Thyroid underactivity (Hypothyroidism)
Underactivity of the thyroid gland (hypothyroidism) can arise for a number of reasons. Hypothyroidism can occur in newborn infants (Congenital Hypothyroidism, CHT) as well as later on in childhood (acquired Hypothyroidism, AHT.) Affected newborns look, and usually act, normal for the first few days and the national screening programme for CHT, by blood spot collection, ensures that affected infants are identified and treated as early as possible. The commonest cause of CHT is a poorly developed or absent thyroid gland; the underlying basis to this abnormality is not understood. CHT can also occur if there is a defect in thyroxine production within the gland. These defects may be permanent or transient. Although every child with CHT should be started on thyroxine promptly, they should be evaluated at some time later to exclude a transient problem. The commonest causes of acquired hypothyroidism include an immune reaction against the thyroid gland by the body itself (Autoimmune Hypothyroidism, Hashimoto's disease) or damage to the thyroid gland or the part of the brain which controls the thyroid.

We're sick of hearing that there are no harmful effects of phytoestrogens. Phytoestrogens disrupt thyroid function. (Read the British Food Standards Agencies Committee on Toxicity Report Here) Theodore Kay of the Kyoto University Faculty of Medicine noted in 1988 that 'thyroid enlargement in rats and humans, especially children and women, fed with soybeans has been known for half a century'. In fact, thyroid problems associated with soy were also well known to bird-breeders , which is how Soy Online Service first became aware of the goitrogenicity of soy. Well known, but that fact seemed to escape manufacturers of the first commercially available soy formulas. Those formulas were known to cause in goitre in infants and one can only wonder how many other infants were left hypothyroid or suffering from permanent thyroid damage by soy formulas. The iodine levels in soy formulas were increased and instances of goitre in infants fed these products ceased. However, there appears to have been no attempt to isolate or remove the goitrogenic properties from soy formulas. This is of grave concern because, although elevated iodine levels would have helped to nullify the effects of the goitrogens, the goitrogens would still have been actively suppressing thyroid function in infants. Hence millions of babies (particularly in the United States where soy formula feeding is most common) have needlessly been exposed to goitrogenic agents; Soy Online Service believe that infants fed soy formulas unnecessarily risk abnormal thyroid function and a greater risk of thyroid disease in later life.

One of the best diagnostic tools your doctor has available is simply to feel or “palpate” your thyroid gland. This is especially helpful in determining if your gland is enlarged or if nodules (lumps) are present.
Your doctor also will check for disorders of the thyroid by means of a blood test. Your blood can be tested for levels of thyroid hormone (T4), thyroglobulin (produced by normal thyroid tissue and cancer cells), thyroid stimulating hormone (TSH) and antibodies created by your immune system.

A goiter is a swelling of the thyroid gland. Oftentimes the swelling can be seen or felt by you or your doctor. However, in cases in which there is little swelling, a goiter can go undetected.
To determine the cause of your goiter, your doctor may perform a blood test to identify the level of thyroid hormone (T4) in your bloodstream as well as the level of thyroid stimulating hormone (TSH). These tests can determine if your goiter is being caused by an underactive (hypothyroid) or overactive (hyperthyroid) thyroid gland.

Functional Thyroid Disorders, Part I
by David Brady,DC,CCN,DACBN Thyroid disorders are among the most commonly encountered metabolic disorders in private practice. The strong tendency for thyroid disorders to cause musculoskeletal symptoms make the understanding of these disorders even more critical to the practicing chiropractor. In this two-part article, I will attempt to provide the reader with an overview of thyroid disorders, along with suggestions on proper diagnosis and treatment. I will explain why conventional medicine does not recognize many cases of thyroid disease, and why many patients diagnosed with thyroid disease do not respond to conventional therapy. Part I will provide information on hypothyroidism, with Part II providing information on hyperthyroidism and thyroid cancer. A follow-up article on fibromyalgia will discuss the current state of research and treatment of this complex metabolic disorder, including the strong role that thyroid dysfunction plays in many cases of fibromyalgia and fibomyalgia-like disorders. The thyroid gland is located in the anterior neck region and normally weighs approximately 15-20g. It consists of two lobes: one on either side of the trachea in the lower neck, and a connecting portion called the isthmus, giving the entire gland an H-shaped appearance. The thyroid gland is responsible for synthesizing several hormones that have vast effects on overall body metabolism. It is unique among endocrine glands in that large amounts of hormones are created and stored in the thyroid and then released very slowly.

In the March 20 issue, I explored hypothyroidism in detail. In this second and final part, I'll discuss hyperthyroidism and thyroid cancer. Hyperthyroidism (thyrotoxicosis): Like hypothyroidism, there are many subtypes of hyperthyroidism. The most common form is Grave's disease. Grave's disease is an autoimmune disorder of the thyroid, resulting in thyroid enlargement, hyperactivity, and detectable serum antibodies to fractions of the thyroid gland. The disease is more prevalent in females (8:1), with an average age of onset of between 20 and 40 years of age. Other causes of hyperthyroidism include: subacute thyroditis (possibly due to viral infection); autonomous toxic thyroid adenomas (hyperfunctioning thyroid nodules); Jod-Basedow disease (iodine-induced and commonly seen as a result of multiple radiographic contrast studies or drugs); pituitary (TSH hypersecretion by the pituitary); thyrotoxicosis factitia (secondary to over-medication with thyroid hormone replacement medication or consumption of ground beef contaminated with bovine thyroid tissue); and Hashimoto's thyroiditis (an auto-immunity disorder affecting the thyroid).
