Syphilis (previously called lues ) is a sexually transmitted disease (STD) that is caused by a spirochaete bacterium Treponema pallidum . Syphilis has many alternate names, such as: Miss Siff, the Pox, and has been given many national attributions, e.g. the French disease. The route of transmission is almost invariably by sexual contact; however, there are examples of direct contact infections and of congenital syphilis (transmission from mother to child in utero The signs and symptoms of syphilis are myriad; before the advent of serological testing, diagnosis was more difficult and the disease was dubbed the "Great Imitator" because it was so often confused with other diseases. In the United States, about 36,000 cases of syphilis are reported each year, and the actual number is presumed to be higher. About three-fifths of the reported cases occur in men. If not treated, syphilis can cause serious effects such as damage to the nervous system, heart , or brain . Untreated syphilis can be fatal. If you think you might have syphilis, or if you find out that a sex partner had or might have had syphilis, see a doctor as soon as possible. Syphilis can be treated with penicillin or other antibiotics Health care professionals suggest that safer sex practices such as the use of condoms should always be used in sexual activities, but they should by no means be considered an absolute safeguard. Syphilis has signs and symptoms that can look like other more common illnesses. The main sign of syphilis in one stage could be a rash; in advanced stages, the chief sign might be joint problems or disordered thinking. For some people who develop syphilis, there are no obvious signs at all. Congenital Syphilis
When a pregnant woman has syphilis, she can transmit the infection to her unborn child, causing congenital syphilis. By the age of 2, children born with congenital syphilis show symptoms such as problems affecting the skin, teeth, bones, liver, blood, kidneys, eyes, nerves, and brain. Acquired Syphilis
Syphilis is a sexually transmitted disease , but if not diagnosed and treated, the disease can progress to affect the entire body. Syphilis has three stages - primary, secondary, and late (tertiary) - each with its own signs and symptoms.
  • Primary Syphilis
    Primary syphilis begins 2 to 6 weeks after sexual contact with an infected person. A painless red sore called a chancre appears, usually on the genitals. Depending on the type of contact, the chancre may also appear on the mouth or in the rectal area. Usually there are also enlarged lymph nodes (swollen glands) near the area of the chancre. Without treatment, the chancre heals after 4 to 6 weeks, leaving a thin scar, and the second stage of syphilis begins.
  • Secondary Syphilis
    This stage usually begins 2 to 10 weeks after the chancre heals. Syphilis bacteria enter the blood and spread through the body causing many different symptoms, including rash (small red bumps), fever, headache, loss of appetite, weight loss, sore throat, muscle aches, joint pain, a generally ill feeling, and enlarged lymph nodes. What Is It?
Syphilis (pronounced: siff -ill-iss) is caused by bacteria called Trepenoma pallidum . This type of bacteria is known as a spirochete , which is extremely small and can live almost anywhere in the body. The spirochetes that cause syphilis can be passed from one person to another through sexual intercourse (vaginal, anal, or oral sex). The infection can also be passed from a mother to her baby during pregnancy. You cannot catch syphilis from a towel, doorknob, or toilet seat. How Does a Girl Know She Has It?
Syphilis occurs in four different stages:
    Stage One.
    Red, wet sores that don't hurt appear on the vagina, rectum, or mouth. This type of sore is called a chancre ( shank -er), and it appears in the first place that the spirochetes moved from one person to another. After a few weeks, the chancre will disappear. Swollen glands may also appear during the first stage. Syphilis is highly contagious during this first stage.

