About Your Diagnosis
Thromboangiitis obliterans is an inflammatory disease of the small and medium-sized arteries of the arms and legs. This inflammatory process leads to scarring and obstruction of the artery and loss of blood flow to the affected extremity. Thromboangiitis obliterans is also known as Buerger's disease. This disease is extremely rare in women and is typically found in men before the age of 50 years. Thromboangiitis obliterans is an infrequent cause of "poor circulation" and is found in 12 people per 100,000 population in the United States. The prevalence is much higher in Japan, India, and Southeast Asia. The cause of thromboangiitis obliterans is not known; however, the disease is found solely in tobacco smokers. The disease progresses if one continues to smoke and will improve if one abstains from smoking.

OVERVIEW:
Occlusion of small and medium sized arteries and veins caused by inflammatory changes in these vessels. It primarily occurs in men who smoke.
System(s) affected: Cardiovascular
Genetics: Greater prevalence of HLA-A54, HLA-A9 and HLA-B5. Familial cases reported rarely.
Incidence/Prevalence in USA:
Predominant age:
20 to 40 years
Predominant sex: Male > Female (3:1). Increasing numbers of women are being diagnosed, presumably due to increased smoking.
SIGNS AND SYMPTOMS:
Symptoms tend to wax and wane in early disease and are often asymmetric. Symptoms may be gradual or have a sudden onset related to impaired vasculature.

Essentials of Diagnosis
" Almost always in young men who smoke. " Extremities involved with inflammatory occlusions of the more distal arteries, resulting in circulatory insufficiency of the toes or fingers. " Thromboses of superficial veins may also occur- " Course is intermittent and amputation may be necessary, especially if smoking is not stopped.
General Considerations
Buerger's disease is an episodic and segmental inflammatory and thrombotic process of the arteries and veins, principally in the limbs. The cause is not known. It is seen most commonly in men between ages 25 and 40 who smoke. The effects of the disease arc almost solely due to occlusion of the arteries. The symptoms are primarily due to ischemia, complicated in the later stages by infection and tissue necrosis. The inflammatory process is intermittent. with quiescent periods lasting weeks, months, or years. The arteries most commonly affected are the plantar and digital vessels in the foot and those in the lower leg. The arteries in the hands and wrists may also become involved. Different arterial segments may become occluded in successive episodes: a certain amount of recanalization occurs during quiescent periods. Superficial migratory thrombophlebitis is a common early indication of the disease.
Clinical Findings
The signs and symptoms are primarily those of arterial insufficiency, and the differentiation from arteriosclerotic peripheral vascular disease may be difficult; however, the following findings suggest Buerger's disease:

Thromboangiitis obliterans, also known as Buerger's disease, is a nonatherosclerotic inflammatory disease of the small-sized and medium-sized arteries and veins of the arms and legs. Although inflammatory by nature, it spares the blood vessel walls. Unlike other types of vasculitis, the acute-phase reactants (such as the erythrocyte sedimentation rate and C-reactive protein) are normal in patients with Buerger's disease. A pathologic finding of acute disease is an occlusive, highly cellular, inflammatory thrombus. The disease is found worldwide, but the highest incidence of thromboangiitis obliterans occurs in the Middle East and Far East. Prevalence ranges from about 12 percent in the United States to 63 percent in India to 80 percent among Jews of Ashkenazi ancestry living in Israel. Part of this difference in disease incidence may be due to variability in diagnostic criteria. Olin reviewed the current concepts of this disease. Although the cause of Buerger's disease is unknown, the strongest association is with tobacco use. Occurrences of the disease have been reported, but uncommonly, in persons who use smokeless tobacco or snuff. A current or previous history of smoking is usually a required diagnostic criterion.

Prevention Alternative names: Buerger's disease Definition: A disorder that leads to obstruction of the blood vessels of the hands and feet. Causes and Risks Vasculitis (an aggressive form of inflammation) targets the lining of the small arteries and veins, causing thromboangiitis obliterans (Buerger's disease). The blood vessels of the hands and feet become constricted or totally obstructed because of the inflammation and clots. This reduces the availability of blood to the tissues, causing pain and eventually damaging or destroying the tissues, and making them more prone to infections and gangrene.
