TENDONITIS OF THE FOOT (POSTERIOR TIBIAL TENDON DYSFUNCTION) INTRODUCTION Tendonitis in the foot is a common problem amongst hard working people. This is a classic over use injury and affects a particular tendon in the foot, the posterior tibial tendon. This tendon helps to hold the arch of the foot and prevents your feet from rolling in too much. The patients in step of the foot may appear inflamed and may have a pronounced flat foot deformity. Symptoms include pain at the in step of the foot, especially along the course of the tendon. There may be burning, tingling, shooting or stabbing pain present in the foot. This is due to the inflammation of the nerve that surrounds the tendon. If a patient is asked to stand on their toes, intense pain will be present in the arch of the foot.

ACHILLES PAIN INTRODUCTION TO ACHILLES PAIN Achilles tendonitis is a painful condition that can cause pain with every step you take. As the name suggests achilles tendonitis affects the achilles tendon. The achilles tendon connects the heel of the foot to the calve muscles. Symptoms include pain which may be present at the achilles tendon during activity and may also increase once activity has ceased. Tenderness can usually be felt along the course of the tendon and in some cases there may be the presence of a localized nodule (small sack of fluid) on the tendon. Another sign to look for is that the soles of shoes tend to wear out quickly around the heel region.

Calcific Tendonitis
What is calcific tendonitis?
Calcific tendonitis results from the inflammation and calcification of tendons around a joint. A tendon is a band of strong fibrous tissue forming the end of the muscle that attaches the muscle to bone.
How does it occur?
Injury, trauma, or stress to parts of the body where muscles and tendons are important to function can cause calcific tendonitis. It often occurs in the tendons of the shoulder joints, but tendons of any joint may be involved in such a process. Calcification is the process by which fibers of a tendon become hardened by a deposit of calcium salts, causing inflammation.

What is it?
: Wrist tendonitis is an irritation of one of the tendons where it crosses the wrist, which causes pain.
Areas of Body Directly Affected: Wrist
Symptoms: Ache or pain at the wrist, which is worsened with forceful gripping, rapid wrist movements, or moving the wrist and fingers to an extreme position. Tendons cross the wrist from the forearm to the hand on all sides of the wrist. The most common sites of the wrist for problems are at the base of the thumb (DeQuervain's tenosynovitis), on the back of the wrist, and on the palm side of the wrist.
Treatment: May include a brief period of use of anti-inflammatory agents, such as Motrin or ibuprofen. Reducing forceful gripping can also help. At the end of a workday or after a flare, ice applied for 15 minutes can reduce swelling and pain. Wrist splints may help the tendons to heal by reducing the tendon motion and load. Physical therapy can help with instructions on proper ways to use the hands, methods of applying ice, methods for performing gentle stretching exercises, and the fitting and use of a splint. Injections with steriod medication are useful. If symptoms are very severe and unresponsive to all of these measures, after they have been tried for many months, surgery may be successful for some types of tendonitis.

    ACHILLES TENDINOSIS (AT) Achilles tendinosis = "the bane of many runners"-especially true for the older athlete! T he term "Achilles tendinitis" is a misnomer. "itis" means inflammation. There is no inflammation of the tendon! The correct name is Achilles tendinosis. So, if you are taking NSAID's for the inflammation, you might want to switch to acetaminophen which will be easier on your stomach. The following ABSTRACT is from the British Journal of Sports Medicine. Last fall, I had to stop running completely becasue of the pain in my left Achilles. I read this paper in December and started doing an eccentric stretch of the Achilles. I started running again five ewwks ago. A cortisone injection made it feel better, but I have no strength or stability, especially when I stand on one leg. What else can I do?
    Answer Patellar tendonitis is an overuse or overload syndrome, the hallmark symptom being pain in front of the knee. Usually there is a distinct localized tender spot. Patients also describe night pain, pain with sitting, squatting, or kneeling. Climbing stairs can also be painful.
    There may be puffiness around the front of the knee; usually the patient has pain when the knee is bent (flexed) extremely, as well as sometimes with full straightening (extension).
    If the condition has been present for some time, the quadriceps muscle (front of the thigh) may be smaller than on the other leg.
    Treatment involves primarily rest, but also the use of non-steroidal anti-inflammatory medicines (NSAIDs), such as ibuprophen, acetaminophen, or diclofenac. Cortisone injections should not be injected directly into the tendon, as this may increase the risk of rupture of the tendon.

    Rotator Cuff Injury October 23, 2001 (Reviewed: May 20, 2003) Question A recent MRI showed that I have "infraspinatus tendonopathy". My internist says that I need surgery. I am unclear about what this means. Can you please clarify?
    Answer Based on your MRI, you seem to have a case of shoulder tendonitis. Tendonitis (or tendonopathy) is inflammation (redness, soreness, and swelling) of a tendon. In tendonitis of the shoulder, the rotator cuff and/or biceps tendon become inflamed, usually as a result of being pinched by surrounding structures. The injury may vary from mild inflammation to involvement of most of the rotator cuff. When the rotator cuff tendon becomes inflamed and thickened, it may get trapped under the acromion (the top of your shoulder). Squeezing the rotator cuff is called the impingement syndrome. Repeated motions involving the arms, or the aging process involving shoulder motion over many years, may also irritate and wear down the tendons, muscles, and surrounding structures.
    Your first step when an injury occurs is to see your doctor. I would expect that your own doctor would direct you to see a specialist, an orthopedic surgeon. This type of doctor can determine what treatments are required to help you. In general, there are a number of treatment options for patients with tendonitis. These range from the use of anti-inflammatory medicines to surgery. Other potential treatments can include physical therapy, ice packs, and even injection of corticosteroids into the space under the acromion. You should know that while steroid injections are a common treatment, they must be used with caution because they may lead to tendon rupture. Please see an orthopedic surgeon regarding your problem.
