  • Substance-related disorders affect every segment of the population, regardless of age, race, ethnicity, socioeconomic status, gender or sexual orientation. American high school students have a higher level of illicit drug use than any other industrialized nation. By senior year of high school, approximately 40 percent of American high school students have tried an illegal drug. Intoxication is typically the initial substance-related disorder and usually begins in the teen years. Dependence can occur at any age but typically begins in one’s 20s, 30s and 40s. Substance-related disorders are more common in males than in females. People with substance-related disorders usually find that their general health worsens.
More Far-Reaching than Illicit Drug Activity The term "substance disorder" encompasses more than illegal drug abuse. After all, nicotine, alcohol and caffeine are perfectly legal substances, but people still develop dependence tolerance and withdrawal symptoms from heavy or long-term usage.

Introduction Adolescence is a time for adventure and experiment. Many teenagers try alcohol or drugs at least once. Most don't develop a problem. But not all children and adolescents escape the dangers as easily. Instead, they are at risk for developing serious problems as a result of their youthful experiments. These problems are called substance-use disorders. It's important to know that "recreational use," such as using a substance once or twice, is not a substance disorder. To qualify as a substance disorder, the effects of alcohol or drug use must have a serious, negative impact on a teenager's life . Research and surveys have yielded a great deal of information about children and adolescence and substance use. There is ample evidence that substance abuse disrupts the normal process of adolescent development. A variety of negative outcomes, including serious drug use later in life, school failure, and poor judgment are associated with substance disorders. Substance abuse is the strongest predictor of car accidents, homicides, suicide, HIV exposure, and other medical and mental health conditions. Teenagers who abuse substances often have

Substance-Related Disorders Description Although any teenagers try alcohol or drugs at least once, most stop using them. Some teenagers however, go from experiment to abuse and when they reach the point where using the drug is a need and getting it a priority they have developed a substance-related disorder. Following are three basic types of substance-related disorders:
  • Substance dependence Pattern of repeated substance use that results in tolerance (need for increasingly larger quantity of the substance), withdrawal (unpleasant physical reactions) and compulsive drug-taking behavior.
    Substance abuse
    A maladaptive pattern of substance abuse that results in recurrent and significant adverse consequences, such as failure to fulfill obligations at work, home or school, use in situations in which it is physically hazardous, legal problems, and social and interpersonal problems .
    Substance-induced disorder
    M ental or physical problems that result solely from the drug's chemical effects on the body. They should disappear within a month or so after substance use has stopped.

    Smoking
    Smoking leads to the greatest number of problems of any drug in use in the world today. Smoking contributes to more than 400,000 deaths each year in the United States. These deaths are mainly the result of increased numbers of lung cancers as well as increased numbers of cases of atherosclerotic heart disease and emphysema of the lung. Smoking increases the risk for cancers of the bladder, pancreas, kidney, and cervix. There is an increased risk for gastritis and gastric ulceration in persons who smoke. Cataracts of the crystalline lens of the eye occur with increased frequency in smokers. Young women who are pregnant and who smoke put their fetuses at increased risk for decreased birth weight, premature birth, and perinatal mortality. The risk for spontaneous abortion is increased with maternal smoking. Fetal deaths late in gestation are increased 50% in mothers who smoke more than 3 cigarettes per day.

  • Alcoholism
    The abuse of alcohol contributes to many deaths per year in the United States. One of the most common drug overdoses leading to death is ingestion of a large amount of alcohol. Chronic alcoholism leads to liver disease. Liver disease can be manifested as fatty change. Excessive alcohol ingestion for many years can lead to micronodular cirrhosis. A cirrhotic liver leads to portal hypertension and the complication of bleeding esophageal varices with massive, life-threatening gastrointestinal hemorrhage. There is also an increased risk for hepatocellular carcinoma arising in a cirrhotic liver. In the brain, alcoholism can lead to Wernicke's disease.

    Generic Criteria for Substance Dependence
    • The patient's maladaptive pattern of substance use leads to clinically important distress or impairment as shown in a single 12-month period by 3 or more of the following:
      -Tolerance, shown by either of:
      - markedly increased intake of the substance is needed to achieve the same effect or
      -with continued use, the same amount of the substance has markedly less effect
      -Withdrawal, shown by either of:
      -the substance's characteristic withdrawal syndrome or
      -the substance (or one closely related) is used to avoid or relieve withdrawal symptoms
      -The amount or duration of use is often greater than intended.
      -The patient repeatedly tries without success to control or reduce substance use.
