Evaluation of Recurrent Thrombosis and Hypercoagulability

The primary mechanisms for coagulation were described more than a century ago. However, inherited disorders of coagulation have been recognized for only a few decades. Activated protein C resistance and other related protein defects account for many cases of hypercoagulability. Primary care physicians can initiate testing in individuals for whom there is a high degree of suspicion. Testing for specific disorders is best performed before initiation of anticoagulant therapy. Management often includes long-term warfarin therapy following initial anticoagulation with heparin. One hundred fifty years have passed since pathologist Rudolph Virchow identified three factors that are responsible for vascular thrombosis: vessel injury, alteration in blood flow and changes in the coagulability of blood. Virchow's triad, as it has become known, has remained accurate. Despite the simplicity of these concepts, the evaluation of a patient with excessive thrombosis often remains perplexing. TABLE 1

The Hemophilia and Thrombosis Center of Nevada (HTCN) was founded in 1997 as the first treatment center in the state of Nevada dedicated to the diagnosis and treatment of patients with bleeding or clotting disorders. The HTCN is a non profit corporation. We support ourselves with educational grants and research grants. All team members are employed through community organizations who donate their time to the treatment center. We currently care for patients and families throughout Nevada and the neighboring underserved regions. While clinics are held in Las Vegas, we attempt to coordinate care through the primary care physician's office locally. We have an active advisory board composed only of affected and concerned persons. This board promotes the mission of the HTCN and helps to guide the activities of the treatment center staff. If you are interested in volunteering at the HTCN or helping support us, please call the center at the number below. We always need help with our newsletters published three times per year and special events.
Our Mission
The HTCN is dedicated to improving the care of patients with bleeding or clotting disorders regardless of the ability to pay.

A deep vein thrombosis (DVT) is a blood clot (thrombus) that develops in a deep vein, usually in the leg. This can happen if the vein is damaged or if the flow of blood slows down or stops. About one in 2,000 people in the UK develops a DVT each year. ... other circulation or heart problems

Problems caused by a DVT
A DVT below the knee is unlikely to cause complications and may only need to be monitored. But when a clot forms in or above the knee, there is a risk that it will break away and travel up the vein to block a blood vessel in the lung. This is called a pulmonary embolism (PE). Depending on the size of the clot, it can be a life-threatening condition. But with appropriate treatment, it is rare for a DVT to lead to a pulmonary embolism. A DVT can damage the valves in the vein, so that instead of flowing upwards, the blood pools in the lower leg. This is called post-thrombotic syndrome, and can result in pain, swelling, discolouration and sores on the leg.

Thrombosis
    A blood clot a.k.a. as thrombus is the result of a reactions involving various proteins, their activators, and inhibitors. The process of blood clotting also involves the blood vessel wall and cellular components of blood (particularly platelets). Clotlysis (fibrinolysis) occurs via a complex series of reactions as well as a variety of tissue components. Thrombosis of a vein is a clot made up of red blood cells, white blood cells and platelets. The clot sticks to the lining of the vein and may partially or totally block the blood. Thrombosis is the basis of a number of disorders. Clots can develop anywhere in the the body, and can involve the organs. If clotting occurs in the surface veins, it is usually not dangerous and often heals on its own. If both the surface and deep veins are involved, it may be dangerous. Heart attacks and strokes are very often due to thrombosis. The morbidity and mortality associated with clot formation is staggering. There are an estimated 5 to 20 million cases of venous thrombosis in the United States each year. Approximately 40% of the patients with deep vein thrombosis suffer pulmonary embolus which is fatal in 30% of cases. Nearly 50% of patients with deep vein thrombosis suffer sequelae lasting many years, requiring millions of dollars to treat.

WHO probes deep vein thrombosis and world travel
(CNN) International medical experts and officials from 16 major airlines across the globe are taking part in talks about deep vein thrombosis and air travel at meetings today and Tuesday at the World Health Organization headquarters in Geneva, Switzerland. The talks, which are closed to the public, will include reviews of scientific evidence concerning the condition, which has been linked to the low mobility fostered by long-haul flights and other forms of protracted travel in cramped spaces, such as automobile and train trips. "It's the first time that airlines of the world have gotten together under the auspices of the WHO to talk about a problem that has been booming in the past year or so," Jarnail Singh, chairman of Singapore Airlines' civil aviation medical board, told Reuters, calling the sessions "very important." Deep vein thrombosis, also called traveler's thrombosis and DVT, is caused by blood clots that can form when blood pools most often in the legs during long periods of inactivity. The danger occurs when these clots break off and travel to the lungs, brain or heart, causing heart attack and stroke.
