  • What Is It? Syphilis is a sexually transmitted disease (STD) caused by a type of bacteria called Treponema pallidum . In its earliest stage, syphilis produces an open sore (ulcer) that leaks fluid filled with syphilis bacteria. Syphilis can be transmitted by contact with this ulcer, or other infectious sores that form later in the disease, usually during vaginal, oral or anal sex. If untreated, syphilis progresses through a series of stages that affect different parts of the body, although the stages can overlap:

    Statistically, syphilis is a "minor" STD, however the steady decline of many years was markedly reversed in the U.S. in 1987, with several states and cities, including Florida, San Diego, and Los Angeles, reporting rates of primary and secondary syphilis many times greater than the previous year. There has been, not unexpectedly, a parallel increase in congenital syphilis. The reasons for the increase are speculative, but include the recent change in these areas to the use of spectinomycin as the drug of choice for gonorrhea (spectinomycin is not active against syphilis), and increased promiscuity among "crack" users, who trade sex for drugs. Information on syphilis rates outside the U.S. is limited, but the same pattern seems to be emerging there also. Stages of Syphilis
    Primary Syphilis
    Primary syphilis occurs when Treponema pallidum penetrates through mucous membranes or small breaks in the cornified epithelium. Incubation is 10 to 90 days, with an average of three weeks. The typical chancre is solitary, indurated, painless, smooth-based, and heals in three to six weeks. Chancres are usually found on the genitalia, at the rectum, or in the mouth. Atypical lesions are common, including extragenital lesions. Secondarily infected and traumatized lesions may be painful. Intraurethral lesions frequently cause dysuria and watery discharge similar to that of NGU. Chancres usually last three to eight weeks, and heal leaving a thin atrophic scar. In reinfection syphilis, patients may have sufficient immunity to alter the clinical presentation of the disease; the chancre may not appear.

    After reports of a burgeoning syphilis epidemic amongst gay men in Europe and North America, a new study has found alarming cases of drug resistant infections.
    The US study reveals that 8 cases of syphilis between 2002 and 2003 had developed resistance to azithromycin, a regularly used medication to cure the patient. All of the patients were gay men, while 5 were also HIV positive.
    There are currently three commonly used methods for treating syphilis, although azithromycin is the easiest to administer.
    Although all patients in the report were subsequently successfully treated with either doxycycline or penicillin, the findings has alarmed health workers, who have been reporting a significant jump in new syphilis cases amongst the gay community for the past few years.
    Just yesterday, a report found that gay men are accounting for the majority of new cases in the US. Most worryingly, the study revealed that gay men do not get tested for the sexually transmitted infection (STI) until much later than their straight peers, and subsequently remain infectious for much longer.

    The most "poetic" statement about the dreaded plague is not an early physician's hexameter but the modern map of the pathogen's genome. We usually manage to confine our appetite for mutual recrimination to merely petty or mildly amusing taunts. Among English speakers, unannounced departures (especially with bills left unpaid) or military absences without permission go by the epithet of "taking French leave." But a Frenchman calls the same, presumably universal, human tendency filer a l'anglaise, or "taking English leave." I learned, during an undergraduate year in England, that the condoms I had bought (for no realized purpose, alas) were "French letters" to my fellow students. In France that summer, my fellow students of another nation called the same item a chapeau anglais, or "English hat." I first encountered this debate in a succinct summary written by Ludovico Moscardo, who described potential herbal remedies in the catalog of his museum, published in 1672: "Ne sapendo, a chi dar la colpa, li spagnuoli lo chiamorono real Francese, li Francesi male Napolitano, eli Tedeschi, real Spagnuolo" (not knowing whom to blame, the Spaniards call it the French disease, the French the Neapolitan disease, and the Germans the Spanish disease). Moscardo then added that other people attribute the origins of syphilis to bad airs generated by a conjunction of the three most distant planetsMars, Jupiter, and Saturnin the night sky.

    Syphilis is a sexually transmitted (venereal) disease (STD) that is caused by an organism called "Treponema pallidum." Syphilis can affect men, women and infants. What are the signs and symptoms of syphilis?
    Syphilis is dangerous, but it is easily treated if it is diagnosed early. If left untreated, it can cause blindness, insanity, paralysis, heart disease or death. Symptoms may appear 10-90 days after exposure. Syphilis develops symptoms which are broken down into four stages. Signs and symptoms in an adult
    • Stage One: This is an infectious (contagious) stage of the disease. There are two main symptoms in this stage: Swelling of the lymph glands and the appearance of chancres. Chancres are open, oval shaped sores with hard, raised borders. Chancres are usually seen on the genitals (penis or around the vagina), the mouth or anus, but may appear anywhere. Chancres in women may not be noticed because they may occur inside the vagina. Chancres are not painful and rarely bleed. Syphilis is passed from direct contact (during sex) with the chancres. Chancres will appear 10-28 days after sexual contact and will usually disappear in two to six weeks even if not treated. Stage Two: This is also an infectious stage. It begins two weeks to six months after the chancre is gone. A rash appears on the palms of the hands and the soles of the feet. The lymph glands swell, the throat may be sore, and patches appear in the mouth. Flat bumps that look like warts may be seen around the genitals and the mouth. These bumps are very infectious. Other symptoms include hair loss, loss of appetite, constipation, nausea, a low-grade fever and muscle, joint or bone pain. These symptoms may last two to six weeks.

