Syringomyelia Noncommunicating syringomyelia occurs as a complication of trauma, meningitis, hemorrhage or tumor, the cyst or syrinx developing in a segment of the spinal cord damaged by one or more of these conditions. I can find no reports that damage from a decompression accident has been related as a cause. This condition is not mentioned in Edmonds' or Bove's texts - so one will have to use associative reasoning - as we often do about situations such as this. As the body moves in it's day to day activities the syrinx or cyst expands and elongates over time, destroying the center of the cord, and can mimic the effects of a spinal decompression accident. This damage may result in pain, weakness, and stiffness in the back, shoulders, arms or legs. Other symptoms may include headaches and loss of the ability to feel extremes of hot or cold, especially in the hands and disruption in body temperature, sweating, sexual function and bladder and bowel control. Because of the difficulties in differentiating between the residual effects of a diver's decompression accident and the effects of the syrinx - it is felt that diving would be unwise unless carefully documented and evaluated on an individual basis.

Syringohydromyelia
Also known as syringomyelia and hydromyelia I will use the common simpler name of syrinx . Essentially, a syrinx is a fluid collection in the spinal cord. There are many causes of this such as hydrocephalus Chiari Malformation , trauma, tethered spinal cord , tumor and a very large number without any clear cause, the so-called idiopathic . These collections are usually similar to spinal fluid . They can cause symptoms in two main ways. One is due to the direct pressure of the fluid collection on the spinal cord. The symptoms will then be related to the level of the spinal cord at which the collection is. It may cause weakness, numbness, stiffness, pain, scoliosis, and incontinence to name a few. These symptoms usually come on quite gradually.
The other way the symptoms may be found is due to the underlying cause of the syrinx. For instance, a syrinx may be caused by a Chiari Malformation, and the symptom may be one of headaches or neck pain even though the syrinx may be lower in the spinal cord. The important thing is for the physician to recognize the possibilities and perform the appropriate tests.
The availability of MRI has greatly improved our ability to both diagnose and follow these collections. Once one is identified it is usually prudent to study the entire nervous system looking for associated abnormalities which may influence treatment. Once it is determined that the syrinx is of clinical significance (causing problems such injury to the spinal cord or progressive scoliosis) the only treatment available is surgery.

PURPOSE: To clarify the aspects to be considered in regard to surgery for patients presenting scoliosis associated with syringomyelia. PATIENTS AND METHODS: The clinical records of 50 patients (21 males and 29 females), who had been diagnosed as having scoliosis associated with syringomyelia and had been operated on for syringomyelia, scoliosis or both, were investigated. We divided these patients into three groups according to the surgical procedures. Group 1 included 14 patients who were subjected to corrective surgery for scoliosis after surgery for syringomyelia such as foramen magnum decompression or shunt procedure. Group 2 included 21 patients subjected to corrective surgery for scoliosis alone. Group 3 included 15 patients subjected to surgery for syringomyelia alone. In each group, we mainly reviewed the postoperative changes of neurologic symptoms. RESULTS: CONCLUSIONS: This study reported a high frequency (14.0%) of postoperative neurologic complications. It increased to 33.3% in the patients who had not been treated for syringomvelia. Dealing with syringomyelia previous to spinal corrective surgery is necessary to avoid not only early neurologic complications but also delayed complications, especially if the syrinx is large or tonsillar herniation is severe.

Disease - Syringomyelia
Definition: Syringomyelia is damage to the spinal cord, caused by a formation of a fluid-filled cavity within the cord. Alternative Names: Syrinx Causes And Risk: Syringomyelia is caused by a formation of a fluid-filled cavity within the spinal cord. This fluid build-up may be a result of spinal cord trauma , tumors of the spinal cord, or congenital defects. The cavity most often begins in the neck area. It expands slowly, causing progressive damage to the spinal cord due to the pressure exerted by the fluid. Symptoms result from the spinal cord damage. Prevention: There is no known prevention, other than avoiding trauma to the spinal cord. Prompt treatment reduces progression of the disorder. Symptoms:

What is syringomyelia?
Syringomyelia (sear-IN-go-my-EEL-ya) is a disorder in which a cyst forms within the spinal cord. This cyst, called a syrinx, expands and elongates over time, destroying the center of the spinal cord. Since the spinal cord connects the brain to nerves in the extremities, this damage results in pain, weakness, and stiffness in the back, shoulders, arms, or legs. Other symptoms may include headaches and a loss of the ability to feel extremes of hot or cold, especially in the hands. Each patient experiences a different combination of symptoms. Other, more common disorders share the early symptoms of syringomyelia. In the past, this has made diagnosis difficult. The advent of one outpatient test, however, called magnetic resonance imaging (MRI), has significantly increased the number of syringomyelia cases diagnosed in the beginning stages of the disorder. About 21,000 American men and women have syringomyelia, with symptoms usually beginning in young adulthood. Signs of the disorder tend to develop slowly, although sudden onset may occur with coughing or straining. If not treated surgically, syringomyelia often leads to progressive weakness in the arms and legs, loss of hand sensation, and chronic, severe pain.
