SYPHILIS Once the cause of devastating epidemics, syphilis now can be effectively controlled with antibiotics. Although syphilis remains a significant health problem in the United States, the number of cases reported nationally and in Illinois during 1998 was the lowest in any year since the mid 1960s. Preventing syphilis is especially important because persons infected with the disease are at increased risk for acquiring and transmitting HIV, the virus that causes AIDS. The U.S. Centers for Disease Control and Prevention, in collaboration with state and local health departments and with affected communities, is launching a national campaign to eliminate syphilis in the United States by 2005. This goal is within reach for two important reasons: the low number of reported cases in the past few years and the fact that half of all new cases in 1998 were reported in only 28 U.S. counties. Cook County, which includes Chicago and surrounding communities, was one of those counties and ranked second in the nation in 1998 in reported cases. What is syphilis?

A patient information handout on syphilis, written by the authors of this article, is provided on page 2245. The diagnosis and treatment of syphilis can present difficult dilemmas. Serologic tests can be negative if they are performed at the stage when lesions are present, and the VDRL test can be negative in patients with late syphilis. Cerebrospinal fluid examination is not required in patients with primary or secondary disease and no neurologic signs or symptoms, but it may be warranted in patients with late latent syphilis or in whom the duration of infection is unknown. Patients with penicillin allergy can be treated with alternative regimens if they have primary or secondary syphilis. Penicillin is the only effective drug for neurosyphilis; oral desensitization should be accomplished before treatment of penicillin-allergic patients. Other dilemmas may be encountered in the treatment of patients who have concurrent human immunodeficiency virus infection. S yphilis is a sexually transmitted disease (STD) caused by the spirochete Treponema pallidum. Previously known as the "great imitator," this disease can have numerous and complex manifestations. Family physicians should understand its presentations, stage-specific diagnostic testing, and appropriate antibiotic treatments, because missed or inappropriately treated syphilis can result in devastating cardiovascular and neurologic disease, as well as congenital syphilis. See page 204 for definitions of strength-of-evidence levels. Epidemiology The incidence of syphilis decreased significantly with the introduction of penicillin in the 1940s but rose sharply again with the advent of human immunodeficiency virus (HIV) infection in the 1980s. From 1990 through 2000, primary and secondary syphilis infection rates decreased by 89.2 percent. Despite the overall decreases, outbreaks of syphilis have recently been reported in men who have sex with men. In the United States, syphilis is more prevalent in the South, in urban areas, in men, and in blacks.

Syphilis Die Syphilis oder Lues wird durch Bakterien (sog. Treponema pallidum) verursacht. The experiments have left a legacy of mistrust in the African-American community that is tangible enough to be measured by social scientists in the Birmingham, Alabama, area. "About 22 percent of African-Americans who we surveyed in the Birmingham area had some mistrust with regards to participating in research studies because of the Tuskegee Syphilis Study," said Lee Green of the University of Alabama. The Tuskegee study is a symbol of racial and scientific exploitation. "We found that it's both government and doctors. A lot of the people we spoke with mentioned how they're treated in health settings," Green said. James Jones' book, "Bad Blood: The Tuskegee Syphilis Experiment," chronicles the study that ran from the 1930s into the 1970s. "They did not understand that treatment was being withheld. They did not understand they had syphilis," Jones said. "They were not given enough information to make anything like an informed decision."

    What is syphilis?
    Syphilis is a complex sexually transmitted infection (STI) caused by the bacteria Treponema pallidum . Episodes of active disease occur, followed by latent periods, when the person remains infected but there are no signs or symptoms. Initially syphilis appears as a painless sore (ulcer) where the infection entered (usually around the genitals) and may go unnoticed. The sore is known as a chancre and this phase is known as primary syphilis. Widespread rash and 'flu-like symptoms appear next (secondary syphilis).
    Who is at risk of syphilis and how is it spread?
    Sexually active people may be at risk of syphilis. It is passed from person to person through direct contact with syphilis ulcers. These are most commonly on the genitals and anal area but may also be found on the lips or mouth. Hence, vaginal, anal or oral sex is the main way of passing the infection from one individual to another. Syphilis can also be passed on through:

    What is syphilis?
    Syphilis is a serious bacterial infection, which may be transmitted by an infected person during intimate, usually sexual contact. It is often called "the great imitator" because so many signs and symptoms are indistinguishable from those of other diseases.
    How is syphilis transmitted?
    Syphilis is caused by the spread of bacterial from the sores of an infected person to the mucous membranes of the genital area, the mouth, or the anus of a sexual partner. The bacteria also can pass through broken skin on other parts of the body. Once the bacteria enter the body through these moist mucous membranes, it travels through the bloodstream and can infect vital organs. Syphilis cannot be spread by toilet seats, doorknobs, swimming pools, hot tubs, shared clothing or eating utensils.
    What are the signs and symptoms of syphilis?
    It usually takes between 10-90 days with an average of 21 days for primary symptoms to develop. The symptoms of syphilis vary according to the stage of the disease.

    In this article, the terms "sexually transmitted infections" and "STIs" are replacing the outdated terminology "sexually transmitted diseases" and "STDs." Sexually transmitted infections (STIs, formerly sexually transmitted diseases or STDs) are infections that can be transferred from one person to another through sexual contact. Sexually transmitted infections have probably been around for thousands of years, but the most worrisome, acquired immunodeficiency syndrome or AIDS, has only been recognized since 1984. It is important to realize that sexual contact includes more than just sexual intercourse (vaginal and anal). Sexual contact includes kissing, oral-genital contact, and the use of sexual "toys," such as vibrators. Most STIs are treatable. However, even the once easily cured gonorrhea has become resistant to many of the older traditional antibiotics. Other STIs, such as herpes, AIDS, and genital

    SYPHILIS SIGNS AND SYMPTOMS
    Syphilis is a multi-system, multi-symptom disorder that occurs primarily through sexual transmission, though the disease can be spread through blood transfusion and direct contact with an infected lesion. Patients tend to be younger, with a history of unprotected sex. Elderly patients may manifest late-stage syphilis left untreated many years before. In cases of congenital syphilis, the patient may manifest Hutchinson's triad (interstitial keratitis, deafness and malformed teeth), osteochondritis (inflammation of both bone and cartilage), chorioretinitis, hepatosplenomegaly (enlargement of the liver and spleen), and anorexia. In the primary stage of acquired syphilis, the patient develops a painless chancre at the site of inoculation, as well as regional lymphadenopathy. While primarily genital, chancres may develop on the eyelid and conjunctiva. Other ocular signs in the primary stage include conjunctivitis, blepharitis, and alopecia. In the secondary stage of acquired syphilis, the patient will develop malaise, lymphadenopathy, fever, maculopapular skin lesions on the palms and soles, joint pain, headache, and loss of appetite. Ocular signs are most common in secondary syphilis and include episcleritis, anterior uveitis, uveitic glaucoma, neuroretinitis, chorioretinitis, ischemic retinal vasculopathy, and infectious optic neuropathy.