It is one of the great imitators in medicine affecting most of the organs in the body. It is legally defined as venereal disease under the Venereal Disease Ordinance H.K. (1952).
2. EPIDEMIOLOGY Syphilis can be acquired or congenital. For epidemiological convenience, it has been further divided into early and late syphilis with an arbitrary dividing line of two years. In early syphilis (primary, secondary, early latent), the disease is infectious and it is important to seek the recent sexual contact. In late syphilis (late latent, tertiary), infectivity lapses rapidly with time and it is more important to examine spouse and family members. The first recognizable epidemic of syphilis occurred in Europe at the end of 15th century. Since then, there is another epidemic of early syphilis in many countries during and immediately after Second World War. Again, there is a rise between 1965 and 1975. It is more prevalent in urban than rural areas. The high risk groups include prostitutes and male homosexuals. Male is more common than female. The incidence of syphilis recorded in Social Hygiene Clinics in recent five years is shown as follows: Total (except congenital) % of total new VD cases Congenital % of total new VD cases As shown from the figures, the incidence of syphilis in Hong Kong is steady in recent years and relatively low as compared with other places in Asia. This is the result of the successful control of the infectious diseases here. The antenatal screening with VDRL also explains the low incidence of congenital syphilis.

Syphilis is caused by the spirochete Treponema pallidum Treponema is a Gram-negative, thin, motile, spiral shaped bacterium in the order Spirochaetales . The name spirochete is derived from the Greek words for "coiled hair." Their spiral cellular shape is approximately 16 to 18 bends consisting of an outer sheath, periplasmic space with periplasmic flagella, and a peptidoglycan layer.

The approximate size of Treponema is 10 um X 0.2 um. Therefore, dark field microscopy, which takes advantage of the Tyndall Effect, must be used to observe Treponema cells. The cells can also be observed microscopically after staining with specific anti-treponemal antibodies labeled with florescent dyes. The spirochetes are able to swim in viscous environments (e.g. oral cavity, intestinal tract), but are only able to spin in water due to the lack of friction. Any contact with air, antiseptics, or sunlight will kill the microbe. T. pallidum

Disease may be acquired or congenital. In acquired syphilis, T. Pallidum enters the body through skin or mucous membranes, usually during sexual contact. Congenital syphilis is transmitted to the fetus from the infected mother when the spirochete penetrates the placenta. Syphilis is a systemic disease, involving tissues throughout the body. After initial penetration, the spirochetes multiply rapidly. First they enter the lymph capillaries where they are transported to the nearest lymph gland. There they multiply and are released into the blood stream. Within days the spirochetes invade every part of the body. Three stages mark the progression of the disease; primary, secondary, and tertiary. Primary Stage: 10-60 days after infection. Primary lesion usually appears at point of contact, usually genitals. Typically a painless, slightly elevated, round ulcer, the chancre may be so small as to elude detection. Barring secondary infection, chancre will heal without treatment within 30-60 days leaving a scar the persists for several months.

Introduction
Treponema constitutes one of the five genera in the order of Spirochetes, the others being Borrelia, Spirochetaeta, Leptospira, and Cristispira. Treponemes that are pathogenic for humans include Treponema pallidum, T. pertennue, and T. caratium; these cause syphilis, yaws, and pinta respectively. It remains unclear what factors determine which mothers, particularly those in the latent stage of the infection, will pass the disease to their fetuses. Nor is it clear why some infants who are infected in utero are born asymptomatic, but develop overt disease in the first weeks or months of life. Humans are the natural host of T. pallidum and also serve as the vector. Sexual contact provides the usual means of transmission between adults. Rarely, the disease is passed to health care personnel or others who accidentally touch infectious lesions or to lab workers who handle infected animals. The infant is usually infected in utero by transplacental passage of T. pallidum from an infected mother, but infection may occur from contact with an infectious lesion during passage through the birth canal. T. pallidum does not survive well outside the host and is easily destroyed by heat, drying, and soap and water. Because the organism spreads most exclusively by person to person contact, the only precaution that must be taken by health personnel caring for syphilis infants is to avoid touching the infectious lesions. Except for the open skin lesions and mucous membrane lesions of the neonate, congenital syphilis is not infectious by contact. Therefore, no special isolation procedure is required.

How is syphilis spread?
Syphilis is spread directly from person to person by direct contact with an infectious lesion. That contact can be genital intercourse, oral intercourse, or anal intercourse. The bacteria pass through intact mucous membranes and abraded skin; they are then carried by the blood stream to every organ in the body. Babies can also get syphilis from their mothers if the mothers are infected during pregnancy. What are the signs and symptoms of syphilis?
