Home Table of Issues Vol.40 No.9 Stiff-person Syndrome Associated with Cerebellar Ataxia and High Glutamic Acid Decarboxylase Antibody Titer Glutamic acid decarboxylase (GAD) is the main target of humoral autoimmunity in patients with insulin-dependent diabetes mellitus (IDDM) and stiff-person syndrome. We reviewed the case of a 46-year-old woman who had cerebellar ataxia before getting stiff-person syndrome and IDDM with high anti-GAD autoantibody titers. This was a rare case in which there were both the clinical symptoms of stiff-person syndrome and cerebellar ataxia. In western blot analysis her serum reacted with 65-kDa proteins from rat cerebellum, cerebral cortex, and spinal cord. Autoantibodies to GAD may cause functional impairment of ƒÁ-aminobutyric acid (GABA) neurons in the spinal cord as well as in the cerebellum.

Background: Stiff person syndrome is rather unique among neurologic diagnoses because of its lack of significant similarity to any other neurologic diseases. Although rare, once observed it is quite unforgettable. Possibly the closest related disease is tetanus because both conditions affect peripheral inhibition via central mechanisms and both conditions inhibit central gamma-aminobutyric acid (GABA) systems. In 1956, Moersch and Woltmann, who also coined the term stiff man syndrome, first clearly described stiff person syndrome as a neurologic clinical entity at the Mayo Clinic. The eponym for this syndrome, Moersch-Woltmann syndrome, is one of the few instances in which the eponym may be the most inclusive and at the same time most appropriately limiting name for the disease. Stiff person seems to exclude babies, and stiff man is inappropriate for children and women; perhaps stiff individual most perfectly describes the affected patient. Some confusion and controversy has also ensued as a result of cases that include other neurologic findings, such as encephalomyelitis or cerebellar deficits, in addition to the classic clinical syndrome. To further confuse matters, the most common pathologic correlate, anti–glutamic acid decarboxylase (GAD) antibodies, have been associated with a wide range of human disease, including diabetes mellitus and seizures.

The authors believe their research supports the theory that SPS results from an autoimmune response gone awry in the brain and spinal cord of the person with this ailment. SPS is an extremely rare neurological disorder characterized by progressive stiffness and spasms of certain muscles in the trunk and limbs. General Discussion Stiff-person syndrome is an extremely rare progressive neurological disorder characterized by persistent stiffness (rigidity) and spasms of certain voluntary muscles, especially those of the legs and the feet. In some cases, muscles of the neck, trunk, and shoulders may also be involved. Stiff-person syndrome may begin as recurring (intermittent) episodes of stiffness and spasms, often precipitated by surprise or minor physical contact. Definition Stiff Person Syndrome (SPS) is a rare, progressive neuromuscular condition in which a hyperactive startle reflex results in constant painful contraction of the muscles, thus causing violent spasms of the voluntary muscles, particularly the muscles of the back and upper legs. These spasms are capable of slamming the victim into walls and furniture. The disorder's cause is unknown.

These spasms are capable of slamming the victim into walls and furniture. The disorder's cause is unknown, although evidence is pointing increasingly to the fact that it is an autoimmune disorder. Stiff person syndrome is rather unique among neurologic diagnoses because of its lack of significant similarity to any other neurologic disease. Although rare, once observed it is quite unforgettable. Possibly the closest related disease is tetanus because both conditions affect peripheral inhibition via central mechanisms and both conditions inhibit central gamma-aminobutyric acid GABA ) systems.
Although they appear completely normal, those afflicted with Stiff Man Syndrome must avoid any situation where they might be exposed to sudden sounds or stimulation. Often, they remain isolated in their home and excluded from normal living - orphaned from society by this rare and ruthless disorder.

Registered: Jun 2002 posted 06-16-2002 04:38 PM I'm new to this group so I think I may not be going about this in the best way. I am curious to find out more about SS, since I've been diagnosed with it but nothing more than that has been done. My primary disorder is Stiff Person's Syndrome and my main concern is in keeping that under control. It would be interesting to hear from others with Sjogren's, though. I am curious how it affects you and what you have as treatment. I welcome any responses at knowak@execpc.com. Lovely day
