PHLEBITIS AND thrombosis. Basically it means a clot in the vein. Superficial vein thrombosis or SVT is also called superficial thrombophlebitis/phlebitis.

PHLEBITIS AND THROMBOSIS
Basically it means a clot in the vein. As the venous system is divided in deep veins and superficial veins one can have clot in each of the systems. Only very rarely clotting can occur in both of the systems in the same time. Superficial vein thrombosis or SVT is also called superficial thrombophlebitis/phlebitis. The "itis" denotes that the vein is engulfed in an inflamatory process: the skin around the vein is read and painful. Swelling can be present as well. Unfortunately, many physicians treat this condition with antibiotics. This is completely unnecessary as in most of cases there is no bacterial infection. The main cause for the condition are varicose veins. Blood stagnates in those venous pools and will clot easily especially during inactivity. Injecting varicose veins can lead to superficial phlebitis as well. In the absence of varicose veins one should look for other reasons such as malignancy elsewhere in the body, gout, Buerger's disease. or an inherited tendency to clot. ( See FAQ The treatment of the condition is simple: evacuation of clot if the patient comes early to the physician, local compression with ace bandages or surgical stockings, and ambulation. The only medication to be given is ibuprofen (Motrin/Advil) 5-7 days that will ease the pain and calm the inflamatory reaction. Occasionally SVT can be associated with DVT. Only in these cases is anticoagulation treatment necessary. If the underling condition for the superficial phlebitis was varicose veins after the inflamatory process has cooled off, surgery for the varicose veins is indicated.

Factor V Leiden and Venous Thrombosis Overview V enous thrombosis is an important cause of morbidity; incidence is low in young people but increases with age to 1% per year in the elderly. In a small proportion of cases, venous thrombosis leads to pulmonary embolism, which can be fatal. Persons with an initial venous thrombosis are at increased risk for recurrence; however, long-term use of anticoagulant prophylaxis can result in major hemorrhagic complications. This challenging clinical problem has received new attention since the discovery of certain genetic variants that increase susceptibility to venous thrombosis.

Evans syndrome is defined as the simultaneous or sequential occurrence of Coombs' positive hemolytic anemia and immune thrombocytopenia without known underlying etiology. We are reporting a case of Evans syndrome in which the patient had recurrent thrombosis with elevated anticardiolipin antibodies. To our knowledge, this is the first case of such an association to be reported.
Case Report
The patient improved although it was noted that his left leg had been markedly swollen throughout this period, presumably as a result of thrombus extending to the left iliofemoral vein. Thus far, a lupus inhibitor could not be demonstrated. The patient was started and maintained on warfarin as the heparin and steroids were tapered. He became extremely mobile and was fitted with an artificial leg. The CBC showed a Hb of 123 g/L, WBC 6.9x10-9/L and platelet count 252x10-9/L. He was discharged on warfarin and Persantin.
Discussion
Our patient presented with Coombs' positive hemolytic anemia and immune thrombocytopenia but no evidence of underlying cause verified the diagnosis of Evans syndrome.1,2 In addition to these hematologic disorders, the

Deep Veins Deep venous thrombosis (DVT) affects mainly the veins in the lower leg and the thigh. It involves the formation of a clot ( thrombus ) in the larger veins of the area. This thrombus may interfere with circulation of the area, and it may break off and travel through the blood stream (embolize). The embolus thus created can lodge in the brain, lungs, heart, or other area, causing severe damage to that organ.

There is increasing suspicion amongst the travelling public and the media of an association between the occurrence of venous thromboembolism (VTE) and air travel. However VTE can occur with other forms of travel and hence the term travellers' thrombosis. Doctors must be aware of the risk factors for travellers' thrombosis, and if appropriate discuss the need for prophylaxis with at-risk patients who are contemplating long-distance travel. The effectiveness of aspirin or low molecular weight heparin as prophylaxis in moderate and high-risk groups is theoretical at this stage. It would be prudent to advise all travellers about lower leg exercises, hydration, and the symptoms of VTE. The added risk of VTE after long-distance travel is unknown Travellers' thrombosis rather than economy class syndrome
Pre-existing risk factors contribute to DVT development and are independent of travel

The minimum prophylactic recommendation is adequate hydration and mobilisation
