Pathophysiology

Thoracic Outlet Syndrome
Thoracic Outlet Syndrome is a very complex condition involving muscle, connective tissue, bones, blood vessels and nerves throughout the entire upper body. Because of its complexity and the variety of body conditions that can contribute to its symptoms, it is arguably the most complex repetitive strain injury of all. The development of symptoms of Thoracic Outlet Syndrome is completely dependent on the posture of the upper body. Numbness, pain, and interference with blood flow throughout the entire upper body are characteristic of this condition. Because we depend on adequate blood flow and use the muscles and bones of the upper body in almost anything we do, such distressing symptoms can be especially detrimental to the quality of life. Once you understand the anatomy of the thoracic outlet area of the body, solutions can be developed. The neck vertebrae, the first rib and the collarbone must all be in the correct relationship with each other to allow enough room for the brachial plexus, the main nerve trunk that serves the upper torso and arm, to pass without interference. In addition to the bones, the muscles of the upper body also play a pivotal role in the development of this condition. If the powerful muscles of the scalene group on the side of the neck become strained or shortened, they can compress the joint spaces between the neck vertebrae, as well as lift the position of the first rib. Additionally, if the scalenes are tight, they are capable of squeezing and compressing the brachial plexus and the major blood vessels nearby. If the large and powerful muscles across the chest become shortened or strained, they can begin to adhere to the protective sheath covering the brachial plexus, leading to symptoms.

Thoracic outlet syndrome affects the so-called thoracic outlet area that lies between the base of the neck and the armpit. This area is a passageway for nerves and blood vessels to pass through muscle, bone, and other tissues. When the passageway becomes narrowed, thoracic outlet syndrome occurs. What causes thoracic outlet syndrome?
Conditions that can narrow the thoracic outlet and cause thoracic outlet syndrome include enlargement of the muscles in this area by body building, injuries, presence of an extra rib from birth, and tumors. How is thoracic outlet syndrome diagnosed?
The symptoms of thoracic outlet syndrome include:
  • pain in the neck, shoulder, and arm and
  • numbness or impaired circulation to the arm.
The symptoms often occur when the arm is raised above the shoulder or extended. Thoracic outlet syndrome is diagnosed by a doctor's examination and special x-rays and nerve conduction studies. How is thoracic outlet syndrome treated?

Nerve Compression in The Thoracic Outlet Vein Compression in The Thoracic Outlet Artery Compression in The Thoracic Outlet Thoracic outlet syndrome is actually a poorly understood group of diseases that, in its most common form, is difficult to diagnose, frustrating to treat, and often not well understood by both patients and physicians. In this section, we will discuss the conditions that are lumped together under the heading of thoracic outlet syndrome, even though they have widely differing signs and symptoms. The thoracic outlet can be understood as a triangle comprised of the collarbone (clavicle), the underlying first rib, and two small parallel muscles each roughly the size of a finger. One can locate this area on one's own body by feeling just behind the part of the collarbone closest to the base of the neck. The area is an outlet in the sense that the nerves and the main artery and vein to the arm pass through this rather tight, small triangle on their way out of the neck and chest (thoracic cavity). The pathologic conditions grouped under the diagnosis of thoracic outlet syndrome usually involve compression of the nerves, vein, or artery as they pass through the triangle of the thoracic outlet.

Thoracic outlet syndrome comprises a group of symptoms (upper extremity, chest, neck, shoulders, and head) produced by a positional, intermittent compression of the brachial plexus and/or subclavian artery/vein and vertebral artery. There are three orders of thoracic outlet syndrome:
  • Neurological syndrome
  • Arterial syndrome
  • Venous syndrome Thoracic outlet syndrome is more prevalent in women than men between the ages of 30-50 years of age. There are several predisposing factors to thoracic outlet syndrome:
    • Poor muscle development
    • Droop of scapula
    • Obesity
    • Breast hypertrophy
    • Trauma
    • Occupational
    • Tenderness or mass in the supra-clavicular fossa
    • Compression of the inferior trunk C8/T1
    • Sensory changes in the ring and little finger
    • Intrinsic muscle weakness in the hand (inability to bring all fingers together)
    • Shoulder pain with arm pain
    • Positive orthopaedic tests
