Stevens-Johnson syndrome is a severe and sometimes fatal form of erythema multiforme, a disorder that occurs in response to medications, illness, or infections. Erythema multiforme is believed to be an allergic reaction resulting in damage to blood vessels of the skin followed by infection of the skin tissue. More than half of the cases are caused by herpes simplex, and virtually any drug , including penicillins and barbiturates, may cause the viral infection. Herb reactions and Stevens-Johnson syndrome have also been linked. Erythema is characterized by patches of red, raised skin. In Stevens-Johnson syndrome, the systemic symptoms are more severe. Blisters break out along the mouth, throat, genital region, anus and eyes. Lesions are usually extensive and may cause acute damage of the mouth. Eating is often difficult, and closing the mouth may be painful. The eyes may swell and eventually seal shut. Urination may also become difficult.

    Dangerous Drug Eruptions Erythema Multiforme, Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis Conceptually, there is a spectrum of reactions with erythema multiforme being the mildest and toxic epidermal necrolysis the severest. Erythema multiforme (EM) is self-limited, characterized by target or iris lesion made up of a central dusky area, surrounded by an edematous pale ring and then macular erythema. This is usually associated with herpes simplex or mycoplasma infection (EM-Minor). A more severe form, Stevens-Johnson Syndrome (SJS), characteristically has impressive involvement of mucosal surfaces (oral and genital, ophthalmic) in addition to blistering cutaneous lesions which are less typical of target lesion of EM. Toxic epidermal necrolysis (TEN) is a severe form, though the exact relationship to SJS is not clear. SJS and TEN belong to the group of "Dangerous Drug Eruptions" that will be discussed. Stevens-Johnson Syndrome (SJS) Stevens-Johnson Syndrome: Case History A 35 year old HIV positive woman presents at the ER complaining of painful lesions in the mouth, and diffusely painful, blistered areas of skin over the arms. Trimethoprim- Sulfamethoxazole was begun 2 weeks prior for Pneumocystis carinii prophylaxis.

    ABSTRACT Background syndrome are rare, life-threatening, drug-induced cutaneous the risks associated with the use of specific drugs. Methods Data were obtained through surveillance networks in France, Germany, Italy, and Portugal. Drug use before the onset of disease was compared in 245 people who were hospitalized syndrome and 1147 patients hospitalized for other reasons (controls). Crude relative risks were calculated and adjusted for confounding by multivariate methods when numbers were large enough. Results Among drugs usually used for short periods, the risks sulfonamide antibiotics (crude relative risk, 172; 95 percent confidence interval, 75 to 396), chlormezanone (crude relative risk, 62; 21 to 188), aminopenicillins (multivariate relative risk, 6.7; 2.5 to 18), quinolones (multivariate relative risk, 10; 2.6 to 38), and cephalosporins (multivariate relative risk, 14; 3.2 to 59).     Physicians in France describe the case of an HIV-infected patient who developed Stevens-Johnson syndrome 13 days after beginning treatment with Ziagen (abacavir).
    The 37-year-old man was diagnosed with HIV-1 infection in 2001, Dr. Philippe Bossi, of the Pitie-Salpetriere Hospital in Paris, report in the October 1st issue of Clinical Infectious Diseases . At that time, the patient's HIV-1 RNA level was 118,000 copies/mL and his CD4 count was 1 cell per microliter. The patient began treatment with zidovudine, lamivudine and abacavir in December.
    Thirteen days later, he presented with a generalized maculopapular rash. "Physical examination revealed a disseminated cutaneous eruption of discrete dark-red macules on 90% of the body surface area, a detachment of 5% of the epidermis, genital ulcerations, erosive stomatitis, and conjunctival lesions with hyperemia, and a pseudomembranous formation occurred without keratitis or corneal erosions," the authors write.

    Physicians writing prescriptions for their patients must warn them about possible side effects. One such potential complication of drugsincluding tetracyclineis Stevens-Johnson syndrome, a potentially fatal condition that manifests mainly on the skin and mucosal surfaces but also affects other vital organs. Many types of therapy have proved efficacious for treating the syndrome, but use of steroid agents for this purpose remains controversial. Care for patients with Stevens-Johnson syndrome consists of treating the presenting symptoms. Introduction Erythematous rash Stevens-Johnson syndrome, otherwise known as erythema multiforme majus, is thought to represent a continuum of disease, the most benign type of which is erythema multiforme, whereas toxic epidural necrolysis is the most severe. The condition was first described in 1922 by Stevens and Johnson as a febrile illness with stomatitis, purulent conjunctivitis, and skin lesions.
