Editorial Testicular cancer management We have to keep our eye on the ball MJA
Almost 600 young Australian men are diagnosed with testicular cancer each year. It is the second most common malignancy in men aged 18-35 years, behind melanoma, and its incidence has more than doubled over the past 30 years. Fortunately, this disease is one of the major successes of modern oncology, and almost all of these men can now expect to be cured. With the advances in the management of the disease over the past 25 years, most men with metastatic testicular cancer are now curable with combination chemotherapy. During the same time, patients with earlier stages of the disease have benefited from changes that reduce the toxicity of treatment. These changes include:
  • a reduction in the number of cycles of chemotherapy required for treatment of good-prognosis metastatic disease,
  • a reduction in the radiation dose and field size for the treatment of early-stage seminoma, and
  • the introduction of active surveillance (close, structured follow-up to detect and treat relapse) for Stage I non-seminomatous tumours.

Please note: In 2003, the CTF updated its Grades of Recommendations to include an "I Recommendation" for situations where insufficient evidence exists to allow a recommendation to be made. (Formerly, these situations were captured under a "C Recommendation".) This change is not retroactive, and all "C Recommendations" made prior to 2003 have not been reevaluated in light of the new "I" recommendation grade. In March of 2000, unconventional MTV personality and Comedian Tom Green was diagnosed with testicular cancer. Testicular cancer is the most common form of cancer in men aged 15-35. USRF salutes Tom's courage in bringing attention to this important health issue, as only he can.
Tom's MTV Cancer Special
On May 23, 2000, MTV aired a one-hour special episode of the Tom Green Show. The special followed Tom through his treatment and included graphic footage of the surgical procedure during which doctors removed Tom's right testicle. Tom uses humor to educate, and MTV has graciously allowed USRF to share video clips of some lighter moments from the show.

Stats and Facts The incidence peaks between ages 15 and 35, with TC as one of the most common cancers in males in this age group. The incidence of TC is increasing worldwide. At the same time, associated mortality has decreased significantly. The American Cancer Society (ACS) estimates that in 2003, the number of newly diagnosed patients will reach about 7,600 new cases, and of these, 400 men will die of TC. Still unknown, the cause of TC has long held the interest of clinical researchers. Their investigations have focused on the relative importance of intrauterine and postpubertal risk factors. Intrauterine exposure to estrogen has been linked to reproductive developmental disorders including testicular cancer and undescended testicle(s), known as cryptorchidism.