    Syphilis What you should know:
    Syphilis is a serious sexually transmitted infection that is caused by a bacterium. This bacterium can cross the placenta and infect the baby. Fortunately, syphilis is uncommon in the United States.
    Without treatment, up to 40 percent of babies who are infected in the womb die before or shortly after birth. Those who survive are at risk for developing brain damage, blindness, hearing loss and bone and tooth problems, if they are not treated with an antibiotic shortly after birth.
    An early symptom of syphilis is a hard, painless sore, often in the genital area. Up to three months later, infected persons can develop a rash and fever. If the disease is left untreated, damage to the heart, brain and other organs can occur years after infection.
    What you can do: You will probably be tested for syphilis at an early prenatal visit. A single dose of penicillin can cure syphilis if you’ve had the infection for less than one year. Older infections will require more treatment. If treatment begins before the fourth month of pregnancy, the baby will probably not be affected. You can protect yourself from sexually transmitted infections by following these guidelines:
    • Have sex with only one partner who does not have sex with anyone else and who does not use drugs.

    Syphilis is a sexually transmitted infectious disease caused by the bacterium Treponema pallidum A. T. pallidum has a number of characteristics which are important for diagnosis.
    • The spiral shaped morphology and characteristic motility pattern (they spin around their longitudinal axis in a corkscrew type manner) are important for diagnosis via darkfield microscopy. Here is a picture of the cross section of the organism. T. pallidum cannot be grown in vitro
    B. Epidemiology of syphilis
    • Man is the only known host and transmission is virtually always by direct contact with infectious lesions, generally through sexual contact. The incidence is highest in sexually active people (20-29 year old group). The number and rate of 1 o and 2 o Approximately 30% of exposed people contract the disease. The South continues to have the highest rate in the country (rate: 4.5) From 1998 to 1999, rates declined 10% in the South (from 5.0 to 4.5) and 12.5% in the Northeast (0.8 to 0.7). The rate for the West remained unchanged (1.0), and the rate for the Midwest increased from 1.9 in 1998 to 2.2 in 1999.

      Sexually transmitted diseases are those that are transmitted during sexual intercourse or other intimate contact with the sex organs. Syphilis is an infectious venereal disease caused by a spirochete (Treponema pallidum) and usually is transmitted by sexual intercourse, or acquired congenitally. Syphilis, however, can be contracted through other physical contact such as kissing. If untreated, it can ultimately lead to the degeneration of bones, heart, nerve tissue, etc.

    Chlamydia, Gonorrhea, and Syphilis Chlamydia Chlamydia is the most prevalent STD in the U.S. It is most common among people in their late teens and early twenties and can coexist with gonorrhea and other STDs. It is estimated that one in five college students are infected with Chlamydia. The infection is most commonly transmitted through sexual intercourse. Babies can also be infected while passing through the birth canal of an infected mother. If Chlamydia is left untreated, women can develop Pelvic Inflammatory Disease (PID) . There is also an increased danger of having an ectopic pregnancy if Chlamydia is left untreated.
    People with Chlamydia do not necessarily know that they are infected. Sixty to eighty percent of women and ten percent of men who have Chlamydia exhibit no symptoms. In women, symptoms include: genital itching and burning, vaginal discharge, dull pelvic pain, bleeding between periods, and cervical inflammation. In men, symptoms include: mucus discharge from the penis (gradual onset five to twenty-one days after exposure) and painful urination. Again, these symptoms may be so mild that a man may not notice them. Treatment with an antibiotic is usually successful. Some people choose to be retested after the course of treatment has been completed. Testing for Chlamydia is performed at the Health Center free on request.

    What is syphilis?
    Syphilis (say: "siff-uh-liss") is a serious infection passed from one person to another during sex. It's caused by a bacteria (germ). You could get syphilis by touching the blood or sores of a person who has syphilis, especially sores on the person's mouth, penis, vagina or anus (the opening to the rectum). Return to top
    What are the signs of syphilis?
    In men, the first sign of syphilis may be a sore on the penis. In women, the first sign may be a sore around or inside the vagina. You might not even notice the sore, because syphilis sores don't hurt. The sores go away after 3 to 6 weeks. If you don't treat syphilis early, it spreads from the sore into your blood. When syphilis gets into your blood, it can cause many problems. The most common sign is a rash. The rash appears 2 weeks to 2 months after the appearance of the sores, often on the palms of the hands or the soles of the feet. Other signs of syphilis include fever, sore throat and swollen lymph glands. After many years, people with syphilis who don't get treatment may begin to have problems in their brain and spinal cord. Syphilis may damage the heart and other organs, too.

The agency began an all-out prevention campaign, stepping up syphilis surveillance in New York City and the few counties across the country where it still existed. By 2000, the sexually transmitted infection was at its lowest point since 1941. Soon, the country's top doctors predicted, the dread illness that afflicted Henry VIII, Ivan the Terrible, and even a pope would be nothing more than an unpleasant memory, gone the way of smallpox and other eradicated diseases. But just six years after the bold elimination plan, syphilis is back. Nationally, cases in gay men shot up more than 15 percent in 2001. In New York City, the number of people with symptoms of syphilis has gone up even further, increasing a total of more than 500 percent between 1998 and 2003, from 82 to 531 people, according to preliminary health department data. As of last week, there were an additional 953 people infected with syphilis but without apparent symptoms, according to the health department. Many more cases of syphilis likely go unreported.
