Today, uncrossing the squint, a major issue for many parents and not a few adults too; for Science Week, Australian research into how a herpes virus hides from the immune system to come back and taunt us in the future; the link between organ transplantation and resisting HIV infection, and - in this week's edition of the prestigious New England Journal of Medicine, there's good news for fish eaters.
Norman Swan : Having a squint is a relatively common problem in both children and adults. The technical terms for the two commonest causes of an eye which isn't straight are strabismus and amblyopia - the lazy eye.
Last week the World President of the International Strabismological Association was in Australia. He's Emilio Campos, Professor of Opthalmology at the University of Bologna in Italy. And I caught him to find out what was new in an area which worries many parents and not a few grown-ups.
Emilio Campos : Squint means that one eye turns in a direction different from the one of the fixing eye, and the eye can turn in, out, down and up. As far as lazy eye is concerned, the lazy eye means the fact that one eye did not develop properly vision during the plastic period of the visual system. This can be due to the fact that one eye was squinting, or there was something which is called cataract. All this type of situation

Is "strabismus" the same as "lazy eye"?
We do not use the term "lazy eye" because it means different things to different people. "Strabismus" is the general term for misalignment of one or both eyes, where one eye turns in, out, down or up, compared to the other eye. Is my child's crossed eye due to a weak muscle?
Probably not. Most types of childhood strabismus (crossed or wandering eye) are due to a central problem of controlling eye position. Many types are inherited or "run in the family." Some types of strabismus are caused by a need for glasses. Rarely, a nerve paralysis can weaken a muscle and cause eye crossing. A careful examination is needed to distinguish between the types of strabismus. Do children grow out of this condition?

A 6 month old female infant presents with crossed eyes. Her parents say her eyes have been crossed since birth, and the left eye seems to cross more than the right. She seems to see fine. She plays with toys and recognizes people across the room. She was a full term infant without perinatal complications, and has no known medical problems. Clinical course: A referral is made to a pediatric ophthalmologist. No structural ocular abnormalities are found, and there is no significant refractive error. The patient is diagnosed with 1) infantile esotropia and 2) amblyopia, left eye. She wears a patch 6 hours a day over the right eye for a month, and vision in her left eye improves so that she can follow toys equally well with either eye. This also means that either eye crosses spontaneously. She then has surgery to correct her strabismus. Her medial rectus muscles are recessed at age 8 months. Postoperatively, her eyes are straight (orthophoric). Strabismus (ocular misalignment) is a common pediatric health problem, affecting approximately 5% of U.S. children. If left untreated, strabismus can lead to severe visual consequences, including poor vision and inability to use the eyes together. Strabismic amblyopia is defined as poor vision (usually in one eye, often termed "lazy eye") that results from prolonged ocular misalignment. Amblyopia can be very pronounced (sometimes able to see only hand motions in the affected eye) and is a major cause of blindness among children. Amblyopia can be prevented with timely, appropriate intervention.

What is amblyopia?

Amblyopia (commonly referred to as lazy eye) is the failure of one eye, although apparently healthy, to develop normal vision. It is treatable vision disorder if discovered and treated early, usually by age 4. What causes amblyopia?
Amblyopia is caused primarily by one of two factors: improper alignment and unequal refractive power. If the child's eyes are not properly aligned (known as strabismus), the brain will receive two different images from the eyes. Perceiving double vision, the brain won't use the image from the weaker eye. This eye, through lack of use, gradually loses its ability to function. The second primary cause of amblyopia is significantly unequal refractive power in the two eyes. For example, one eye may be very farsighted or nearsighted while the other eye may have normal vision. Again the brain receives different images from the two eyes and suppresses the image from the weaker eye. What is strabismus?
Strabismus is a term used to describe eyes that are not straight and parallel. One eye may look straight ahead, while the other turns inward, outward, upward or downward. Strabismus is a serious eye condition that requires early medical treatment if it is to be successfully corrected.
