TINEA - Full extract Karl is about 70 and loves his daily walk. He hasn't changed a jot in the 15 years I've known him. One morning he came in complaining of itchy sore feet and toes. When he removed his sneakers and socks, the problem was fairly evident. The skin between his toes was very moist and broken in places, especially between the fourth and fifth toes of each foot. The tops of his toes also looked rather inflamed, as if he had been scratching them. There seemed little doubt Karl had a tinea infection and I prescribed an anti-fungal cream. I also suggested he keep his sneakers off for a while and take extra care drying in between his toes. When he came in for a flu shot recently I had another look at his feet. They were considerably improved. I suggested he try dusting powder between his toes to prevent the condition recurring. Tinea is a group of fungal infections that have a special affinity for the skin, nails and hair. They are very common, not always obvious, and sometimes difficult to treat. Certain types of tinea have a preference for areas of the body not exposed to light and air, for instance around the groin and between the toes. Excessive perspiration can be a factor; must fungal infections are more common in humid conditions. Some people are also more prone to tinea than others.

If you have this infection, you may see small, red spots that grow into large rings almost anywhere on your arms, legs or chest. When a fungus grows in the moist, warm area of the groin, the rash is called tinea cruris. (Cruris comes from the Latin for leg.) The common name for this infection is "jock itch." Tinea cruris often occurs in men, especially if they often wear athletic equipment. Tinea capitis, which is usually called "ringworm," causes itchy, red areas, usually on the head (capitis comes from the Latin for head). The hair is destroyed, leaving bald patches. This tinea infection is most common in children. Tinea pedis is usually called "athlete's foot" (Pedis is a Latin word for foot). The fungus lives on the skin and breeds under warm, moist conditions. Athlete's foot is more common during hot weather and sweaty footwear is usually the culprit. Adult males suffer most often from this condition. The fungus most commonly attacks the feet because shoes create a warm, dark, and humid environment which encourages fungus growth.

Definition: Tinea corporis is an infection of the body surface by mold-like fungi called dermatophytes.

Causes, incidence, and risk factors: The body normally hosts a variety of microorganisms, including bacteria, mold-like fungi (dermatophytes), and yeast-like fungi (such as Candida). Some of these are useful to the body. Others may multiply rapidly and cause symptoms. Tinea corporis (often called ringworm of the body) is a common skin disorder, especially among children, but it may occur in people of all ages. It is caused by mold-like fungi (dermatophytes). See also tinea capitis (involving the scalp), tinea cruris (jock itch), and tinea pedis (athlete's foot).
Fungi thrive in warm, moist areas. Susceptibility to fungal infection is increased by poor hygiene, prolonged wetness of the skin (such as from sweating), and minor skin or nail injuries.
Tinea
infections are contagious and can be passed by direct contact with affected individuals or by contact with contaminated items such as combs, clothing, shower, or pool surfaces. They can also be transmitted by contact with pets that carry the fungus (cats are common carriers).

    Background: The dermatophytes are a group of fungi (ringworm) that invade the dead keratin of skin, hair, and nails. Several species of dermatophytes infect humans; these belong to the Epidermophyton, Microsporum, and Trichophyton genera. Dermatophytosis (tinea) is a fungal infection caused by dermatophytes. The infection may spread from person to person (anthropophilic), animal to person (zoophilic), or soil to person (geophilic). The most common of these organisms are Trichophyton rubrum, Trichophyton tonsurans, Trichophyton interdigitale and/or Trichophyton mentagrophytes, Microsporum canis, and Epidermophyton floccosum Pathophysiology: Dermatophytes have the ability to invade keratinized tissue (eg, hair, nails, any area of the skin) but are restricted to the dead cornified layer of the epidermis. Humid or moist skin provides a very favorable environment for the establishment of fungal infection. Clinically, tinea infections are classified according to the body region involved.
