoms depend on which site is infected, and how advanced the disease is. On the palms or soles, redness may be the only sign; sometimes, on the soles, there may be deep seated blisters which, over time, dry and end up as brown crusts. Between the toes there may be thick, white scaling. Nails can turn thick and white, and eventually crumble if untreated. On the scalp, there may be hair loss as the hairs break off at their shafts - "black dot ringworm."

Superficial fungal infections caused by a dermatophyte (species of fungi belonging to the genera Trichophyton, Microsporum, or Epidermophyton ) are referred to as "tinea." Dermatophytes digest and invade keratin and may infect skin, nails, and hair. Infections may be acquired through human-to-human (anthropophilic) spread, animal-to-human (zoophilic) spread, or soil-to-human (geophilic) spread. The latter two categories tend to produce a more severe cutaneous inflammatory response in immunocompetent hosts. Infections of scalp hair and general body surfaces are most frequent during childhood; hand, foot, or nail infections are much more common after puberty. Dermatophyte infections are more common in association with hot, humid environments, sweating or maceration of the skin, occlusive footwear, use of hair greases or oils, diabetes mellitus, and defects of cellular immunity (e.g., AIDS). The various clinical presentations of dermatophyte infections are designated by use of the term tinea followed by the Latin term for the affected body area. What causes it?
Tight-fitting trainers are a common cause of athlete's foot. Athlete's foot is a fungal infection of the foot caused by parasites on the skin called dermatophytes. Dermatophytes can be divided into three groups according to their favourite hosts:
  • fungi preferring soil (geophile)
    fungi preferring animals (zoophile)
    and fungi preferring humans (anthropophile).
Athlete's foot is usually caused by anthropophile fungi. The most common species are Microsporum, Epidermophyton and Trichophyton. These account for 90 per cent of all skin fungal infections, commonly referred to as ringworm

TINEA (S KIN FUNGUS) Tinea is the name given to a fungal skin infection. Most people will develop some resistance to skin fungus after being infected. Others appear to have a susceptibility to fungal infections. Sometime the susceptibility will run in the family. Tinea pedis (athlete's foot) This is the commonest type of fungal infection. It is the most common and most stubborn cause of tinea pedis and only affects humans. It is spread by direct contact, most often through bare feet in bathrooms and health clubs. Leather or plastic footwear that doesn't "breathe" encourages tinea pedis. It is rare in children. In most cases, the skin be comes white, soft and peels away between the toes (especially between the fourth and little toes). It may infect the sole of the foot. resulting in peeling, scaling, itching and sometimes blistering. Only one, or both feet may be involved. Onychomycosis (Tinea Ungium of the nail) Toe nail infection is usually associated with tinea pedis. It is very difficult to eradicate. Often the great toenail is the first to show signs, especially if it has been injured. The nail yellows, and after years thickens and breaks easily. Finger nail infections are similar, but less common.

The practicing allergist may be called upon to treat this disease for various reasons. Sometimes tinea infection may be mistaken for atopic dermatitis or allergic eczema. In other patients, tinea pedis may complicate allergy and asthma and may contribute to refractory atopic disease. Patients with recurrent cellulitis may be referred to the allergist/immunologist for an immune evaluation and discovered to have tinea pedis as a predisposing factor. From a molecular standpoint, superficial fungal infections may induce a type2 T helper cell response (Th2) that can aggravate atopy. Th2 cytokines may induce eosinophil recruitment and immunoglobulin E (IgE) class switching by B cells, thereby leading to exacerbation of atopic conditions.
