Stiff-Person Syndrome
Stiff-person syndrome is a rare progressive neurological disorder characterized by constant painful contractions and spasms of voluntary muscles, particularly the muscles of the back and upper legs. Symptoms may occur gradually, spreading from the back and legs to involve the arms and neck. Symptoms may worsen when the affected individual is anxious or exposed to sudden motion or noise. Affected muscles may become twisted and contracted, resulting in bone fractures in the most severe cases. Individuals with stiff-person syndrome may have difficulty making sudden movements and may have a stiff-legged, unsteady gait. Sleep usually suppresses frequency of contractions. Stiffness may increase and patients may develop a hunched posture (kyphosis) or a swayback (lordosis). Researchers theorize that stiff-person syndrome may be an autoimmune disorder. Other autoimmune disorders such as diabetes, pernicious anemia (a chronic, progressive blood disorder), and thyroiditis (inflammation of the thyroid gland) may occur more frequently in patients with stiff-person syndrome.

Baclofen, phenytoin, clonidine, or tizanidine may provide additional benefit. In some patients, immunomodulatory treatments such as intravenous immunoglobulin may be beneficial. General Discussion
Stiff-person syndrome is an extremely rare progressive neurological disorder characterized by persistent stiffness (rigidity) and spasms of certain voluntary muscles, especially those of the legs and the feet. In some cases, muscles of the neck, trunk, and shoulders may also be involved. Stiff-person syndrome may begin as recurring (intermittent) episodes of stiffness and spasms, often precipitated by surprise or minor physical contact. The exact cause of Stiff-person syndrome is not known.
Symptoms
Stiff-person syndrome is characterized by progressive muscular rigidity. Aching and tightness of the voluntary muscles of the body and limbs are usually the first symptoms. Profuse sweating and a rapid heartbeat (tachycardia) may accompany the muscle spasms. Muscles of the neck, trunk, shoulders, and proximal extremities may be involved on both sides of the body. During attacks of muscular spasm, contractions such as sharp bending and twitching may occur in the muscles of the hand. Extreme bending of the sole (plantar flexion) may also occur. Affected muscles may become twisted and contracted, resulting in bone fractures in the most severe cases. Persons affected by Stiff-person syndrome may have difficulty making sudden movements and exhibit a waddling gait when walking.

Stiff-person, syndrome Menu général CISMeF Définition [MeSH Scope Note ; traduction CISMeF] : Pathologie caractérisée par des spasmes persistants, impliquant de multiples muscles, principalement au niveau des membres inférieurs et du tronc. La maladie tend à se déclarer entre quarante et soixante ans, en commençant par des spasmes intermittents qui deviennent peu à peu continus. De faibles stimuli sensoriels, comme le bruit ou la lumière, peuvent déclencher des spasmes sévères. Les spasmes ne se produisent pas pendant le sommeil et impliquent rarement les muscles crâniens. La respiration peut devenir difficile dans les cas avancés.
